Alcohol interventions for trauma patients are not just for adults: justification for brief interventions for the injured adolescent at a pediatric trauma center.

BACKGROUND: Research on the rates of alcohol and drug misuse as well as developmentally appropriate screening and intervention approaches in a hospitalized pediatric trauma population are lacking. The purpose of this study was to identify the rate of alcohol misuse in an admitted trauma population of adolescents aged 11 years to 17 years and to identify key correlates of alcohol misuse in this population including age, gender, and injury severity. METHODS: A prospective clinical study of 230 injured youth (aged 11-17 years) comprising both hospitalized and emergency department (ED) population

Injuries of the gastrointestinal tract from blunt trauma in children: a 12-year experience at a designated pediatric trauma center.

INTRODUCTION: Nonoperative management of solid organ injury from blunt trauma in children has focused concern on potential delays in diagnosis of hollow viscus injury with resultant increases in morbidity, mortality, and cost. This study of a large pediatric trauma database will review the issues of difficulty and/or delay in diagnosis as it relates specifically to definitive treatment and outcome. METHODS: We surveyed 11,592 consecutive admissions to a designated pediatric trauma center from 1985 to 1997 to identify children with documented injury of the gastrointestinal (GI) tract from blun

Association of plasma transfusion timing with mortality among patients with traumatic intracranial hemorrhage across US adult civilian trauma centers.

BACKGROUND: Traumatic intracranial hemorrhage (ICH) is a major driver of traumatic brain injury-related mortality. Although plasma transfusion has been associated with improved survival in these patients, the timing of transfusion influences outcomes is unclear. We sought to determine how plasma transfusion timing affects survival in patients with tICH, hypothesizing that earlier transfusion improves outcomes and that there is a threshold after which survival benefits diminish. METHODS: We conducted a retrospective analysis of adult trauma patients (≥18 y) with tICH who received plasma-based

Operative timing and surgical complexity in hand trauma: a multicenter analysis of replantation and non-replantation centers in Germany.

BACKGROUND: Hand injuries are common traumatic conditions that often require specialized surgical care. Differences in hospital structures and levels of specialization may influence the timeliness and complexity of treatment. This study evaluates quality of care across different hospital types, focusing on time-to-skin-incision and surgical management of finger and hand injuries. MATERIALS AND METHODS: This retrospective multicenter study is based on data from the HandTraumaRegister of the German Society for Hand Surgery (HTR DGH). A total of 16,726 surgically treated cases with documented fi

A qualitative process evaluation of SBIRT implementation in pediatric trauma centers using the Science to Service Laboratory implementation strategy.

BACKGROUND: Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an evidence-based practice that can identify adolescents who use alcohol and other drugs and support proper referral to treatment. Despite an American College of Surgeons mandate to deliver SBIRT in pediatric trauma care, trauma centers throughout the United States have faced numerous patient, provider, and organizational level barriers to SBIRT implementation. The Implementing Alcohol Misuse Screening, Brief Intervention, and Referral to Treatment Study (IAMSBIRT) aimed to implement SBIRT across 10 pediatric traum…

Are We on the Same Page? Defining Consensus for Pediatric Open Fracture Management Among Level 1 Pediatric Trauma Centers.

BACKGROUND: Open fractures require urgent management, including timely prophylactic antibiotics, to minimize infection and morbidity. Most pediatric treatment protocols are extrapolated from adult data, resulting in practice variation at pediatric centers regarding antibiotic timing and selection. In this study, we aimed to identify consensus regarding open fracture management protocols at Level-1 Pediatric Trauma Centers across the United States. METHODS: A survey was administered to 17 institutions participating in "Children's Orthopaedic Trauma and Infection Consortium for Evidence-Based S…

Are trauma centers penalized for improved prehospital resuscitation?: The effect of prehospital transfusion on arrival vitals and predicted mortality.

BACKGROUND: Prediction models for survival in trauma rely on arrival vital signs to generate survival probabilities. Hospitals are benchmarked on expected and observed outcomes. Prehospital blood (PB) transfusion has been shown to improve mortality, which may affect survival prediction modeling. We hypothesize that the use of PB increases the predicted survival derived from probability models compared with non-blood-based resuscitation. METHODS: All adult trauma patients presenting to a level 1 trauma center requiring emergency release blood transfusion from January 2017 to December 2021 were

Post-traumatic stress disorder (PTSD) symptoms and alcohol and drug use comorbidity at 25 US level I trauma centers.

BACKGROUND: Questions regarding the extent to which post-traumatic stress disorder (PTSD) is comorbid with alcohol and drug use are particularly germane in an era when the American College of Surgeons Committee on Trauma (ACS-COT) is considering policy requiring screening, intervention and/or referral services for patients presenting with psychological sequalae of traumatic injury. Literature review revealed few multisite trauma-center-based investigations that have assessed the association between PTSD symptoms and alcohol and drug use comorbidities in injured patients. METHODS: This investi

Operative Treatment and Clinical Outcomes in Peripheral Vascular Trauma: The Combined Experience of Two Centers in the Endovascular Era.

