Molecular Pathology of Thyroid Tumors: Old Problems and New Concepts.

The molecular signatures of many thyroid tumors have been uncovered. These discoveries have translated into clinical practice and are changing diagnostic and tumor classification paradigms. Here, the findings of recent studies are presented with special emphasis on how molecular insights are impacting the understating of RAS mutant thyroid nodules, Hürthel cell neoplasms, and unusual thyroid tumors, such as hyalinizing trabecular tumor, secretory carcinoma of the thyroid, and sclerosing mucoepidermoid carcinoma with eosinophilia. In addition, the utility of detecting actionable molecular alter

Selected updates in thyroid, parathyroid, and adrenal gland pathology.

Endocrine pathology is continuously advancing with with new classification systems and an increased understanding the underlying pathogenesis and genetic alterations, whether somatic or germline, in endocrine diseases. Many changes have recently occurred in the classification of thyroid carcinoma, the most complex of which involve the new category of "high-grade follicular cell derived non anaplastic thyroid carcinoma" which encompasses both poorly differentiated thyroid carcinomas and differentiated high-grade thyroid carcinomas. The word "hyperplasia" is no longer used with primary multiglan

The Spectrum of Thyroid Gland Pathology in Carney Complex: The Importance of Follicular Carcinoma.

The initial description of Carney complex (CNC) in 1985 included myxomas, spotty skin pigmentation, and endocrine overactivity (of the adrenal, the pituitary, and the testis). In 1997, thyroid neoplasms were found in 3 patients with CNC and involvement of the gland in the syndrome was apparent. Herein, we describe the clinical, pathologic, and follow-up findings in 26 patients with CNC and a disorder of the thyroid gland. The patients were predominantly middle-aged women with an asymptomatic thyroid mass. Four patients had hyperthyroidism, which was caused by follicular hyperplasia in 2 patien

Improving prediction of malignancy of cytologically indeterminate thyroid nodules.

BACKGROUND: Fine-needle aspiration (FNA) is essential in the investigation of thyroid nodules. The British Thyroid Association guidelines recommend clarification of whether follicular nodules are probable follicular neoplasms that require surgical excision. This study assessed the value of the subclassification of cytologically indeterminate thyroid nodules into either follicular neoplasms or other pathology. METHODS: The cytology reports of all thyroid FNAs performed between November 2005 and December 2007 at a single institution reported as Thy 3 (follicular lesions) were reviewed. They wer

Primary solitary fibrous tumor of the thyroid gland: A review starting from a case report.

Primary solitary fibrous tumor (SFT) of the thyroid gland is a rare mesenchymal tumor with fibroblastic differentiation, ramified, thin-walled, enlarged (staghorn) vessels and specific NAB2-STAT6 gene fusion, which is more commonly found in pleura and peritoneum. This neoplasm can be located in a variety of anatomical sites outside pleura and peritoneum including bone, visceral organs and soft tissues, head and neck examples representing only 10-15% of the extra-pleural and extra-peritoneal tumors. Diagnosing this entity can be difficult, especially in thyroid gland, mainly because of the rari

Pink-on-pink: hepatocellular carcinoma metastatic to oncocytic carcinoma of the thyroid.

Hepatocellular carcinoma typically metastasizes within the liver and may involve extrahepatic sites such as the lungs, adrenal glands, and bones at advanced stages. However, hepatocellular carcinoma metastasis to the thyroid is very uncommon and tumor-to-tumor metastasis from a hepatocellular cancer to a thyroid neoplasm is extremely rare. In this report, we present a case of a 70-year-old man with a hepatocellular carcinoma metastasizing to oncocytic thyroid carcinoma, emphasizing the importance of clinical history and of a multidisciplinary approach, as well as the usefulness of site-specifi

Hyalinizing trabecular tumor: Cytologic, histologic and molecular features and diagnostic considerations.

Hyalinizing trabecular tumors are a follicular origin neoplasm of the thyroid that usually present as an asymptomatic, well circumscribed, solitary mass. However, diagnosis of a hyalinizing trabecular tumor may be challenging especially on fine needle aspiration cytology and requires careful examination of the specimen to rule out potential mimickers such as papillary thyroid carcinoma, medullary thyroid carcinoma, paraganglioma, other follicular patterned neoplasms, intrathyroidal parathyroid tissue, and metastatic disease. We will review the cytologic, histologic and molecular features of hy

[Thyroid actinomycosis: a case report].

Actinomycosis is nowadays a not very common festered chronic infection. Cervicofacial actinomycosis is the most frequent clinical form, and it is due in most cases to Actinomyces israelii. Thyroid actinomycosis has been scarcely described in the literature. The lesions, that are characterized to be notably indurated and to contain multiple abscesses that drain to the surface through sinus tracts, frequently simulate other infective clinical entities and often neoplasms, therefore the diagnosis is delayed in the time. From a case of thyroid actinomycosis diagnosed and treated in our service whi

Comparative analysis through propensity score matching in thyroid cancer: unveiling the impact of multiple malignancies.

