Injuries of the internal thoracic and intercostal arteries are observed rather frequently in penetrating wounds of the thorax. In intrapleural hemorrhage the only correct tactics is the urgent surgical intervention. Its efficiency is dependent on the terms of admittance of the patient to a specialized clinic, preparedness of different services (reanimation, x-ray analysis, operation room) for urgent help, on qualification of surgeons and careful management during the postoperative period.

The operation of choice in complicated trauma of the vertebral column with compression of the spinal cord (especially in the thoracic and the thoracicolumbar segments) is laminectomy (as the first stage of the intervention) performed in the earliest period. It is obligatory to conduct extensive inspection of the spinal cord and its roots, perform its anterior and posterior decompression and subsequent reliable stabilization of the spine, which may prevent recurrent compressions of the spinal cord in the residual period.

Thoracic trauma is a frequent injury pattern with high patient morbidity and mortality. Preclinical and clinical emergency treatment is consented in a national S3-guideline. Following emergency therapy one third of patients may develop lung lacerations, pleural fistulation and persisting pneumothorax. An interdisciplinary working group of the German Society for Thoracic Surgery and the German Society for Traumatology reviewed the published medical literature on treatment of those injuries and assessed the existing evidence according to consensus recommendations. An inconsistent classification

BACKGROUND: Overall, spinal injuries in winter sports are extremely rare. The incidence is given as approximately 0.01/1000 ski days. While falls and collisions at high speed are the main concerns for skiers, spinal injuries for snowboarders occur primarily on landing after a jump. The age of a typical spinally injured skier is 40, which is older than that of the average snowboarder at 23.
CLASSIFICATION: Primarily, the thoracolumbar junction is injured and hereby mainly anterior compression fractures (type A1) and burst fractures (types A3 and A4) occur. Injuries caused by flexion or distrac

BACKGROUND: Obtaining a suitable proximal landing zone in blunt thoracic aortic injuries (BTAI) often necessitates coverage of the left subclavian artery (LSA). This study aimed to evaluate the outcomes of physician-modified endovascular grafts (PMEGs) in this indication.
METHODS: We performed a retrospective analysis of a prospective registry including all patients who had undergone a thoracic endovascular aortic repair (TEVAR) for BTAI from October 2008 to October 2020. Starting in 2015, patients requiring coverage of the LSA were either treated using a physician-modified proximal scalloped

Spinal cord injury (SCI) leads to dramatic impairments of motor, sensory, and autonomic functions of affected individuals. Following the primary injury, there is an increased release of glutamate that leads to excitotoxicity and further neuronal death. Therefore, modulating glutamate excitotoxicity seems to be a promising target to promote neuroprotection during the acute phase of the injury. In this study, we evaluated the therapeutic effect of a FDA approved antiepileptic drug (levetiracetam-LEV), known for binding to the synaptic vesicle protein SV2A in the brain and spinal cord. LEV therap

BACKGROUND: While age is a known risk factor in trauma, markers of frailty are growing in their use in the critically ill. Frailty markers may reflect underlying strength and function more than chronologic age, as many modern elderly patients are quite active. However, the optimal markers of frailty are unknown.
METHODS: A retrospective review of The Crash Injury Research and Engineering Network (CIREN) database was performed over an 11-year period. Computed tomographic images were analyzed for multiple frailty markers, including sarcopenia determined by psoas muscle area, osteopenia determin

BACKGROUND CONTEXT: Motor vehicle collisions (MVC) are a leading cause of thoracic and lumbar (T and L) spine injuries. Mechanisms of injury in vehicular crashes that result in thoracic and lumbar fractures and the spectrum of injury in these occupants have not been extensively studied in the literature.
PURPOSE: The objective was to investigate the patterns of T and L spine injuries after MVC; correlate these patterns with restraint use, crash characteristics, and demographic variables; and study the associations of these injuries with general injury morbidity and fatality.

Lesions involving the descending thoracic aorta apart from isthmus are rare and less known by anesthetists. We report the clinical course of two severely injured patients who sustained a thoracic aortic rupture in whom favorable outcome was achieved with endovascular treatment. Mechanisms, diagnosis and therapeutics aspects of these rare lesions are discussed according to literature.

Tracheobronchial injuries in children occur rarely. Their diagnosis is often very difficult and needs a high degree of suspicion, with in-depth knowledge of the anatomy of and radiological findings for the chest. With proper surgical management, even a delayed diagnosis can result in normal lung function. We discuss 2 cases of major airway injuries with successful outcomes and present some interesting surgical maneuvers.

BACKGROUND: Traumatic injury to the aorta and great vessels is a surgical emergency with survivors who reach hospital typically having suffered multiple injuries. There are several diagnostic and treatment options available, with new modalities emerging to challenge the gold standards. A review of recent trends in management of these injuries in Auckland, New Zealand was carried out and patient outcomes assessed.
METHODS: The charts of patients admitted to Auckland and Green Lane Hospital's cardiothoracic intensive care unit, with a diagnosis of injury to the thoracic aorta or great vessels s

INTENTION: Thorax trauma (TT) is associated with a high rate of pulmonary failure and increased mortality. To prevent these complications, the German trauma system recommends intubation and chest tube insertion at the scene of the accident, even in cases without acute respiratory dysfunction. Due to the possible life threatening complications of the therapy, the emergency surgeon should be able to correctly identify a TT at the scene. Therefore, we retrospectively compared the evaluation of chest trauma by the emergency surgeon with objective injury severity according to the Abbreviated Injury

The pathology of intervertebral disc injuries, hyper-extension injuries and spinal cord injuries due to fracture-dislocations of the cervical spine has been reported previously. Between 1957 and 1987, autopsy was performed on 4 patients with fracture-dislocation of the thoracic spine and on one patient with that of the lumbar spine. This paper presents the results of a clinicopathologic study conducted on two patients with a fracture-dislocation of the thoracic spine and on one patient with that pathology of the lumbar spine. The methods employed in the present study are the same as those that

INTRODUCTION: A consensus exists among vascular surgery societies that blunt thoracic aortic injury (BTAI) should be treated with thoracic endovascular aortic repair (TEVAR) in patients with favorable aortic anatomy. However, selecting the correct landing zones and endograft sizing remains challenging in patients with BTAI. To overcome these limitations, conformable endografts are often used for TEVAR, even in patients with BTAI. This study aimed to analyze the outcomes of these patients with respect to anatomical and technical factors.
METHODS: This retrospective study included all patients
