BACKGROUND: Evidence regarding optimal management of the "Cannot Intubate, Cannot Oxygenate" (CICO) scenario in infants is scarce. When inserting a transtracheal cannula for front of neck access direct aspiration to confirm intratracheal location is standard practice. This postmortem "infant airway" animal model study describes a novel technique for cannula tracheotomy.
AIMS: To compare a novel technique of cannula tracheotomy to an accepted technique to assess success and complication rates.
METHODS: Two experienced proceduralists repeatedly performed tracheotomy using an 18-gauge BD Insyte

BACKGROUND: The study examines the suitability of the Fantoni method of translaryngeal tracheotomy (TLT) for airway management after surgery due to oropharyngeal and maxillofacial tumours.
PATIENTS AND METHODS: During a 4-year period, 156 translaryngeal pull-through tracheotomies were performed in 145 patients. This method is the only puncture tracheotomy technique that involves a dilatation process from inside the trachea to the outside through the skin and differs from other established puncture methods regarding practicability and frequency of complications.
RESULTS: The mean puncture tim

OBJECTIVE: Patients with acute respiratory failure due to COVID-19 have a high likelihood of needing prolonged intubation and may subsequently require tracheotomy. Usually, the choice of technique (percutaneous dilatational tracheotomy [PDT] versus open surgical tracheotomy [OST]) depends on the preference of surgeons and patient-related factors. In case of COVID-19, airborne spread of viral particles and limited time of apnea must be considered in the choice of the safest technique. The aim of this study is to compare the complication rates and offer an assessment of relative risks and benefi

This case study was undertaken to describe the use and limitations of glossopharyngeal breathing (GPB) by a ventilator-dependent, tracheotomized patient after cervical tumor resection. A 6-yr, 8-mo-old, tracheotomized, ventilator-dependent boy, after cervical tumor resection, learned GPB on his own and used it for ventilator-free breathing. Over the next 16 yrs, his GPB efficacy improved to the point that, with a vital capacity of 670 ml, his GPB maximum single-breath capacity increased to 3300 ml. This was limited by the fact that at 2.9 l of lung volume, air began to leak around the tracheos

IMPORTANCE: Tracheotomy is sometimes performed in extremely low gestational age newborns requiring prolonged ventilation. Studies suggest better neurodevelopmental outcomes in preterm newborns undergoing earlier tracheotomy (<120 days); however, guidelines for who should undergo tracheotomy and when to perform tracheotomy are unclear regarding infants receiving long-term positive-pressure support.
OBJECTIVE: To determine the characteristics associated with tracheotomy in high-risk, extremely low gestational age newborns.
DESIGN, SETTING, AND PARTICIPANTS: This secondary analysis of infants e

The purpose of this study was to evaluate the accurate identification of the cricothyroid membrane (CTM) by fifth grade dental students, before undergoing the relevant anesthesiology practicum. Moreover, we aimed to determine the educational effectiveness of the cricothyrotomy practicum in anesthesiology. Before the lecture and without prior notification, 119 students were provided with a photograph of a man's neck and instructed to attach a blue sticker to the cricothyrotomy puncture site and to a palpable tracheotomy trainer, after applying sufficient palpation. After this, students attended

BACKGROUND AND AIM OF THE STUDY: Therapeutic education helps patients with a chronic disease to acquire and maintain the ability to live their life while handling their illness. Patients with temporary medium-term tracheotomy or permanent tracheostomy need to acquire skills to be able to handle the stoma, tracheal tube, related issues, and other apparatuses. This was the purpose of our therapeutic education program, which was aimed to take patients and caregiver to an efficient level of self-care.
METHODS: In 2018, was created a CME-accredited (Continuing Medical Education) "Workplace-based L

BACKGROUND: Percutaneous dilatational tracheotomy (PT) is safe and cost effective, and has become a routine method in intensive care units (ICU), but safety concerns persist for obese patients and for patients with a high risk of bleeding. In this prospective study of 1000 PTs, we have investigated whether such patient characteristics were associated with an increased procedural risk.
METHODS: We prospectively recorded all PTs performed in our ICU from 2001 to 2009. Data on blood transfusion were entered from a central database. The association of risk factors with bleeding and other complica

OBJECTIVE: The aim of this study was to analyze whether patients with SARS-CoV-2 who received surgical tracheotomy had a lower incidence of postoperative bleeding if their LMWH was postponed or canceled on the day of surgery.
METHODS: Patients with SARS-CoV-2 admitted to the intensive care units who underwent surgical tracheotomy were assessed retrospectively through their medical records. Data on comorbidity, LMWH dose, and timing were collected. Bleedings < 72 h post surgery were noted as stomal or airway bleedings.
RESULTS: All 101 patients included were on LMWH. Twenty-two patients had n

Neuromuscular diseases (NMD) are indications for long-term home mechanical ventilation (HMV). Noninvasive ventilation is preferred to HMV. However, invasive mechanical ventilation (IMV) is more appropriate if the patient has uncontrollable airway secretions, the possibility of aspiration, failure to wean, or severe weakness of the respiratory muscles. But if the patient undergoes multiple intubation or tracheotomy, it will be more painful and unbearable. For some end-stage NMD patients who need long-term tracheostomy, HMV using noninvasive ventilator via tracheotomy may be a conservative care

BACKGROUND: Although Montgomery T-tube implantation has become an effective method for treating tracheal stenosis, the T-tube needs to be removed as a temporary treatment. At present, there is still a lack of research on the relevant factors affecting T-tube extraction. Therefore, we aimed to explore the clinical characteristics of post-tracheotomy tracheal stenosis (PTTS) disease and the risk factors affecting Montgomery T-tube extraction.
METHODS: Retrospective analysis of patients who underwent Montgomery T-tube implantation in the Respiratory and Critical Care Medicine of Emergency Genera
