Outbreak of African tick-bite fever in six Italian tourists returning from South Africa.

In May 1999, a cluster of cases of African tick-bite fever was detected in six Italian tourists who had returned from South Africa. All of the patients had moderate fever and cutaneous eschars. Regional lymphangitis was observed in three of the patients and skin rash in two. By comparing the number of eschars with the number of detectable bite sites it was suggested that at least two-thirds of the biting vectors were capable of transmitting Rickettsia africae. The clinical course of disease was mild in all cases, and all but one of the patients recovered spontaneously before antibiotic treatme

Molecular detection of Rickettsia helvetica in lesion and blood samples after tick bites in Serbia: two cases with One Health implications.

Rickettsia helvetica is a spotted fever group rickettsia associated mainly with Ixodes ricinus and increasingly detected in European ticks. We describe two tick-bite-associated human cases in Serbia in May 2025. A 65-year-old woman (Case 1) developed pruritic erythematous lesions after two adult female I. ricinus attachments acquired in the Valjevo area. Rickettsia spp. DNA was detected in capillary blood from both lesions, and sequencing of the PCR amplicons identified R. helvetica, while whole blood was PCR-negative and serology did not show reproducible seroconversion. An 8-year-old girl (C

Occurrence of the bat tick Ornithodoros (Alectorobius) Kelleyi Cooley & Kohls (Acari: Argasidae) in Costa Rica and its relation to human bites.

Several persons from two localities in Costa Rica (San Rafael de Coronado, 1510 m and Dulce Nombre de Tres Ríos, 1445 m) were bitten by ticks, subsequently identified as Ornitohodoros (A.) Kelleyi (Argasidae), commonly associated with bats. In the attics of their homes, live adults, larvae, nymphs and skins were found in the bat guano. Since there are no records of adult argasids in this country, a laboratory colony was established to observe the biology of these ticks and their association with bats. In this particular case, the bats were Molossidae and Vespertilionidae, both insectivorous. A

Evidence assessments and guideline recommendations in Lyme disease: the clinical management of known tick bites, erythema migrans rashes and persistent disease.

Evidence-based guidelines for the management of patients with Lyme disease were developed by the International Lyme and Associated Diseases Society (ILADS). The guidelines address three clinical questions - the usefulness of antibiotic prophylaxis for known tick bites, the effectiveness of erythema migrans treatment and the role of antibiotic retreatment in patients with persistent manifestations of Lyme disease. Healthcare providers who evaluate and manage patients with Lyme disease are the intended users of the new ILADS guidelines, which replace those issued in 2004 (Exp Rev Anti-infect The…

Longitudinal evaluation of an educational intervention for preventing tick bites in an area with endemic lyme disease in Baltimore County, Maryland.

The authors attempted to determine whether a targeted educational intervention in an area with endemic Lyme disease could increase knowledge, positive attitudes, and reported behaviors related to tick bite prevention and consequently decrease tick bites, as measured by a biomarker of tick bites. Between April and September of 1999, 317 subjects in Baltimore County, Maryland, were randomized to receive either tick-related or general health-related educational materials bimonthly through the mail. At each of three clinic visits, participants completed a self-administered questionnaire and provid…

Using risk group profiles as a lightweight qualitative approach for intervention development: an example of prevention of tick bites and lyme disease.

BACKGROUND: Many public health campaigns use a one-size-fits-all strategy to achieve their desired effect. Public health campaigns for tick bites and Lyme disease (LD) in many countries convey all relevant preventive measures to all members of the public. Although preventing tick bites (eg, by wearing protective clothing or using repellants) and checking for tick bites after visiting a risk area are effective and cost-efficient methods to prevent an individual from contracting a tick-borne disease, public compliance to these methods is low. OBJECTIVE: We aimed to identify the group of individ…

IgE-mediated anaphylaxis caused by bites of the pigeon tick Argas reflexus: cloning and expression of the major allergen Arg r 1.

BACKGROUND: Anaphylactic reactions caused by bites of the European pigeon tick Argas reflexus are repeatedly reported. This soft-backed tick … immunoblots were performed with sera from 13 patients who had an anaphylactic reaction with pigeon tick bites. A cDNA expression library was constructed from whole ticks and screened with a polyclonal rabbit antiserum raised against the major allergen. RESULTS: The cDNA coding for the dominant allergen Arg r 1 could be isolated. It encodes a protein belong …

Tick bites, clinical symptoms of Lyme borreliosis, and Borrelia antibody responses in Finnish army recruits training in an endemic region during summer.

