Syringomyelia and syringobulbia: pathophysiology, surgical treatment and nursing implications.

Syringomyelia (central cavitation of the spinal cord) and syringobulbia (cavitation of the medulla) are relatively rare disorders. These conditions are often found in association with congenital abnormalities such as Chiari malformations, with neoplasms or as sequelae to spinal cord trauma. The pathology progresses from minor sensory changes to weakness and wasting and, in the case of syringobulbia, to respiratory compromise and even death. Surgical treatment involves drainage or decompression of the syrinx cavity with diversion of fluid to the subarachnoid space or peritoneal cavity. Posterio

Caring for veterans returning home from Middle Eastern wars.

After serving at war, veterans work to resume a normal life at home. For some, the difficulties of reintegration are compounded by significant physical injury or psychological trauma, such as traumatic brain injury and post-traumatic stress disorder. It is equally important to attend to the all-encompassing experience of reintegration for any veteran. The transition is deeply embedded in the context of relationships and, as such, nurses can provide support. Nurses can find guidance on how to better connect with veterans returning home from war in Swanson's work on caring.

Healing Trauma While Staying at Home: Using Telehealth to Conduct a Brief Treatment Program for Posttraumatic Stress Disorder.

Studies suggest that a three-session brief treatment program (Brief Relaxation, Education, and Trauma Healing [BREATHE]) can help treat posttraumatic stress disorder (PTSD) and symptoms of trauma; however, the program has not been examined via telehealth. Thus, the current study evaluated the feasibility of BREATHE delivered via telehealth. The intervention included breathing retraining and psychoeducation about PTSD and trauma. Thirty participants from the community with confirmed PTSD diagnoses participated in this telehealth program. Treatment retention was high, and participants showed dec

Insect bite leading to a leg ulcer: a patient's story.

The patient, John, is a 94-year-old man with mild mobility issues but is otherwise fit and active. He developed a trauma-related ulcer, likely originating from an insect bite, that is now almost fully closed following treatment with compression hosiery. This case study discusses the symptoms, assessment, diagnosis, treatment plan and the outcomes resulting from timely and appropriate nursing interventions.

The Association Between Work- and Trauma-Related Factors and Risky Alcohol Use Among Health Care Providers: A Systematic Review.

BACKGROUND: About 65 million health care providers worldwide are exposed to trauma. Work-related trauma exposure is associated with risky alcohol use, which is correlated with health burdens and productivity impairments among health care providers. Hence, it is important to highlight the association between work-related trauma exposure factors and risky alcohol use among health care providers. OBJECTIVE: In this systematic review, we examined the relationship between work-related factors, trauma exposure, and risky alcohol use among health care providers. METHODS: We conducted a systematic s

Not All Birds Can Fly: My Winter as a Prison Nurse.

BACKGROUND: Rates of incarcerated young people are low in the UK. Yet hundreds of children remain in custody for violent and sexual offences. The prison system presents multiple obstacles for delivering effective nursing care, including a restrictive environment, a lack of positive male role models, and histories of trauma. PURPOSE: This article reflects on my placement at a young offender's institution. It explores the woundedness of the young people, the staff, and the challenges of working in such a desolate space. It also considers how nursing must navigate the moral ambiguity of caring f

Trauma in pregnancy.

Of all cases that come to hospital emergency rooms, the traumatized pregnant patient presents one of the most complicated dilemmas because nurses and doctors who commonly treat trauma victims rarely have comparable expertise in the management of pregnancy. Treatment and care of the traumatized pregnant patient are challenging because advanced pregnancy influences the pattern of trauma, alters laboratory values and clinical assessments, and changes hemodynamic parameters. Pregnancy may alter the usual trauma routines, and trauma may affect the outcome of the pregnancy. It is imperative that bot

Deep venous thrombosis prophylaxis in trauma: improved compliance with a novel miniaturized pneumatic compression device.

OBJECTIVE: Intermittent pneumatic compression (IPC) devices prevent lower-extremity deep venous thrombosis (LEDVT) when used properly, but compliance remains an issue. Devices are frequently discontinued when patients are out of bed, and they are rarely used in emergency departments. Trauma patients are at high risk for LEDVT; however, IPCs are underused in this population because of compliance limitations. The hypothesis of this study was that a new miniaturized, portable, battery-powered pneumatic compression device improves compliance in trauma patients over that provided by a standard devi

Adult respiratory distress syndrome following thoracic trauma.

Thoracic trauma is frequently complicated by the development of Adult Respiratory Distress Syndrome (ARDS). Over the past 20 years numerous articles and book chapter have explored the causes and clinical course of the syndrome. There still remains debate, however, concerning the pathophysiology and most effective treatment modalities. It is apparent that trauma activated numerous complex biochemical pathways, such as the complement and arachidonic acid cascades, which probably resulted in neutrophil and macrophage mediated tissue injury. This article examines some of the more recent thoughts a

Chest injuries in the trauma patient.

