PURPOSE OF REVIEW: Venom immunotherapy has proven to be a very effective method for the prevention of future re-sting reactions. However, initiation of the venom injection program is just the beginning. This review looks at recent papers which shed light on other issues that arise during maintenance venom immunotherapy.
RECENT FINDINGS: Prophylactic antihistamines taken before venom injections reduce the frequency of reactions and one report suggests that they may improve efficacy. The schedule of venom injections usually does not have to be adjusted for patients who develop local reactions;

Research into venoms have been extensively developed during the last years, and some groups of venomous animals seldom studied have been recently the subject of new important results, such as the Cones (Gasteropodes, Molluscs) and Spiders, in the venom of which new types of neurotoxic molecules have been described. Concerning the most studied groups, new toxic molecules have been identified in snake (Dendroaspis sp., Atractaspis sp.) and scorpion venoms. The scorpion venoms contain a large variety of toxins acting on various ionic channels, Na+, K+, Ca++ or Cl-. Many structural relations betwe…

Systemic allergic reactions to hymenoptera stings are most often mediated by venom-specific IgE antibodies. The most frequent symptoms are urticaria, angioedema, asthma and anaphylactic shock. Rarely, cerebral or myocardial ischemia with permanent damage may result from severe anaphylactic shock. Each year several individuals die in Switzerland from allergic reactions following hymenoptera stings. The most valuable diagnostic tests are skin tests and estimation of venom-specific serum-IgE antibodies. The most important drugs for emergency treatment are adrenaline and antihistamines. During ana

Cardiac function was evaluated by serial echocardiography in 30 children affected by scorpion stings. They could be separated into two groups on the basis of the initial echocardiogram. Group 1, consisting of 18 children, had normal left ventricular function, whereas group 2, consisting of 12 children, showed compromised left ventricular function (ejection fraction less than 0.55; fractional shortening less than 27%). In group 2 the left ventricular end-systolic dimension was increased significantly and the interventricular septal thickening fraction was depressed significantly, compared with

Snake venoms are complex mixtures mainly composed of proteins and small peptides. Crotoxin is one of the most studied components from Crotalus venoms, but many other components are less known due to their low abundance. The venome of Crotalus durissus terrificus, the most lethal Brazilian snake, was investigated by combining its venom gland transcriptome and proteome to create a holistic database of venom compounds unraveling novel toxins. We constructed a cDNA library from C. d. terrificus venom gland using the Illumina platform and investigated its venom proteome through high resolution liqu…

Venom immunotherapy (VIT) with Hymenoptera venom extracts is highly effective in preventing large local, systemic allergic, and anaphylactic reactions to insect stings. VIT is not required for patients with cutaneous systemic or large local allergic reactions to stings because it is uncommon for reactions to become more severe. The clinical history, with confirmatory skin or serum tests for venom IgE, can clarify the risk for future anaphylaxis and the need for VIT. For initial treatment, rush regimens are recommended because they have the same or less risk of systemic reactions than slower tr

INTRODUCTION: As an ecological adaptation venoms have evolved independently in several species of Metazoa. As haematophagous arthropods ticks are mainly considered as ectoparasites due to directly feeding on the skin of animal hosts. Ticks are of major importance since they serve as vectors for several diseases affecting humans and livestock animals. Ticks are rarely considered as venomous animals despite that tick saliva contains several protein families present in venomous taxa and that many Ixodida genera can induce paralysis and other types of toxicoses. Tick saliva was previously proposed…

Allergic reactions to Hymenoptera stings usually present as large local reactions or systemic reactions with symptoms of immediate type allergy (anaphylaxis). In Central Europe they are predominantly elicited by stings of the honeybee or Vespula spp. Acute reactions are managed by symptomatic treatment. Long-term care includes patient education (allergen avoidance, course of action at re-sting) and prescription of an emergency kit for self-treatment. Venom immunotherapy is established as specific treatment for Hymenoptera venom allergic patients. Diagnosis of Hymenoptera venom anaphylaxis is b

Systemic IgE-mediated immediate type reactions (anaphylaxis) due to honeybee or vespid stings are potentially life-threatening; they are reported in up to 5% of the general population. Insect venom allergy is diagnosed by history, skin testing and measurement of insect venom-specific serum IgE; sometimes additional tests are needed. The diagnosis is based on the history of a systemic allergic immediate type sting reaction, without such a medical history any other "positive" test results are irrelevant. Nearly always, patients with systemic allergic sting reactions can be protected from further