BACKGROUND: The incidence of thyroid cancer is on the rise worldwide, with childhood exposure to radiation being the sole acknowledged catalyst for its emergence. Nonetheless, numerous other factors that may pose risks are awaiting thorough examination and validation. This retrospective study aims to explore the malignancies linked to thyroid cancer and contrast the survival rates of those afflicted with a solitary tumor versus those with multiple primary neoplasms (MPN). METHODS: This retrospective study examined data from King Hussein Cancer Center (KHCC), Jordan. Among 563 patients diagnos

Gestational diabetes: a contributor to long-term thyroid dysfunction and disease.

BACKGROUND: Emerging evidence links gestational diabetes mellitus (GDM) with thyroid dysfunction, but the long-term risk of clinically diagnosed thyroid diseases after GDM remains unclear. OBJECTIVE: To assess the long-term risk of specific thyroid disorders in women with prior GDM compared with those without GDM. METHODS: We performed a retrospective cohort study using the TriNetX U.S. Collaborative Network, including de-identified records from >80 healthcare organizations. Women aged 16-45 years with pregnancies from 2001 to 2015 were eligible; those with pre-existing hypertension, diabete

Cross-sectional imaging of the thyroid gland.

Directed imaging is useful in assessing the thyroid gland. Nuclear scintigraphy reveals functional information about the thyroid gland, while cross-sectional imaging, including ultrasound, CT, and MR imaging provide important adjunctive anatomic information about the thyroid as well as about related structures in the neck, including the presence or absence of cervical and mediastinal lymphadenopathy, or extension of thyroid disease into adjacent soft tissues or the mediastinum. This article reviews the anatomy and physiology of the thyroid gland and addresses issues related to diseases affecti

Radioactive iodine-associated cytomorphologic alterations in thyroid follicular epithelium: is recognition possible in fine-needle aspiration specimens?

Editor's Note: There is a dilemma in the distinction between radiation effect and neoplasm in patients with thyroid nodules who have been previously treated with (131)I for Graves' disease. The difficulty is to identify the radiation changes and distinguish them from primary thyroid neoplasms. This paper addresses the time response of radiation therapy in the thyroid, as well as distinguishing features between radiation effect and primary carcinomas. Thyrotoxicosis is a condition resulting from presentation to the body's tissues of excessive quantities of thyroid hormones. Graves' disease is t

Estrogen receptor in normal and neoplastic human thyroid tissue.

Clinical evidence suggests the possible influence of estrogen on the biologic behavior of thyroid neoplasm. In this study we evaluated the distribution and characteristics of intracellular receptors for estrogen in normal and neoplastic thyroid tissue obtained from 54 patients. Forty-two percent of all specimens assayed by the protamine sulfate precipitation technique had a detectable intracellular binding site for estrogen. Differentiated carcinomas (eight of eight) and adenomas (seven of nine) had high incidences of receptors compared with goiter (five of 23) and normal thyroid (three of eig

Clinicopathological and molecular characterization of nine cases of columnar cell variant of papillary thyroid carcinoma.

The majority of papillary thyroid carcinoma is indolent and associated with long-term survival. The columnar cell variant, however, is a rare subtype that is variable in biological behavior; some are clinically aggressive, whereas others are more clinically indolent. Tumor size, tumor circumscription, and encapsulation may influence the behavior of columnar cell carcinomas. Other variables including genetic changes and putative biomarkers associated with malignant growth have not been thoroughly examined in these neoplasms. In this study, nine cases of columnar cell variant of papillary thyroi

[Thyroid cancer in a child with Cowden syndrome].

Cowden disease (Cowden syndrome) refers to PTEN-associated hamartoma tumor syndromes. It arises due to a mutation in the phosphatase and tensin homolog gene, one of the main functions of which is cell cycle regulation. The presence of a mutation in the gene leads to uncontrolled cell growth, and patients have a lifelong increased risk of neoplasms of various degrees of malignancy. This article presents a clinical case of Cowden syndrome with an early debut at the age of 7 years. The combination of macrocephaly (SDS of head circumference >2) with various skin manifestations (facial trichilem

Potential variant of multiple endocrine neoplasia in a dog.

This report describes multiple endocrine neoplasia in a dog, which is a rare hereditary disorder characterized by the presence of two or more neoplasms of different endocrine tissues within a patient. A 14 yr old dog was evaluated for polyuria/polydipsia, polyphagia, and abdominal enlargement. Adrenal-dependent hyperadrenocorticism with concomitant left thyroid enlargement and a presumed abdominal metastatic lesion were diagnosed by an adrenocorticotropic hormone stimulation test, ultrasonography, and computed tomography. Trilostane therapy was initiated and resolved the clinical signs for 2 y

[Hereditary Sipple syndrome (author's transl)].

This entity, isolated by Gorlin, is characterized by the presence of cutaneous and mucous neuromas, facial disfiguration resembling acromegalia as well as morphological changes to the extremities similar to those produced by Marfan's disease. These factors are also associated with multiple endocrinological neoplasms type II of Sipple's syndrome. In other words, malignant tumors developing from thyroid "C" cells as well as from bilateral benign pheochromocytoma (neoplasias which derive from the A.P.U.D. system). It is considered to be hereditary in nature and transmitted as a dominant trait. It
