Ocular tuberculosis associated with Epstein-Barr virus myelitis: A case report.

Ocular tuberculosis (OTB) is a chronic eye infection caused by Mycobacterium tuberculosis. Some cases of myelitis are associated with Epstein-Barr virus (EBV), with 1-5% of EBV infections leading to neurologic complications. We describe a 34-year-old Iranian woman with OTB and EBV coinfection. Despite initial success with anti-TB agents, the disease progressed, necessitating enucleation. Mycobacterium tuberculosis was detected by a tuberculin coagulation test, and EBV was confirmed via polymerase chain reaction. MRI showed plaques in the spinal cord and brain. The patient was treated with anti

YELLOW SUBRETINAL PIGMENT EPITHELIUM DEPOSITS: A NOVEL SIGN IN OCULAR TUBERCULOSIS.

PURPOSE: To describe a novel clinical and imaging finding in patients with tubercular posterior uveitis. METHODS: A retrospective review of 3 cases presented at a tertiary referral eye centre in North India was performed between June 2016 and March 2019. All the patients had received an initial diagnosis of noninfective etiologies (sympathetic ophthalmia, necrotizing scleritis, and lymphoma). Fundus photography, fluorescein angiography, fundus autofluorescence, and enhanced-depth imaging optical coherence tomography were reviewed. RESULTS: Three patients (all Asian Indian women: aged 18, 49,

Unique case of presumed ocular tuberculosis presenting as bilateral pseudoretinitis pigmentosa.

Ocular tuberculosis is often a challenging diagnosis with extremely diverse clinical manifestations and, most times, impractical bacilli isolation. Here we describe an unprecedented case of presumed ocular tuberculosis presenting with bilateral pseudoretinitis pigmentosa and retinal vasculitis.

The Collaborative Ocular Tuberculosis Study (COTS) Consensus (CON) Group Meeting Proceedings.

An international, expert led consensus initiative was set up by the Collaborative Ocular Tuberculosis Study (COTS) group to develop systematic, evidence, and experience-based recommendations for the treatment of ocular TB using a modified Delphi technique process. In the first round of Delphi, the group identified clinical scenarios pertinent to ocular TB based on five clinical phenotypes (anterior uveitis, intermediate uveitis, choroiditis, retinal vasculitis, and panuveitis). Using an interactive online questionnaires, guided by background knowledge from published literature, 486 consensus s

Pattern of Recurrent Inflammation following Anti-tubercular Therapy for Ocular Tuberculosis.

PURPOSE: To investigate recurrence patterns following anti-tubercular therapy (ATT) in different anatomical presentations of ocular tuberculosis (OTB). METHODS: Retrospective analysis of clinically diagnosed OTB patients divided anatomically into groups A (anterior + intermediate uveitis) and B (posterior + panuveitis). Recurrence was defined as reappearance of intraocular inflammation in a quiescent eye following completion of at least 6-months ATT. Anatomical sites of recurrence were compared to that of presentation. RESULTS: No significant difference in post-ATT recurrent inflammation was

Spectrum of Choroidal Involvement in Presumed Ocular Tuberculosis: Report from a Population with Low Endemic Setting for Tuberculosis.

OBJECTIVE: To describe the clinical spectrum and outcome of patients with presumed tubercular uveitis and choroidal involvement. METHODS: A retrospective case series nested in a cohort study was enrolled at a tertiary referral eye care center in the UK. Failure was defined as recurrence of lesion within 6 months of completion of antitubercular therapy (ATT) or corticosteroid therapy. RESULTS: Seventy-seven patients with presumed ocular tuberculosis and choroidal involvement were included in the study. Mean age was 45.5 ± 15.7 years, 44 (57.1%) patients were male, and 51 (66.2%) presented wit

Metformin reduces intracellular M. tuberculosis replication and inflammation in vitro: A potential host-directed therapy for ocular tuberculosis?

Ocular tuberculosis (TB) remains a leading cause of infectious uveitis in TB-endemic settings. Antitubercular therapy, with or without adjunctive corticosteroids or immunosuppressive agents, is the mainstay of treatment; however, more rapid resolution of intraocular inflammation is desirable to reduce visual morbidity. This preliminary study investigates metformin as a potential host-directed therapy (HDT) for ocular TB through in silico prediction and validation using in vitro disease models. In silico drug discovery was conducted using a bioinformatics platform. In vitro assays were performe

Bilateral Optic Atrophy and Epiretinal Membranes: An Atypical Presentation of Ocular Tuberculosis.

A 15-year-old boy presented himself to the retina clinic with a six-year history of poor vision in both eyes. Ocular symptoms started with deterioration in the vision of both eyes associated with ocular pain. There was no history of antecedent trauma, headaches, seizures, or loss of consciousness. The general and systemic examinations were normal. On ocular examination, the corrected visual acuity was 6/24 and 6/6 in the right and left eyes, respectively. The anterior segment examination was normal except for a relative afferent pupillary defect in the right eye. Examination of the posterior s

Validation of the Online Collaborative Ocular Tuberculosis Study Calculator for Tubercular Uveitis.

IMPORTANCE: This was the first study, to the authors' knowledge, to statistically evaluate the predictive accuracy of Collaborative Ocular Tuberculosis Study (COTS) calculator in guiding initiation of antitubercular therapy (ATT) in patients with clinically suspicious tubercular uveitis (TBU) in an international cohort. OBJECTIVE: To evaluate the accuracy of a score of 4 or greater on the online COTS calculator in recommending ATT initiation. DESIGN, SETTING, AND PARTICIPANTS: This study was an evaluation of a diagnostic test or technology. Data input required for the COTS calculator were ex