Tick bites, associated clinical symptoms, and antibodies against Borrelia burgdorferi were investigated in 77 Finnish army recruits training during summer in an endemic region and 50 control recruits serving outside the habitat of ticks. During a follow-up of 6 months, 26.9% of the study recruits reported tick bites. None gave a history of erythema migrans. Five study recruits and none in the control group had a combination of two or three nonspecific symptoms compatible with Lyme borreliosis. Three of these five study recruits had been bitten by several ticks. Thirteen (16.9%) study recruits …

Beyond the eschar: African tick-bite fever in a returning traveller - a diagnostic and public health consideration in Europe.

BACKGROUND: African tick-bite fever (ATBF), caused by Rickettsia africae, is the most common tick-borne rickettsiosis among travellers returning from sub-Saharan Africa. In non-endemic regions, its relevance lies mainly in diagnostic challenges in returning travellers and the need for continued vigilance in vector-borne disease surveillance. CASE PRESENTATION: report a 48-year-old woman presenting with fever, headache, myalgia, abdominal pain, and multiple eschars after a 9-day trip to South Africa. Clinical examination revealed fever (38.5 °C), tender inguinal lymphadenopathy, and a maculopa

Scalp eschar and neck lymph adenopathy after a tick bite (SENLAT) in Tuscany, Italy (2015-2022).

INTRODUCTION: The Scalp Eschar and Neck Lymph Adenopathy After a Tick Bite (SENLAT) syndrome is frequently caused by Rickettsia slovaca and Rickettsia raoultii. Only six microbiologically confirmed SENLAT cases have been reported in Italy between 1996 and 2021. We report ten cases of SENLAT seen between 2015 and 2022 in a tertiary care center in Tuscany, Italy. CASES PRESENTATION: All patients were women; most common symptoms were scalp eschar on the site of tick bite (100%) and cervical lymphadenopathy (90%). No microbiological identification was obtained. Persistent alopecia, for several mo

A comprehensive clinical and laboratory evaluation of 224 patients with persistent symptoms attributed to presumed tick-bite exposure.

BACKGROUND: Persistent symptoms attributed to presumed tick-bite exposure constitute an unresolved medical controversy. We evaluated whether … oms were fatigue-related (70%), musculoskeletal (79%), neurological (82%) and neurocognitive (57%). Tick bites were recalled by 74%. The RAND-36 score was significantly below that of the general Swedish population. Signs of immunological/inflammatory reactivity with myositis antibodies were detected in 20% of patients, fibrinogen levels were moderately incre …

Tick bite frequency, prevention practices and Lyme disease diagnoses among U.S. Hispanic survey respondents.

Tick bite prevention practices, knowledge of Lyme disease (LD) symptoms and transmission, and patterns of LD diagnoses among Hispanic persons have been reported but not comprehensively evaluated. In 2014, CDC examined questions from a prospective nationwide survey of U.S. Hispanic adults conducted via the Offerwise QueOpinas panel regarding ticks and LD. From October to November, a total of 2,649 surveys were released and 1,006 completed surveys returned. Overall, 44% of respondents reported routinely practising at least one form of personal protection against tick bites, and wearing repellent…

First identification of Anaplasma phagocytophilum in both a biting tick Ixodes nipponensis and a patient in Korea: a case report.

BACKGROUND: Human granulocytic anaplasmosis (HGA) is a tick-borne infectious disease caused by Anaplasma phagocytophilum. To date, there have been no reported cases of A. phagocytophilum infection found in both the biting tick and the patient following a tick bite. CASE PRESENTATION: An 81-year-old woman presented with fever following a tick bite, with the tick still intact on her body. The patient was diagnosed with HGA. The tick was identified as Ixodes nipponensis by morphological and molecular biological detection methods targeting the 16S rRNA gene. The patient's blood was cultured after

Cutaneous leishmaniasis and rickettsial African tick-bite fever: a combination of exotic traveler's diseases in the same patient.

BACKGROUND: Cutaneous leishmaniasis and rickettsial African tick-bite fever are two zoonoses increasingly diagnosed in industrialized nations due to more international travel to endemic areas. METHODS: A 52-year-old American nurse was evaluated for a 0.5 cm well-demarcated, tender, shallow ulcer on her wrist, nonproductive cough, fever, chills, and night sweats, all of which began three weeks after travel to Botswana and a visit to a game reserve, where she reported being scratched on the ankle by a cheetah. RESULTS: This cutaneous finding was strongly suggestive of leishmaniasis, but the sy

Unusual location of an inoculation lesion in a traveler with African tick-bite fever returning from South Africa.

African tick-bite fever is a common tick-borne rickettsiosis in sub-Saharan Africa. It is an acute febrile illness associated with one (or more) inoculation eschar, an inconstant eruption, and local lymphadenopathies. We describe the first case of mucosal inoculation lesion on the vulva in a female traveler returning from South Africa.
