Long-term follow-up after mitral valve reconstruction: incidence of postoperative left ventricular outflow obstruction.

Reconstructive surgery of the mitral valve has been an alternative to mitral valve replacement in patients with mitral regurgitation. Previously, we reported on postoperative left ventricular outflow tract obstruction associated with systolic anterior motion of the anterior mitral leaflet. The current study was designed to evaluate the incidence of this complication and the long-term results of mitral valve reconstructive surgery. Sixty-five patients, aged 19 to 78 years, had mitral valve reconstructive surgery. Two patients died perioperatively, and three died late after surgery. The 60 survi

Late development of left ventricular outflow tract obstruction after repair of double-outlet right ventricle.

A 22-year-old woman had exertional chest pain and dyspnea secondary to left ventricular outflow tract obstruction more than a decade after repair of a type I double-outlet right ventricle. The obstruction was the result of (1) failure of the ventricular septal defect and the tunnel created between the left ventricle and aorta to enlarge commensurate with growth of the heart and (2) a markedly hypertrophied parietal band of the crista supraventricularis which surrounded and narrowed the aortic orifice. At reoperation, the ventricular septal defect and the tunnel were enlarged and the hypertroph

Trileaflet Mitral Valve as Unusual Contributor to Left Ventricular Outflow Tract Obstruction in Hypertrophic Cardiomyopathy.

BACKGROUND: Mitral valve abnormalities are increasingly recognized as contributors to left ventricular outflow tract obstruction (LVOTO) in obstructive hypertrophic cardiomyopathy (oHCM). Although trileaflet mitral valves have been reported previously in patients with oHCM, this case demonstrates how detailed segmental echocardiographic phenotyping identified distinct mechanisms of dynamic LVOTO, enabling an individualized repair. CASE SUMMARY: A 48-year-old man with oHCM was found to have a trileaflet mitral valve contributing to LVOTO in addition to systolic anterior motion. DISCUSSION: Ca

Progressive Subvalvular Right Ventricular Outflow Tract Obstruction With a Single Coronary Artery in an Adult.

CASE SUMMARY: A 57-year-old man with a history of ventricular septal defect and prior infective endocarditis presented with progressive exertional dyspnea. Multimodality imaging demonstrated severe subvalvular right ventricular outflow tract (RVOT) obstruction with a marked intraventricular pressure gradient (190 mm Hg proximal right ventricle vs 15 mm Hg pulmonary artery). Coronary angiography revealed a single coronary artery arising from the left coronary sinus (Lipton type L1), with a branch crossing anterior to the RVOT. DISCUSSION: Progression of obstruction suggested evolving double-ch

Complicated third sternotomy for left ventricular outflow tract obstruction secondary to mitral valve prosthesis strut.

Introduction: LVOT obstruction after surgical bioprosthetic MVP placement is uncommon and the influence of the MVP strut width has seldom been examined. Case presentation: A 74-year-old female presented with palpitations, chest pain, and shortness of breath. She had MVR with 31-mm bioprosthetic valve 2 years prior. TEE confirmed LVOT obstruction secondary to a MVP strut, causing a peak gradient of 55 mmHg. The redo operation was complicated by iatrogenic Type A aortic dissection. The patient was immediately cooled for DHCA with RCP and ascending aortic replacement was performed. MVR was then p

Predictors and prognostic impact of dynamic left ventricular outflow tract obstruction during dobutamine stress echocardiography.

BACKGROUND: Dynamic left ventricular outflow tract obstruction (DLVOTO) is a recognized phenomenon during dobutamine stress echocardiography (DSE), but its predictors and long-term prognostic significance remain uncertain. OBJECTIVES: To identify clinical and echocardiographic predictors of DLVOTO during DSE and to assess the association between DLVOTO and major adverse cardiovascular events (MACE). METHODS: We conducted a retrospective, single-center study included 355 consecutive patients undergoing DSE for ischemia assessment. DLVOTO was defined as a peak left ventricular outflow tract gr

Haemodynamic response to exercise in patients with hypertrophic cardiomyopathy without left ventricular outflow tract obstruction.

BACKGROUND: Dyspnoea and reduced exercise capacity are common features in hypertrophic cardiomyopathy (HCM) without left ventricular outflow tract (LVOT) obstruction, yet the haemodynamic mechanisms contributing to these symptoms are poorly described. This study aimed to characterise the haemodynamic response to exercise in patients with HCM without LVOT obstruction. METHODS: In this observational study, 121 subjects underwent right heart catheterisation with haemodynamic measurements during rest and supine incremental exercise: 59 patients with HCM without LVOT obstruction (58±12 years) and

Detection of Left Ventricular Outflow Obstruction From Standard B-Mode Echocardiogram Videos Using Deep Learning.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) affects 20 million individuals globally, with increased risk of sudden death and heart failure. Although cardiac myosin inhibitors show great promise as disease-specific treatment, current indications are for obstructive HCM. Obstruction is not always well characterized by echocardiography. Artificial intelligence might assist in the improving the underdiagnosis of left ventricular outflow tract (LVOT) obstruction. OBJECTIVES: The authors aimed to develop a deep learning model to detect LVOT obstruction from non-Doppler B-mode echocardiography. M

Fabry cardiomyopathy presenting as hypertrophic phenotype with left ventricular outflow tract obstruction: a case series.

