A case of persistent dizziness after travel.

Although vertigo is a common complaint with patients presenting to an ED, some uncommon causes also must be considered. This report focuses on a woman who developed sudden-onset vertigo with associated mental status changes after a long-distance flight. Her symptoms were consistent with mal de debarquement syndrome, an uncommon cause of vestibular dysfunction.

Rabies prevention in people who travel overseas.

Increasing numbers of U.K. travellers are visiting rabies-endemic countries. This article describes how this invariably fatal disease is transmitted to humans and how it may be prevented by increased awareness, and pre and post-exposure vaccination. It aims to help practitioners identify patients at increased risk of rabies, and looks at the common myths and misconceptions travellers may have about rabies and rabies vaccination.

Enteric fever: a travel medicine oriented view.

PURPOSE OF REVIEW: Enteric fever continues to be an important infection among populations in endemic countries and among travelers to these areas. This review aims to describe recent epidemiological trends and developments in diagnosis, treatment and prevention. RECENT FINDINGS: Data indicate that the burden of enteric fever is declining in many middle-income countries, whereas the bulk of cases occur in the Indian subcontinent and in south-east Asia. In these regions, Salmonella Paratyphi A is increasingly seen and sometimes surpasses Salmonella Typhi as the main pathogen. Enteric fever caus…

[Common diseases after travel in tropical countries].

About 10% of the travellers are ill after return. The most common diseases diagnosed in returning travellers are more often of cosmopolitan than exotic origin. These diseases are mostly infections and involve the digestive and the respiratory tract, and the skin. Thus a history of travel should not systematically orients towards an exotic disease. Indeed these latter diseases are less commonly diagnosed even in the subgroup of febrile patients. Nonetheless some of these diseases can be fatal whereas other are associated with a risk of transmission in hospital units or in the community. Subsequ…

Meningococcal disease in international travel: vaccine strategies.

International travel and migration facilitate the rapid intercontinental spread of meningococcal disease. Serogroup A, and to a lesser extent serogroup C, have been responsible for pandemics in the past (mainly in Africa), but in recent years there was an international outbreak due to W135 related to the Hajj pilgrimage. The high carriage rates, persistence and transmissibility, in combination with the high case fatality rate of the Hajj-associated W135 outbreak clone, certainly raise considerable concern about the public health consequences of widespread dissemination of this organism and the…

[Tsutusgamushi fever after travel to Southeast Asia].

HISTORY AND CLINICAL FINDINGS: A 49-year-old travel guide fell ill during his return from a 6-week-stay in Vietnam, Myanmar and Thailand. He developed high fever and severe headache. On admission, the body temperature was at 39.5 degrees C with relative bradycardia. A black, crusted skin lesion of approximately 5 mm diameter was noted on the chest wall and was interpreted as an insect bite. INVESTIGATIONS: CRP and liver enzymes were elevated. Total leucocyte count was normal but the differential count showed a left shift and aneosinophilia. Imaging procedures revealed splenomegaly and small p…

Travel Associated Legionnaires' Disease in Europe : 2003.

Six hundred and thirty two cases of travel-associated legionnaires' disease with onset in 2003 were reported to the EWGLINET surveillance scheme by 24 countries. Eighty nine clusters were detected, 35 (39%) of which would not have been detected without the EWGLINET scheme. One hundred and seven accommodation sites were investigated and 22 sites were published on the EWGLI website. The proportion of cases diagnosed primarily by the urinary antigen test was 81.2%, and 48 positive cultures were obtained. Thirty eight deaths were reported to the EWGLINET scheme, giving a crude fatality rate of 6%.…

Pediatric travel consultation in an integrated clinic.

BACKGROUND: In May 1997, a pediatric travel service was created within a larger integrated University-County Health Department international travel clinic. The purpose of the service was to further enhance the travel advice and care provided to children and their parents or guardians. The current study was designed to describe the care of children in this setting and to compare the care of children seen in the Pediatric Travel Service with that of children seen by other providers. METHODS: All pediatric patients (defined as individuals < or = 18 years of age) receiving care in the travel clin…

Travel and the emergence of infectious diseases.

Travel is a potent force in the emergence of disease. Migration of humans has been the pathway for disseminating infectious diseases throughout recorded history and will continue to shape the emergence, frequency, and spread of infections in geographic areas and populations. The current volume, speed, and reach of travel are unprecedented. The consequences of travel extend beyond the traveler to the population visited and the ecosystem. When they travel, humans carry their genetic makeup, immunologic sequelae of past infections, cultural preferences, customs, and behavioral patterns. Microbes,…

Prevention and treatment of travel-related illness.

Traveler's diarrhea, malaria, acquired immunodeficiency syndrome and jet lag are among the issues for the traveler preparing for a trip to or returning from developing countries. With appropriate measures, most travel-related diseases can be prevented. Diarrheal diseases, schistosomiasis, sexually transmitted diseases and AIDS can be prevented with proper avoidance behavior. Diseases such as hepatitis, rabies, yellow fever and meningitis can be prevented with immunization. Chemoprophylaxis can prevent malaria, altitude sickness and sinus barotrauma. Diagnosing an illness in a returning travele…

Health problems after travel to developing countries.

Travelers to developing countries participated in a follow-up study of the health risks associated with short (less than three months) visits to these nations. Travelers to the Greek or Canary Islands served as a control cohort. Participants completed a questionnaire to elicit information regarding pretravel vaccinations, malaria prophylaxis, and health problems during and after their journey. Relevant infections were confirmed by the respondent's personal physician. The questionnaire was completed by 10,524 travelers; the answer rate was 73.8%. After a visit to developing countries, 15% of th…

[Health advice before travel to tropical countries].

Some 8% of Swiss citizens travel in the developing countries annually. Advising these travellers is an important task for the physician, though the most frequent infections are the less serious travellers' diarrhea and common colds. To some extent the more serious tropical diseases can be avoided by appropriate measures, e.g. vaccination (against yellow fever, tetanus, polio etc). and by chemoprophylaxis (malaria). Individual fitness for travel should be checked prior to any journey abroad, and special advice must be given to travellers with special risks (e.g. pregnant women, infants, persons…
