PURPOSE: To study the safety and efficacy of intracameral lidocaine as anaesthesia in trabeculectomy for primary open-angle glaucoma.
METHODS: Trabeculectomy under intracameral anaesthesia using 1% preservative-free lidocaine was performed in 10 patients with primary open-angle glaucoma. Intraoperative pain score, postoperative intraocular pressure (IOP) and endothelial cell count at 3 months were recorded.

PURPOSE: To report supraciliochoroidal effusion after trabeculectomy with the use of ultrasound biomicroscopy.
METHODS: In a prospective study, 28 eyes of 19 patients with primary open-angle glaucoma that underwent trabeculectomy were evaluated preoperatively and postoperatively by ultrasound biomicroscopy.
RESULTS: Four eyes showed a hypoechogenic suprachoroidal space that remained stable for 6 months postoperatively. These four eyes had intraocular pressures of 11 mm Hg or less on no antiglaucoma medications and without signs of choroidal detachment.

A clinical audit was conducted for a 4-year period at the Universiti Kebangsaan Malaysia (UKM) Ophthalmology Department in which 61 eyes of adult patients with primary glaucoma underwent trabeculectomies without antimetabolites. A 45-year-old male underwent a trabeculectomy with mitomycin C for a secondary glaucoma after complicated cataract surgery. Because of early bleb failure a needling procedure with subconjunctival injection of mitomycin C was performed. Five days postoperatively the patient developed an occlusion of both the arterial and venous retinal vasculature. A relation between the diffusion of mitomycin C into the vitreous cavity and the development of retinal toxicity is suggested.

The relationship between ciliochoroidal detachment on the late hypotensive results of trabeculectomy was studied in 64 patients with primary open-angle glaucoma. Seventy patients, matched for their initial condition and age, in whom the postoperative period ran without complications, made up the reference group. The mean follow-up period was 24.2 months. Ciliochoroidal detachment was found to have virtually no effect on the late hypotensive results of trabeculectomy, though such detachment had a negative effect on the lens transparency.

The authors report the results obtained in 74 cases, or 105 eyes, with congenital glaucoma operated on by trabeculectomy. Three different trabeculectomy procedures were used: closing the scleral flap with two sutures; placing a silastic passing in bridge under the flap; and tightly suturing the scleral flap.
