Fractal dynamics in spontaneous motor activity associate with effective antidepressant chronotherapeutic response, cerebellar volumes, and white matter microstructure in bipolar disorder.

AIM: Bipolar disorder (BD) is characterized by circadian dysregulation, altered sleep-wake behaviors, and heterogeneous antidepressant response. Chronotherapeutic interventions such as total sleep deprivation combined with light therapy (TSD + LT) produce rapid antidepressant effects, but predictors of outcome are lacking. We investigated whether fractal dynamics of motor activity, indexed by the short-term scaling exponent α₁, are associated with antidepressant response, sleep-wake regulation, and structural brain measures and may serve as a biomarker of treatment responsiveness. METHODS: Si

Differential effects of cognitive behavioral therapy and pharmacotherapy on cognitive function in patients with major depressive disorder: A propensity-weighted longitudinal study.

BACKGROUND: Neurocognitive dysfunction is a core feature of major depressive disorder (MDD), predicting relapse, persisting after remission, and hindering functional recovery. Although cognitive behavioral therapy (CBT) and pharmacotherapy are both established treatments, their differential effects on specific neurocognitive domains remain poorly characterized. We investigated the distinct neurocognitive effects of CBT and pharmacotherapy in patients with MDD. METHODS: This study assessed 69 patients with MDD treated with either manual-based CBT combined with pharmacotherapy (n = 41) or pharm

Heterogeneity in the efficacy of bosentan for hypertensive nephropathy: a study on individualized benefit prediction models based on pathological subtyping and dynamic trajectories.

BACKGROUND: Many patients with hypertensive nephropathy progress to chronic kidney disease (CKD). Targeting the core pathophysiological pathway of endothelin-1 activation, this study aimed to elucidate response patterns to the endothelin receptor type A/B (ETA/ETB) antagonist bosentan and construct a personalized prediction model. METHODS: This single-centre retrospective cohort study enrolled 166 patients with hypertension-related nephropathy confirmed by renal biopsy. Risk stratification was performed based on a pathological injury scoring. Three efficacy trajectories were identified using

Gender disparities in high-risk pulmonary embolism: a nationwide analysis of management strategies and clinical outcomes.

BACKGROUND: Pulmonary embolism (PE) remains a significant cause of cardiovascular mortality. Despite advancements in diagnosis and treatment, gender disparities in high-risk PE management and outcomes remain poorly understood. This study aimed to investigate gender-based differences in the presentation, treatment, and outcomes of patients with high-risk PE. METHODS: We analyzed data from the National Inpatient Sample for patients with high-risk PE from 2016 to 2021. High-risk PE was defined as PE with cardiogenic shock, mechanical ventilation, mechanical circulatory support, or vasopressors. M

Socioeconomic and Demographic Influences on Biologic Treatment Outcomes in Patients With Chronic Rhinosinusitis.

IMPORTANCE: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a prevalent inflammatory disease that significantly impairs quality of life (QOL) and imposes a substantial economic burden. Socioeconomic and demographic factors influence access to care, treatment adherence, and outcomes, yet their impact on response to biologic therapy remains poorly defined. OBJECTIVE: To examine the impact of socioeconomic and demographic determinants on symptom severity and QOL before and after monoclonal antibody therapy for CRSwNP. DESIGN: Exploratory retrospective cohort study with prospective socioeco

Sex Differences in Exercise Capacity and Outcomes Following Outpatient Cardiac Rehabilitation.

BACKGROUND: Cardiac rehabilitation (CR) leads to increased exercise capacity (VO2peak) and risk reduction of major adverse cardiovascular events (MACEs). Women exhibit blunted VO2peak improvements following CR. The purpose was to examine sex differences in the clinical predictors of VO2peak improvement and odds of MACEs following CR. METHODS: From 1999 to 2017, all consecutive patients undergoing cardiopulmonary exercise testing before and after CR were included. Sex differences in peak exercise metrics in response to CR were compared. Multiple logistic regression models were fit to assess cl

Genome-wide association analysis of treatment response to onabotulinumtoxinA in Han Chinese patients with chronic migraine: a pilot study.

BACKGROUND/OBJECTIVES: OnabotulinumtoxinA (BoNT-A) use is an established preventive treatment for chronic migraine (CM); however, individual response rates vary. Although genetic factors may contribute to this variability, the underlying genetic determinants remain largely undefined, especially in Asian populations. Therefore, we aimed to identify genetic variants associated with the response to BoNT-A treatment in Han Chinese patients with CM. METHODS: We conducted a genome-wide quantitative trait locus (QTL) analysis using the Taiwan Precision Medicine (TPM) array in Han Chinese patients wit

Dorsolateral Prefrontal rTMS Symptom Cluster Response Trajectories for Depression.

IMPORTANCE: Repetitive transcranial magnetic stimulation (rTMS) is an established intervention for treatment-resistant depression, but only a minority of patients achieve remission, and characteristics associated with response are lacking. Sum-score analyses of the Hamilton Depression Rating Scale (HDRS) obscures how distinct symptom clusters change over time. OBJECTIVE: To identify distinct longitudinal symptom-cluster response trajectories during dorsolateral prefrontal rTMS and to determine baseline characteristics associated with trajectory membership. DESIGN, SETTING, AND PARTICIPANTS:

Mapping the anatomical landscape of colorectal tumours: Location-specific efficacy of anti-epidermal growth factor receptor antibodies: Pooled analysis of randomised trials.

BACKGROUND: The selection of biologic therapies in metastatic colorectal cancer (mCRC) is traditionally guided by right-left classification; however, this binary classification may not fully capture heterogeneity in responsiveness to anti-epidermal growth factor receptor (EGFR) therapy. We evaluated whether treatment efficacy varies across detailed anatomical tumour segments. METHODS: Individual patient-level data from 12 randomised controlled trials in the ARCAD database were analysed. Patients with RAS wild-type (RAS-WT) mCRC receiving first-line doublet chemotherapy plus anti-EGFR therapy

Evidence-based personalised medicine in critical care: a framework for quantifying and applying individualised treatment effects in patients who are critically ill.

Clinicians aim to provide treatments that will result in the best outcome for each patient. Ideally, treatment decisions are based on evidence from randomised clinical trials. Randomised trials conventionally report an aggregated difference in outcomes between patients in each group, known as an average treatment effect. However, the actual effect of treatment on outcomes (treatment response) can vary considerably between individuals, and can differ substantially from the average treatment effect. This variation in response to treatment between patients-heterogeneity of treatment effect-is par