BACKGROUND: Arterial traumas of the extremities are quite rare in civilian records; nevertheless, patients with trauma of limbs are admitted daily in emergency departments worldwide. The up-to-date information about epidemiology and treatment (open vs. endovascular surgery) comes from war records and it is not always easy getting data on mortality and morbidity in these patients. The aim of this study is to analyze the approach (open or endovascular) and the outcome of patients with vascular trauma of upper limbs (from the subclavian artery) and/or lower limbs (distal to the inguinal ligament)

Variation Among Trauma Centers in the Use of Angioembolization and Splenectomy Rate in Isolated High-Grade Blunt Splenic Injuries.

BACKGROUND: Non-operative management (NOM) is often successful with an adjunctive use of splenic angioembolization (SAE) in patients with high-grade blunt splenic injuries (BSI). This study aimed to evaluate the variation between US trauma centers in the use of SAE for high-grade BSI undergoing NOM. METHODS: This retrospective cohort study utilized the American College of Surgeons Trauma Quality Improvement Program database. We included adult patients (age ≥ 16 years) with isolated high-grade BSI (Abbreviated Injury Scale 3-5) from 2013 to 2021. A logistic regression model was developed to es…

An analysis of transfers into designated trauma centers from referring institutions - the potential for virtual consultation to reduce transfers.

INTRODUCTION: Trauma care frequently happens in emergency departments (ED) outside of major trauma centers. Many injuries often exceed the specialty capabilities of referring hospitals, requiring transfer to larger trauma centers. However, the proportion of patients discharged home without admission from receiving facilities remains unclear, suggesting potential overutilization of transfers. We sought to determine the proportion of transfer patients that are discharged home from the receiving ED. METHODS: We studied patients ≥15 years captured in the Trauma Quality Improvement Program (TQIP) …

Comparing Chest Tube Thoracostomy in East Tennessee Trauma Centers: A Prospective Study Highlighting Differences in Urban vs Rural Locations.

Chest tube thoracostomy (CTT) is essential for lung expansion, but protocol discrepancies exist across trauma centers. This prospective study compared CTT protocols between an urban (center 1) and rural (center 2) level 1 trauma center in East Tennessee from June to August 2023. 66 trauma patients required CTT (51 from center 1 and 15 from center 2). Diagnostic practices and post-pull chest X-rays (CXR) differed significantly. Center 1 favored CXR for diagnosis (P = 0.012), while center 2 relied more on clinical presentation (P = 0.012). Post-pull CXR was less common at center 2 (P = 0.012). C…

The Demographics of Non-motor Vehicle Associated Railway Injuries Seen at Trauma Centers in the United States 2007 - 2014.

Introduction The majority of railway injury studies are limited by small sample size, restricted to a small geographical distribution, or located outside the United States (US). The aim of our study was to assess the demographic patterns associated with non-motor vehicle railway injuries in the US using a national trauma center database. Materials and Methods Data from the National Trauma Data Bank data from 2007 - 2014 were used; 3,506 patients were identified. For all statistical analyses, a p-value < 0.05 was considered significant. Results The patients were 81% male with an average age of

Endovascular versus open management of blunt traumatic aortic disruption at two military trauma centers: comparison of in-hospital variables.

BACKGROUND: Blunt traumatic aortic disruption (BTAD) carries significant mortality and morbidity. Traditional open repair has appreciable risks of perioperative mortality and spinal cord ischemic complications. Endovascular repair may reduce the incidence of these adverse outcomes. We present the experience at two military trauma centers with thoracic aortic endografting for trauma (TAET) and compare this with recent open experience. METHODS: A review of inpatient records was performed. All patients undergoing open repair or TAET for acute BTAD were studied. Collected data included demographi…

Does the availability of non-cross matched blood in the trauma bay effect the outcome of trauma patients? A retrospective study from a level-II trauma center.

BACKGROUND: Timely administration of blood products is critical in trauma resuscitation. Many trauma centers worldwide do not store blood products within the emergency room (ER), potentially delaying transfusion. This study evaluated the impact of immediate availability of uncrossmatched O-type packed red blood cells (UORBC) in the trauma bay on patient management and outcomes. METHODS: We conducted a retrospective cohort study at a single level-II trauma center, comparing all trauma patients triaged to the trauma bay in the year before (April 2020-March 2021) and after (April 2021-March 2022…

Variation in Management of Blunt Splenic Injury by Trauma Center Training Status.

BACKGROUND: Pediatric trauma surgeons have been the vanguard of nonoperative management of blunt splenic injuries. However, there is no uniform consensus on the determination of failure of nonoperative management and variability exists in management strategies across institutions. We hypothesized that centers with pediatric surgical fellowships would be less likely to pursue surgical management of blunt splenic injuries than other trauma centers without fellowship programs. STUDY DESIGN: The Pediatric Health Information System was queried for all patients 15 years old or younger admitted with…