BACKGROUND: Fabry disease is a cardiomyopathy with multisystemic manifestations that is easily to be misdiagnosed as another type of cardiomyopathy. Fabry cardiomyopathy may present with left ventricular outflow tract obstruction (LVOTO), mimicking obstructive hypertrophic cardiomyopathy (oHCM). We present three cases of genetically confirmed Fabry disease that were initially misdiagnosed as oHCM, highlighting the importance of early recognition and tailored management. METHODS: We performed Fabry disease screening on patients diagnosed with oHCM and ultimately identified three individuals wh

Radiofrequency Septal Reduction After Transcatheter Mitral Valve Replacement to Treat Left Ventricular Outflow Tract Obstruction.

BACKGROUND: Transcatheter mitral valve replacement (TMVR) has emerged as an important treatment for mitral stenosis. Radiofrequency catheter ablation has been shown to be effective as a pre-emptive strategy to mitigate left ventricular outflow tract (LVOT) obstruction. CASE SUMMARY: A 78-year-old man undergoes a TMVR. He develops significant LVOT obstruction. Radiofrequency ablation is used as a therapeutic procedure. DISCUSSION: LVOT obstruction is an important complication of TMVR. Various strategies have been used preoperatively to prevent the development of LVOT obstruction. Although rad

Percutaneous stenting for late outflow graft obstruction in left ventricular assist device: A case report.

Left ventricular assist device (LVAD) outflow graft obstruction (OGO) is an uncommon but serious late complication requiring individualized treatment. We describe a 71-year-old HeartMate 3 recipient with long-segment OGO stenosis more than 3 years after implantation. Because surgical revision was high risk, overlapping self-expanding venous stents were implanted percutaneously, with brief LVAD deactivation enabling controlled proximal deployment near the pump. Imaging confirmed restored graft patency and improved flow. A later distal thrombotic stenosis was managed conservatively with intensif

Percutaneous endovascular intervention for left ventricular assist device outflow graft obstruction: A single-center experience.

BACKGROUND: Outflow graft obstruction (OGO) is an uncommon yet life-threatening complication in patients with left ventricular assist devices (LVADs). In this retrospective, single-center case series, we identify the baseline demographics and presenting features of patients who develop LVAD OGO and the procedural details and outcomes surrounding percutaneous endovascular intervention (PEI). METHODS: We conducted a retrospective review of patients with LVADs at our institution between January 2010 and February 2023 who developed OGO and were treated with PEI. Details of the PEI including proce

Prevention of left ventricular outflow tract obstruction in transapical mitral valve replacement: the MitraCut procedure.

BACKGROUND: The MitraCut procedure employs beating heart transapical (TA) cannulation and endoscopic scissors for dividing the anterior mitral leaflet (AML) to prevent left ventricular outflow tract (LVOT) obstruction in transapical transcatheter mitral valve replacement (TA-TMVR). AIMS: We present the first multicentre experience of the MitraCut procedure prior to TA-TMVR to prevent LVOT obstruction. METHODS: In 6 European centres, the clinical outcomes of all 13 high-risk patients who had undergone the MitraCut procedure during TA-TMVR procedures were retrospectively reviewed regarding tec

Large ventricular myxoma causing inflow and outflow obstruction of the right ventricle; A Case Report.

BACKGROUND: Myxomas are the most common primary benign heart tumors, typically found in the left atrium, with only 2-4% occurring in the right ventricle. Clinical presentations vary widely, including congestive heart failure and systemic embolic phenomena. This case report describes a rare right ventricular myxoma causing both inflow and outflow obstruction, presenting as progressive exertional dyspnea. CASE PRESENTATION: A 23-year-old male presented with two weeks of worsening exertional dyspnea. He was stable but tachypneic with a systolic murmur over the tricuspid area. Elevated erythrocyt

Transannular bovine jugular vein monocusp for the reconstruction of severe right ventricular outflow tract obstruction.

OBJECTIVE: Severe right ventricular outflow tract obstruction in tetralogy of Fallot and variants necessitates the use of transannular patch in a significant proportion of children undergoing repair. We have used a Contegra monocusp together with delamination of native leaflet tissue in order to create a functioning pulmonary valve. METHODS: In total, 18 (2017-2022) consecutive Contegra monocusp implantations were included. Median age and weight were 3.65 [2.00; 9.43] months and 6.12 [4.30; 8.22] kg, respectively. Nine of 18 patients had undergone palliation. Native pulmonary leaflet tissue w

A surgical mouse model of neonatal right ventricular outflow tract obstruction by pulmonary artery banding.

BACKGROUD: Diseased animal models play an extremely important role in preclinical research. Lacking the corresponding animal models, many basic research studies cannot be carried out, and the conclusions obtained are incomplete or even incorrect. Right ventricular (RV) outflow tract (RVOT) obstruction leads to RV pressure overload (PO) and reduced pulmonary blood flow (RPF), which are 2 of the most important pathophysiological characteristics in pediatric cardiovascular diseases and seriously affect the survival rate and long-term quality of life of many children. Due to the lack of a neonatal
