Haploidentical Hematopoietic Stem Cell Transplantation: A Global Overview Comparing Asia, the European Union, and the United States.

One of the major projects of the Worldwide Network for Blood and Marrow Transplantation (WBMT) is to promote hematopoietic stem cell transplantation (HSCT) in emerging countries in the world. For these countries, HLA haploidentical HSCT (haplo-HSCT) from family members is an attractive approach because of its cost effectiveness. To learn the current status, including recent trends, of haplo-HSCT, the WBMT invited speakers from major transplant centers in 3 regions (Asia, Europe, and North America) to present at its annual WBMT Joint Session. This article represents the direct reports from thes

Unmanipulated haploidentical peripheral blood stem cell transplantation for precursor T-cell lymphoblastic lymphoma relapsed after autologous PBSCT.

Precursor T-cell lymphoblastic lymphoma (T-LBL) is a clinically aggressive disease, associated with a high subsequent relapse rate. We treated a pediatric patient with T-LBL who relapsed after an autologous peripheral blood stem cell transplantation. Unmanipulated peripheral blood was used as a source for the graft. Hematopoietic engraftment was prompt and the risk of graft-versus-host disease was acceptable. The patient has been in continuous complete remission without chronic graft-versus-host disease for 32 months. Hence, haploidentical peripheral blood stem cell transplantation might be on

Fibrin glue for refractory hemorrhagic cystitis after unrelated marrow, cord blood, and haploidentical hematopoietic stem cell transplantation.

BACKGROUND: Patients undergoing hematopoietic stem cell transplant (HSCT) are particularly exposed to the risk of developing hemorrhagic cystitis (HC), which is characterized by symptoms ranging from macroscopic hematuria to renal failure. Although HC significantly affects the quality of life and in a few cases becomes intractable leading to patient death, its therapeutic management has not been established. Fibrin glue (FG), a hemostatic agent derived from human plasma, has been largely employed in different surgical settings including urologic procedures. STUDY DESIGN AND METHODS: In this p

Haploidentical haematopoietic stem cell transplantation for acute leukaemia in adults: experience in Europe and the United States.

Cardiovascular disease (CVD) remains the leading cause of mortality worldwide, with atherosclerosis being the primary pathological substrate underlying most CVD. Epigenetics, defined as a set of regulatory mechanisms that dynamically modulate gene expression patterns or protein functional states through chemical modifications without altering the primary sequence of the genome, has been increasingly recognized as a pivotal driver in the pathogenesis of various diseases. Histone post-translational modifications, such as acetylation, methylation and lactylation, are catalyzed by specific enzymes

Haploidentical allogeneic hematopoietic cell transplantation in adults using CD3/CD19 depletion and reduced intensity conditioning: an update.

Haploidentical hematopoietic cell transplantation (HHCT) after high dose conditioning with CD34-selected stem cells has been complicated by high regimen related toxicities, slow engraftment and delayed immune reconstitution leading to increased treatment related mortality (TRM). A new regimen using reduced intensity conditioning (RIC) and graft CD3/CD19 depletion with anti-CD3 and anti-CD19 coated microbeads on a CliniMACS device may allow HHCT with lower toxicity and faster engraftment. CD3/CD19 depleted grafts not only contain CD34+ stem cells but also CD34 negative progenitors, natural kill

Artificial intelligence-based predictive model for relapse in acute myeloid leukemia patients following haploidentical hematopoietic cell transplantation.

BACKGROUND AND OBJECTIVES: Relapse is one of the most critical causes of transplant failure in patients with acute myeloid leukemia (AML) receiving haploidentical-related donor (HID) hematopoietic stem cell transplantation (HSCT). We aimed to develop an artificial intelligence (AI)-based predictive model for post-transplant relapse in patients with AML receiving HID HSCT. METHODS: This study included patients with consecutive AML (aged ≥ 12 years) receiving HID HSCT in complete remission (CR). We randomly selected 70% of the entire population (n = 665) as the training cohort for developing th

Low-dose ATG/PTCy for graft-versus-host disease prevention in haploidentical transplantation: a single-center experience.

BACKGROUND: The combination of anti-thymocyte globulin (ATG) and post-transplant cyclophosphamide (PTCy) has been administered for graft-versus-host disease (GVHD) prophylaxis of haploidentical hematopoietic stem cell transplantation (haplo-HSCT) in recent years. Varied doses of ATG and PTCy were applied in multiple studies with promising outcomes. METHODS: We retrospectively analyzed 51 consecutive leukemia patients who underwent haplo-HSCT with the joint use of low-dose ATG (27 patients with 7.5 mg/Kg and 24 patients with 5 mg/Kg) and PTCy (29 mg/Kg) for GVHD prophylaxis in our center. Impact of Early Cyclosporine on Cytokine Release Syndrome and Outcomes in Pediatric Haploidentical Hematopoietic Stem Cell Transplantation.

