A 76-year-old diabetic woman was referred to our hospital with an episode of high fever and sub-abdominal pain. Computed tomography (CT) of the pelvis revealed gas accumulation within the lumen and wall of the bladder and CT of the abdomen demonstrated bubbles in the inferior vena cava. She recovered by urinary drainage and antibiotic therapy. Urinary culture revealed Escherichia coli. CT after the therapy didn't demonstrate gas accumulation of the bladder and bubbles in the inferior vena cava. Emphysematous urinary tract infections (UTIs) have the high fatality rate, it seems to be a possibil

INTRODUCTION: Inferior vena cava (IVC) interruption was established as a procedure to treat refractory venous thromboembolism (VTE) complicated by pulmonary embolism. Ilio-caval thrombosis and lower limb chronic venous insufficiency (CVI) are well known long-term complications of IVC interruption, where subsequent treatments may carry significant morbidity and mortality.
PRESENTATION OF CASE: We present here a case of chronic venous insufficiency resulting from IVC interruption with a vascular clip placed forty years previously. A novel approach utilising endovascular stents was used to recon

The authors analysed the results of examination and dynamic follow up (from 2003 to 2011) of twenty-one 15-to-55-year-old male patients presenting with hypoplasia or aplasia of the inferior vena cava. The diagnosis was verified by means of SCT- or MRT-phlebography. The disease was newly diagnosed at the age of 15-55 years (mean 25.9±2.6). The pathology manifested itself as a clinical course of thrombosis of deep veins of lower limbs in 16 (76.2%) patients and by oedema thereof in 5 (23.8%) subjects. Of the 16 patients with symptomatology of deep vein thrombosis, 13 patients had proximal (iliof

BACKGROUND: Temporary vena cava filters have been used for protection from potentially fatal pulmonary embolism. However, recent reports suggested that they may be associated with serious adverse complications including filter-related thrombosis. The purpose of this study was to examine the clinical complications of temporary vena cava filter placement.
METHODS: We enrolled 40 consecutive patients from January 2006 to December 2010 who underwent percutaneous temporary vena cava filter insertion in Saitama Medical Center, Jichi Medical University.
RESULTS: Major filter complications related t

AIM: To evaluate the results of an aggressive surgical approach of resection and reconstruction of the inferior vena cava (IVC).
METHODS: The approach to caval resection depends on the extent and location of tumor involvement. The supra- and infra-hepatic portion of the IVC was dissected and taped. Left and right renal veins were also taped to control the bleeding. In 12 of the cases with partial tangential resection of the IVC, the flow was reduced to less than 40% so that the vein was primarily closed with a running suture. In 3 of the cases, the lumen of the vein was significantly reduced,

A 85-year old patient with an history of low grade follicular lymphoma was admitted in the hospital with a superior vena cava (SVC) syndrome. The computed tomography scan of the thorax and the trans-esophageal echocardiography revealed a voluminous mass in the right atrium, extending to the origin of the SVC. A transvenous biopsy was done under echocardiographic control. The immunohistology showed a diffuse large B cell lymphoma. He received chemotherapy associating rituximab, doxorubicine, cyclophosphamide, vincristine and prednisone. The clinical response after the second cycle was remarkabl

HISTORY AND FINDINGS: A previously healthy 43-year-old woman was admitted because of pain in the lower abdomen. Abdominal and transvaginal ultrasound revealed a cystic structure in the right ovary, providing the indication for an exploratory laparoscopy. A hemorrhagic corpus luteum was enucleated. Laboratory tests had merely shown a raised C-reactive protein level and marginally elevated leukocytes. A family history of thrombotic episodes had been elicited.
FURTHER COURSE, DIAGNOSIS AND TREATMENT: Although heparin had been applied increasing swelling developed in both thighs and lower legs at

The superior vena cava syndrome (SVCS) comprises various symptoms due to occlusion of the SVC, which can be easily obstructed by pathological conditions (eg, lung cancer, due to the low internal venous pressure within rigid structures of the thorax [trachea, right bronchus, aorta]). The resulting increased venous pressure in the upper body may cause edema of the head, neck, and upper extremities, often associated with cyanosis, plethora, and distended subcutaneous vessels. Despite the often striking clinical presentation, SVCS itself is usually not a life-threatening condition. Currently, rand

This was a retrospective study of 144 patients with retrievable inferior vena cava (IVC) filters inserted between 2004 and 2008 at a community/teaching hospital. The purpose was to evaluate the incidence of complications and the rate and success of retrieval. Retrieval of IVC filters was attempted in 14 of 144 (10%) patients at an average of 4.6 months. Retrieval was successful in 10 of 14 (71%). Within 6 months of insertion, retrieval was successful in 10 of 12 (83%). Unsuccessful attempts were at 3, 6, 8 and 9 months. Non-bleeding complications of IVC filters occurred in 12 of 144 (8.3%). Ha

Patency of the left superior vena cava (persistent left superior vena cava--PLSVC), although uncommon, is the most frequent anomaly of the thoracic vessels. It has been diagnosed in 0.3-0.5% of the general population. PLSVC results from the abnormal persistence of an embryological vessel that normally involutes into the coronary sinus during fetal life. Catheterization of this vessel, including hemodialysis (HD) catheter placement, appears to be safe if adequate drainage of the PLSVC to the right atrium exists. We present the case of a 50-year-old HD patient in which the placement of a cathete

CASE REPORT: A 59-year-old man with a 4-week history of dyspnea, coughing, and chest discomfort was referred to hospital for further evaluation. Moreover, he reported fever and fatigue. There were neither cardiovascular risk factors nor drug medication. 6 months earlier, a deep vein thrombosis of his left lower limb was diagnosed followed by an anticoagulation for 4 weeks. Physical examination revealed a dilatation of the neck veins with a present Kussmaul sign and a diastolic murmur at the left lower sternal border. The findings on the rest of his physical examination were unremarkable. Elect

We report the case of a 70-year-old male with a complication of misplacement of a vena cava filter into the spinal canal. This likely happened as a result of penetration of the wire and filter sheath through the iliac vein or vena cava into the retroperitoneum, vertebral foramina, and spinal canal at the level of L2 and L3. Due to the patient's condition, the filter was not removed and no neurologic symptoms have occurred. This represents the first reported case of a filter deployment into the spinal canal. Although placement of vena cava filters is a relatively safe procedure, complications a

Venous aneurysms are uncommon. Despite their infrequency, venous aneurysms can present with significant clinical complications such as thrombosis, pulmonary embolism, and death. In this report, we present the case of a thrombosed inferior vena cava aneurysm discovered in a 16-year-old male who had deep vein thrombosis of the right lower extremity. Thrombosis of the inferior vena cava is uncommon in the pediatric population. Therefore, congenital abnormalities such as an inferior vena cava aneurysm should be considered when evaluating pediatric patients who present with deep vein thrombosis.

PURPOSE OF REVIEW: To summarize currently available literature regarding indications for inferior vena cava filters, potential problems associated with the different filters available, and the efficacy and safety of retrievable inferior vena cava filters.
RECENT FINDINGS: The placement of permanent filters may present a number of long-term complications such as filter occlusion and an increased risk of recurrent deep vein thrombosis. Furthermore, patients who require inferior vena cava interruption often have short-term contraindications to anticoagulant therapy, and thus only require filters
