PURPOSE: The aim of this study was to clarify the definition of the anterior wall of the ulnar tunnel and to reveal the topographical characteristics of the various components of the ulnar tunnel.
MATERIALS AND METHODS: Twenty-two forearms from 11 embalmed cadavers (7 males, 4 females; mean age, 67.8 years) were carefully dissected.
RESULTS: In all cases, the anterior wall of the ulnar tunnel comprised the hypothenar fascia, which originated from the tendon of the flexor carpi ulnaris muscle. The palmar carpal ligament, the distal extent of the antebrachial fascia, was located deep to the an

The cubital tunnel syndrome is one of the most common entrapment neuropathy of the upper limb. The ulnar nerve can be compressed in the oteofibrous tunnel by the bone structures, the Osborne's ligament, the fascia of the ulnar flexor muscle of the carpus or of the aponeurosis of the deep flexor of the fingers. Pressure values in the cubital tunnel >50 mm Hg induce blocking of intraneural circulation with electrodiagnostic modifications, clinical signs and histological changes including demyelinazion of the nerve proximal to the cubital tunnel. Surgery becomes essential in case of failure of co

On January 17, 1995, the Hanshin-Awaji earthquake directly hit a metropolis and destroyed approximately 120,000 houses. Many people were buried under the debris of the Japanese style wooden houses or were injured by falling furniture. Twenty-five patients (35 extremities) with earthquake associated peripheral nerve palsy were studied and followed up for 2 years. The characteristic feature of earthquake associated palsies was combined nerve palsy induced by prolonged compression. There were 11 extremities with brachial plexus palsy; two extremities with combined radial, median, and ulnar nerve

Six patients with cubitus varus deformity after a supracondylar fracture of the humerus had ulnar nerve palsy 7 (3-14) years following the fracture. All the patients showed anterior dislocation of the ulnar nerve during elbow flexion. In cubitus varus deformity, medial shifting of the triceps muscle occurs, which pushes the ulnar nerve anteriorly and frequently causes ulnar-nerve dislocation. Five of the 6 patients underwent surgery with subsequent improvement.

The carpal tunnel syndrome is the most common entrapment syndrome of the upper limb. Compression of the median nerve is most often idiopathic and typically occurs in women aged 50. The diagnosis is clinical and must look for signs of gravity (hypoesthesia, thenar atrophy). The electromyogram is not required but recommended for surgical indication, It assesses the severity of the disease and identifies other injury. Conservative treatment is available in the beginner to moderate forms. In case of failure of this treatment or with severe objective signs, treatment is surgical. The ulnar nerve at

BACKGROUND: Simultaneous compression of the median and ulnar nerve at the elbow region has not been sufficiently highlighted in the literature. The purposes of the present study are to report our experience regarding this entity, to elucidate the clinical features, and to describe the operative technique and findings as well as the results of simultaneous decompression performed through the same medial incision.
METHODS: We performed a retrospective study of thirteen elbows in thirteen patients -nine men and four women- with simultaneous compression of the median and ulnar nerve at the elbow

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Understand the anatomy of the median and ulnar nerves. 2. Perform clinical examination of the upper limb. 3. Analyze examination results to diagnose level of nerve compression.
SUMMARY: Numbness and loss of strength are common complaints in the hand surgery clinic. Two nerves that are commonly entrapped (median and ulnar nerves) have several potential sites of entrapment, and in busy clinical practice, the less common sites may be overlooked, leading to wrong or missed diagnoses. This article reviews the a

The purpose of this study was to modify the classical longitudinal skin incision to avoid injury to the cutaneous nerve branches on the medial side of the elbow when a surgical release of the ulnar nerve was performed. Injury to the medial antebrachial cutaneous nerve is not a rare complication after release of the ulnar nerve at the elbow. This will leave an area of hyposthesia on the posteromedial aspect of the proximal forearm, to which the patient will usually get accustomed. Some patients may have an area of skin dysesthesia and a painful amputation neuroma requiring surgical treatment. T