BACKGROUND: Cytokine release syndrome (CRS) is a frequent early complication of T cell-replete peripheral blood haploidentical hematopoietic stem cell transplantation (PB-haploHSCT), particularly in pediatric recipients. Post-transplant cyclophosphamide (PTCy) is the standard for graft-versus-host disease (GVHD) prophylaxis, but the role of early cyclosporine initiation in this setting remains underexplored. METHODS: We retrospectively analyzed 50 pediatric patients with hematolymphoid malignancies who underwent PB-haploHSCT with uniform myeloablative conditioning between 2017 and 2024. GVHD

Impact of CD34+ cell dose on outcomes of haploidentical peripheral blood stem cell transplantation in acute leukemia.

PURPOSE: Allogeneic hematopoietic stem cell transplantation remains a curative option for acute leukemia. While an adequate CD34+ cell dose is essential for engraftment, the optimal upper threshold in haploidentical peripheral blood stem cell transplantation (haplo-PBSCT) remains unclear. METHODS: We retrospectively analyzed 81 patients with acute leukemia who underwent haplo-PBSCT with reduced-intensity conditioning between 2010 and 2020. Patients were stratified by CD34+ cell dose (< 8 × 106/kg vs. ≥ 8 × 106/kg). Clinical outcomes, including overall survival (OS), non-relapse mortality (NRM

Outcomes of Haploidentical Stem Cell Transplantation for Acute Lymphoblastic Leukemia Patients: A Single-Center Experience in Mexico.

Access to allogeneic hematopoietic stem cell transplantation (allo-HSCT) for acute lymphoblastic leukemia (ALL) remains limited in low- and middle-income countries (LMICs). Haploidentical stem cell transplantation (haplo-SCT) with post-transplant cyclophosphamide (PTCy) has expanded donor availability, but data from Latin America are scarce. To evaluate clinical outcomes of haplo-SCT in pediatric and adult ALL patients in a single center in Mexico using a standardized protocol. Impaired thymic iNKT cell differentiation at early precursor stage in murine haploidentical bone marrow transplantation with GvHD.

INTRODUCTION: Early recovery of donor-derived invariant natural killer T (iNKT) cells are associated with reduced risk of graft-versus-host disease (GvHD) and overall survival. Patients with severe GvHD, however, had much slower iNKT cell reconstitution relative to conventional T cells. METHODS: To characterize the delay of iNKT cell reconstitution and explore its possible causes, we used a haploidentical bone marrow transplantation (haplo-BMT) mouse model with GvHD. We found the delayed recovery of thymic and peripheral iNKT cell numbers with markedly decreased thymic NKT1 subset in GvHD mic

Haploidentical Donor Blood or Marrow Transplantation for Myelodysplastic/Myeloproliferative Overlap Neoplasms: Results from a North American Collaboration.

Haploidentical donors offer a potentially readily available donor, especially for non-White patients, for blood or marrow transplantation (BMT). In this collaboration across North America, we retrospectively analyzed outcomes of first BMT using haploidentical donor and posttransplantation cyclophosphamide (PTCy) in MDS/MPN-overlap neoplasms (MDS/MPN), an otherwise incurable hematological neoplasm. We included 120 patients, 38% of non-White/Caucasian ethnicity, across 15 centers with median age at BMT 62.5 years. The median follow-up is 2.4 years. Graft failure was reported in 6% patients. At 3

Cidofovir, a choice for salvage treatment of cytomegalovirus infection in patients with haploidentical hematopoietic stem cell transplantation.

BACKGROUND: Cidofovir (CDV) is a nucleotide analogue with broad antiviral activities. It remains unclear about the CDV administration for anti-cytomegalovirus (CMV) treatment in patients with haploidentical hematopoietic stem cell transplantation (haplo-HSCT). PATIENTS AND METHODS: In this study, 31 out of 101 haplo-HSCT recipients who suffered CMV infection in the CT group (conventional treatment) were enrolled into the CDV-ST group (CDV second-line treatment). These patients were treated with CDV as they failed conventional treatment or they were unavailable to the preemptive antiviral ther

Comparison of 2 Different Doses of Antithymocyte Globulin in Conditioning Regimens for Haploidentical Hematopoietic Stem Cell Transplantation.

OBJECTIVES: Antithymocyte globulin is extensively used for prophylaxis of graft-versus-host disease in patients undergoing haploidentical hematopoietic stem cell transplantation. However, different doses of antithymocyte globulin are administered in clinical practice. This study aimed to identify the optimal dose of antithymocyte globulin (thymoglobulin) in haploidentical hematopoietic stem cell transplantation. MATERIALS AND METHODS: We retrospectively analyzed the effects of 10 mg/kg (2.5 mg/kg on days -5 to -2) versus 7.5 mg/kg thymoglobulin (2.5 mg/kg on days -4 to -2) on patients receivi

Comparison of outcomes after human leukocyte antigen-matched and haploidentical hematopoietic stem-cell transplantation for multiple myeloma.

BackgroundAllogeneic stem-cell transplantation (SCT) is a well-established immunotherapeutic strategy for multiple myeloma (MM) with a potent and often sustained graft-vs.-myeloma effect. This multicenter investigation aimed to analyze the complications and survival of haploidentical SCT in patients with MM, and compare the main outcomes with matched-related donors (MRDs).MethodsHaploidentical and MRD SCT was identified from a cohort of 97 patients with MM who received a myeloablative transplantation in 13 hospitals from May 2001 to December 2017. A matched-pair analysis was designed. For each