The primary survey of a trauma victim always begins with the ABCs used in resuscitation. Chest trauma compromises the normal physiology of respiration and circulation. Although some chest injuries are overt, some are not. The mechanism of injury is important for increasing the degree of suspicion used to identify covert injuries. The most lethal injuries include tension pneumothorax, sucking chest wound, massive hemothorax, flail chest, and cardiac tamponade. Cardiac and aortic ruptures often cause death before the victim enters the health care system. Potentially lethal injuries include myoca

Head trauma in the older adult population.

The elderly person who is brain injured requires some special considerations because of the effects of aging on the body and brain. These individuals are increasingly admitted to the intensive care units for trauma from accidents, falls, and so forth. In addition to the care for the problems associated with brain trauma, the nurse must also be aware of the patient's medical history to prevent complications related to drug interactions, fluid overload, or volume deficit. This article addresses these considerations.

Once BITTEN, Twice Shy: An Applied Trauma-Informed Healthcare Model.

Nurses need a pragmatic theory to understand and respond to the impact of vulnerable patients' previous healthcare experiences, as these are likely to influence response and adherence to treatment plans. The authors of this paper present the new BITTEN (Betrayal history by health-related institutions, Indicator for healthcare engagement, Traumas related to healthcare, Trust in healthcare providers, patient Expectations and Needs) Model of Trauma-Informed Healthcare. BITTEN identifies patients' current healthcare expectations and needs as a function of their previous betrayal by healthcare syst

[Evaluation of radiation exposure of personnel in an orthopaedic and trauma operation theatre using the new real-time dosimetry system "dose aware"].

AIM: There is a positive correlation between operation time and staff exposure to radiation during intraoperative use of C-arm fluoroscopy. Due to harmful effects of exposure to long-term low-dose radiation for both the patient and the operating team it should be kept to a minimum. AIM of this study was to evaluate a novel dosimeter system called Dose Aware® (DA) enabling radiation exposure feedback of the personal in an orthopaedic and trauma operation theatre in real-time. METHOD: Within a prospective study over a period of four month, DA was applied by the operation team during 104 orthopa

Learning from History: Applying World War II Palliative Care Lessons to Future Conflicts.

Because of the volume of casualties projected in future large-scale combat operations (LSCO), military nurses need to be prepared to perform palliative care and end-of-life care due to the projected lack of air superiority and resources required to rapidly evacuate patients. Much of the mental trauma experienced by military nurses who served during Operation Iraqi Freedom and Operation Enduring Freedom was because they felt unprepared and that they couldn't do enough. The lessons learned from WWII related to combat palliative care can inform the future needs of military nurses in preparation f

The Trajectories of Platelet, Fibrinogen, and D-dimer in Severe Trauma Patients and Their Value in Predicting Venous Thromboembolism: A Retrospective Study.

BACKGROUND: Venous thromboembolism (VTE) is a major contributor to morbidity and mortality in severe trauma (ST) patients. Platelet count (PLT), fibrinogen level (FIB), and D-dimer level are important coagulation indicators; however, few studies have described their trajectory in ST and assessed predictive value for VTE in clinical and nursing practice. OBJECTIVE: To describe the trajectories of PLT, FIB, and D-dimer on days 1, 3, 5, and 7 after ST, and to evaluate their predictive value for post-traumatic VTE. METHODS: Clinical data, including PLT, FIB, and D-dimer, were retrospectively col

Risk factors and consequences of mental health problems in nurses: A scoping review of cohort studies.

Mental health problems in nurses are prevalent and impairing. To date, no literature has comprehensively synthesised cohort evidence on mental health among nurses. This scoping review aimed to synthesise the existing literature on the risk factors and consequences of mental health problems in nurses. A systematic search was conducted on PubMed, EMBASE, Epistemonikos database, Web of Science, CINAHL, and PsycINFO from inception to March 2023. We identified 171 cohort studies from 16 countries, mostly (95.3%) from high-income economies. This review indicated that nurses worldwide encountered sig

Analysis of posttraumatic syndrome following a mild head injury.

Medical, financial and legal implications of prolonged disability after mild head injury are significant. In this study, researchers examined symptoms and psychosocial sequelae of head trauma three and six months after mild head injury in a group of 75 adult patients admitted to the trauma service of St. Anthony Hospital, Denver, Colorado. Results support the following conclusions: (1) nursing observation plays a key role in identification of patients at risk for posttraumatic syndrome; (2) early intervention may facilitate the recovery process; (3) support from family and significant others i

The management of patients with spinal cord injury.

Few diseases or injuries have greater potential for causing death or having a devastating impact on a person's quality of life than cervical spine trauma. All patients admitted to hospital after significant trauma must therefore be assumed to have a potentially unstable spinal fracture until it is proven otherwise, to prevent their sustaining further injury owing to inappropriate management. It is vital that nurses are familiar with the signs and symptoms of such injuries and aware of the appropriate management techniques.