Initial treatment of most compressive neuropathies at the elbow is nonoperative, consisting of rest, avoidance of elbow flexion, and, when necessary, temporary immobilization of the elbow and wrist. If symptoms persist, particularly when accompanied by muscle weakness, surgery is usually indicated. Operative procedures include decompression without transposition of the nerve (in situ or by means of medial epicondylectomy) and decompression with transposition of the nerve carried out in a subcutaneous, intramuscular, or submuscular fashion. The indications, advantages, disadvantages, and surgic

Ulnar nerve compression at the elbow can occur at any of five sites that begin proximally at the arcade of Struthers and end distally where the nerve exits the flexor carpi ulnaris muscle in the forearm. Compression occurs most commonly at two sites-the epicondylar groove and the point where the nerve passes between the two heads of the flexor carpi ulnaris muscle (i.e., the true cubital tunnel). The differential diagnosis of ulnar neuropathies at the elbow includes lesions that cause additional proximal or distal nerve compression and systemic metabolic disorders. A complete history and a tho

Ulnar neuropathy at the elbow (UNE) is described as a nerve injury arising from trauma. For practical management purposes, two general types of UNE have been defined. One group is comprised of those cases in which identifiable bone deformity is present at the elbow and the other comprises cases in which only nerve dysfunction is found. The concept of cubital tunnel syndrome is discussed, as is the importance of differential diagnosis and the use of diagnostic electrophysiology. Nerve transposition is considered the therapeutic measure of choice when UNE is a consequence of anomalous elbow, and

A number of musculo-tendinous variations around the medial epicondyle have been alleged to cause ulnar nerve compression. Subluxation of the ulnar nerve, a "snapping" separate medial head of triceps, a prominent medial head of triceps covering the ulnar nerve, anconeus epitrochlearis, Osborne's band, and the ligament of Struthers have been implicated. We present a case of clearcut compression of the ulnar nerve at two levels just at and posterior to the epicondyle by a tightly applied prominent head of the triceps, and at a more distal level beneath an anconeus epitrochlearis muscle.

Some cases of cubital tunnel syndrome are caused by anatomical abnormalities such as the epitrochleo-anconeus muscle or snapping and bulkiness of the medial head of the triceps brachii muscle. We report a rare cause of cubital tunnel syndrome that has not been reported previously. It was caused by an abnormal insertion of the medial head of the triceps muscle into the medial epicondyle. The clinical course and operative findings are described.

BACKGROUND: Nerve compression underlying carpal tunnel syndrome (CTS) results in an increase in the threshold of superficial sensation in the area supplied by the median nerve, which is a mixed nerve dominated by sensory fibres. The distribution of sensory symptoms is strongly dependent on the degree of electrophysiological dysfunction of the median nerve. The association between carpal tunnel syndrome and ulnar nerve entrapment at wrist level is still unclear. Patho-logical processes leading to median neuropathy in CTS may affect ulnar nerve motor and sensory fibers in the Guyon canal. This m

The objective of this paper is to present a case of rare location of schwannoma in the medial cutaneous nerve of the arm at the level of the axilla. Preliminary diagnostic examination of the tumour was carried out in another hospital by means of open biopsy. In the preoperative period symptoms of ulnar nerve dysfunction (paraesthesias, positive Hoffmann-Tinel sign) dominated the clinical picture. After having performed imaging studies and electromyographic (EMG) examination, the patient was scheduled for an operation. The tumour, measuring 3.5 × 3.0 × 1.5 cm, was resected without damage to the

Ulnar nerve entrapment at the elbow is the second most common upper limb entrapment neuropathy other than carpal tunnel syndrome. There have been many causes identified ranging from chronic aging joint changes to inflammatory conditions or systemic disorders. Among them, uncommon anatomical variants accounts for a small number of cases. Here, we report our experience in managing ulnar nerve entrapment caused by a rare vestigial structure, anconeus epitrochlearis ligament, and provide a brief review of the literature of its management.
