Early outcomes of video-assisted thoracic surgery (VATS) Ivor Lewis operation for esophageal squamous cell carcinoma: the extracorporeal anastomosis technique.

PURPOSE: Although the use of a minimally invasive approach in esophageal cancer surgery is gradually increasing, it is generally performed using cervical anastomosis because of the difficulty of intrathoracic anastomosis. Here, we describe our technique for performing intrathoracic esophagogastrostomy using a typical video-assisted thoracic surgery (VATS) approach. METHODS: Between September 2009 and July 2011, VATS esophagectomy and intrathoracic anastomosis was performed in 31 esophageal cancer patients with a utility incision made by a segmental rib resection to enhance the extracorporeal

Pathological diagnosis and cytoreduction of cardiophrenic lymph node and pleural metastasis in ovarian cancer patients using video-assisted thoracic surgery.

BACKGROUND: The aim of this study was to assess the benefit of video-assisted thoracic surgery (VATS) in pathological diagnosis and intrathoracic cytoreduction of cardiophrenic lymph node (CPLN) and pleural metastasis on computed tomography (CT) in patients with ovarian cancer. METHODS: We reviewed a database of ovarian cancer patients who underwent VATS from June 2007 to November 2008. The criterion for suspicion of malignancy on CT was either a CPLN with short-axis dimension >5 mm or multinodular pleural thickening. Seven patients with primary ovarian cancer and five patients with recurrent

Indocyanine green-guided video-assisted thoracoscopic surgery thoracic duct ligation and talc pleurodesis for refractory chylothorax after Ivor Lewis oesophagectomy.

Refractory chylothorax following oesophagectomy is a challenging complication, often requiring surgical re-intervention. We describe a standardized technique for thoracic duct ligation using indocyanine green (ICG) fluorescence guidance. A 62-year-old male presented with high-output chylothorax 40 days after a minimally invasive Ivor Lewis oesophagectomy, refractory to conservative management. The procedure involved pre-operative bilateral inguinal lymph node injection of ICG. A right-sided three-port VATS approach was utilized. Intra-operatively, a provocative fatty meal test (cream and olive

Comparative short-term outcomes of robotic-assisted vs video-assisted thoracic surgery in lung cancer: a multicenter retrospective study from EPITHOR with a quality audit.

Advancements in diagnostic imaging and surgical techniques have significantly evolved the treatment landscape of non-small cell lung cancer (NSCLC). The shift toward parenchymal-sparing approaches, such as segmentectomy for cT1a-bN0 tumors, is challenging the traditional lobectomy. This retrospective multicenter cohort study evaluates short-term outcomes of Video-Assisted Thoracoscopic Surgery (VATS) and Robotic-Assisted Thoracic Surgery (RATS) in NSCLC patients using data from the French EPITHOR registry, enhanced by an in-depth quality audit. The audit ensured the completeness and accuracy o

Uniportal video-assisted thoracoscopic surgery for retained shrapnel in the thoracic cavity: An update surgical approach for removal of foreign body.

Uniportal video-assisted thoracoscopic surgery commonly used for removal of bullet, tumours and foreign bodies in the chest cavity. However, shrapnel fragments related war can injure many organs and life-threatening condition. Furthermore, standard treatment for removal foreign bodies retained in the chest is conventional thoracotomy. Alternatively, single port video-assisted thoracoscopic surgery approach can be used in patients hemodynamically stable. In addition, retained foreign bodies after a penetrating thoracic injury remain challenge for u-VATS surgeons. Here we present A 35-year-old m

Effective application of indocyanine green for visualization of the thoracic duct during video-assisted thoracoscopic surgery for chylothorax: a case report.

BACKGROUND: Chylothorax is an accumulation of typically milky-appearing lymphatic fluid within the pleural cavity. High-output chylothorax may lead to a severe risk of death if not treated promptly. Several studies have reported the effectiveness of indocyanine green (ICG) for chylothorax following lung cancer surgery, esophagectomy, and congenital and recurrent idiopathic chylothorax. Here, we report a case of a successful treatment of idiopathic chylothorax in an adult by identifying and ligating the thoracic duct (TD) using fluorescent thoracoscopy with inguinal lymph node injection of ICG.

Devising the guidelines: the techniques of pulmonary nodule localization in uniportal video-assisted thoracic surgery-hybrid operating room in the future.

Pulmonary nodules beneath the pleura can be hard to visualize or palpate, especially during the uniportal thoracoscopic surgery. Conventionally, thoracic surgeons would use adjuvant modalities to localize the lesion preoperatively, of which computed tomography-guided hookwire implantation has been adopted most widely due to its feasibility and high success rate. However, procedure-associated complications such as pneumothorax and wire dislodgement can cause patient discomfort or localization failure. Occasionally more healthy tissue is resected than needed to guarantee the lesion is removed an

Continuous erector spinae plane block versus thoracic epidural analgesia in video-assisted thoracic surgery: a study protocol for a prospective randomized open label non-inferiority trial.

BACKGROUND: Thoracic epidural analgesia is considered the gold standard for pain relief in video-assisted thoracoscopic surgery. This neuraxial technique blocks pain sensation by injecting a local anesthetic agent in the epidural space near the spinal cord to block spinal nerve roots. Recently, the erector spinae plane block has been introduced as a practical alternative to the thoracic epidural. This interfascial regional anesthesia technique interrupts pain sensation by injecting a local anesthetic agent in between the muscular layers of the thoracic wall. Several case series and three RCTs
