Tuberculosis-Specific Antigen/Phytohemagglutinin Ratio Combined With GeneXpert MTB/RIF for Early Diagnosis of Spinal Tuberculosis: A Prospective Cohort Study.

Spinal tuberculosis (TB), the most common form of musculoskeletal tuberculosis, is an infection-related disease globally, with paraplegia occurring in severe cases. Therefore, identification of spinal TB at an early stage is important for early intervention and eventual therapy. In this study, we conducted a prospective cohort study in routine clinical practice to investigate the diagnosis of different TB tests. A total of 519 patients were recruited based on the radiology of spinal TB. The diagnostic model was computed by regression analysis and was determined by receiver operating characteri

IL-10 and TGF-β1 may weaken the efficacy of preoperative anti-tuberculosis therapy in older patients with spinal tuberculosis.

Spinal tuberculosis is a common extrapulmonary type that is often secondary to pulmonary or systemic infections. Mycobacterium tuberculosis infection often leads to the balance of immune control and bacterial persistence. In this study, 64 patients were enrolled and the clinicopathological and immunological characteristics of different age groups were analyzed. Anatomically, spinal tuberculosis in each group mostly occurred in the thoracic and lumbar vertebrae. Imaging before preoperative anti-tuberculosis therapy showed that the proportion of abscesses in the older group was significantly low

Changing trends of presentation of central nervous system tuberculosis: Relative prevalence of cranial and spinal tuberculosis and drug resistance patterns.

OBJECTIVE: Inappropriate use of antituberculosis drugs and a poor compliance has led to an increase in the prevalence of resistant Mycobacterium tuberculosis. The aim of this study was to document the changing trends in clinical presentation and drug resistance in patients with tuberculosis (TB) of the brain and the spine. MATERIALS AND METHODS: The authors retrospectively analyzed data from 243 patients admitted in a neurosurgical unit for surgical management of TB of the brain and spine from 2000 to 2013. To establish changes in trends, the patient population was arbitrarily divided into tw

Atypical Presentation of Pediatric Spinal Tuberculosis as Chronic Abdominal Pain: A Case Report.

Spinal tuberculosis, also known as Pott's disease, in children is a rare manifestation of extrapulmonary tuberculosis. The nonspecific clinical features, such as chronic abdominal pain, of extrapulmonary TB or spinal TB in children tend to delay diagnosis. We present the case of a 14-year-old girl who has been suffering from abdominal and backpain associated with spinal tuberculosis. This is a case study reporting on the treatment and outcome of a 14-year-old girl who presented with progressive lower abdominal and back pains for 6 months, accompanied by slight kyphotic deformity. The involveme

Combined posterior-anterior surgery with autologous bone grafting for multi-segmental spinal tuberculosis.

BACKGROUND: Multi-segmental spinal tuberculosis is a severe infectious disease of the spine affecting multiple vertebral bodies, often leading to spinal instability, progressive kyphosis, and irreversible neurological impairment. Its diagnosis and treatment are notably challenging due to the extensive distribution of lesions and severe bone destruction. METHODS: A retrospective analysis was conducted on 95 patients with multi-segmental spinal tuberculosis involving four or more vertebrae, who were treated at the Department of Orthopedics, General Hospital of Ningxia Medical University, from J

Sensitivity and specificity of Xpert MTB/RIF for extrapulmonary non-spinal osteoarticular tuberculosis.

BACKGROUND: Conventional diagnostics for Osteoarticular Tuberculosis (OATB) are slow and prone to inaccuracy due to low bacterial load, delaying treatment. Xpert MTB/RIF is WHO-recommended for TB cases, but there is a dearth of literature for non-spinal OATB. METHODS: In a 4-year study (Sept 2016-Feb 2020), non-spinal osteoarticular infections were retrospectively analyzed. Xpert MTB/RIF sensitivity and specificity were evaluated against tuberculosis defined by histology and culture against patients having culture-isolated pyogenic infections as a reference standard for negative tuberculosis.

Value of Postoperative Serum Albumin to Predict Postoperative Complication Severity in Spinal Tuberculosis.

BACKGROUND: Many complications occur after surgery in patients with spinal tuberculosis (STB); however, the severity varies in different patients. The complications' severity is evaluated from grades I to V by the Clavien-Dindo classification (CDC), and grade V is the most severe. Most complications are mild, and only severe complications are life threatening, and thus, it is important to identify severe complications in patients with STB. The purpose of this study was to identify the risk factors of postoperative complication severity in patients with STB. METHODS: Between January 2012 and M

Spinal Tuberculosis in Young HIV-exposed Infants: Two Cases of Probable Congenital Transmission.

The presentation of congenital tuberculosis (TB) is varied, with frequent extrapulmonary disease, but congenital spinal TB has been rarely reported. We present 2 cases of spinal TB (1 confirmed, 1 clinical diagnosis) in HIV-exposed infants with likely congenital transmission. Increased vigilance for congenital TB, including uncommon presentations, is needed, particularly in countries with high HIV and TB prevalence.

Complete debridement for treatment of thoracolumbar spinal tuberculosis: a clinical curative effect observation.

BACKGROUND CONTEXT: Traditional focal debridement involves clearing of cold abscesses, caseous necrosis, residual intervertebral disc tissue, sinus tracts, bony sequestration, and inflammatory granulation. Reports have demonstrated that approximately 13% to 26% of patients were not better or relapsed after traditional focal debridement; these patients required a second surgery or prolonged antituberculous therapy. The presence of retained and diseased focal tissues requiring removal remains poorly understood. The contents of these retained tuberculous foci, improvement of surgical strategies,
