[Dissections of the vertebral artery following cervical chiropractic manipulations].

In a 37-year-old female patient complaining of increasing pain in the neck and occiput, chiropractic manipulations at the cervical vertebral column were associated with ischaemias of the brain stem presenting as vertigo, transient "locked-in" syndrome followed by vomiting, and sensorimotor hemiparesis. Digital subtraction angiography (DSA) revealed complete obstruction of the right and slight dissection of the left vertebral artery. The symptoms receded within a few days after heparinisation with 1000 IU/h intravenously. A 39-year-old female patient developed vertigo, nystagmus, tetraparesis a

[Dissecting aneurysm of the vertebral artery as a cause of Wallenberg's syndrome].

Although it is well known that Wallenberg's syndrome is caused by occlusion of the vertebral artery (VA) or the posterior inferior cerebellar artery (PICA), the etiology of the occlusion is rarely documented. During the course of Wallenberg's syndrome, patients often complain of headache. We thought that these headaches might be caused by dissecting aneurysm (DA) of the vertebral artery, and so we studied the incidence of DA in our cases with Wallenberg's syndrome. Although many variants exist, Wallenberg's syndrome encompasses several neurological symptoms due to a disorder of the nucleus and

Nontraumatic dissecting aneurysm of the vertebral artery. Case report.

A case is described of a nontraumatic dissecting aneurysm of the vertebral artery, which presented as a subarachnoid hemorrhage (SAH). Differentiation from vasospasm and from atherosclerosis is critical. Dissection between the adventitia and media should be noted in the differential diagnosis of SAH when an aneurysm or vascular malformation is not demonstrated angiographically.

[Wallenberg's syndrome due to a dissecting aneurysm of the vertebral artery].

A 54 year old man without pathologic past history but mild hypertension, obesity and gastric ulcer, presented with a syndrome of Wallenberg. He had complained for five days of progressive and diffuse headache. The neurological condition improved initially, but the patient died suddenly two weeks later. Pathological examination showed no significant alteration except for left ventricular enlargement and mild arteriosclerosis. There was a hemodissection (dissecting aneurysm) of the left vertebral artery next to the inferior oliva. It induced a lateral infarct and a limited dorsal infarct at the

Development of a dissecting aneurysm on the vertebral artery immediately after occlusion of the contralateral vertebral artery: a case report.

A 49 year old female presented with subarachnoid hemorrhage due to a ruptured dissecting aneurysm on the left vertebral artery (VA). Following an occlusion test, we performed proximal occlusion of the left VA with detachable balloons. However, a dissecting aneurysm on the right VA developed three weeks later. After an occlusion test had showed no change in cerebral blood flow, auditory brain stem response, or neurological status, proximal occlusion of the right VA was performed. The patient has returned to normal life without neurological deficits. Bilateral dissecting aneurysms of the VA are

Vertebral artery occlusion after subarachnoid hemorrhage from a dissecting aneurysm of the vertebral artery: case report.

BACKGROUND: Generally speaking, occlusion of the vertebral artery is a finding of a dissecting aneurysm associated with completed stroke. We present a case of a vertebral dissecting aneurysm that produced subarachnoid hemorrhage (SAH). Angiography on the day of hemorrhage, however, demonstrated complete occlusion of the vertebral artery. CASE PRESENTATION: A 44-year-old hypertensive woman suffered a sudden onset of headache and vomiting followed by loss of consciousness due to SAH. Angiography on the day of hemorrhage revealed a complete occlusion of the right vertebral artery just distal to

Occlusion of the vertebral artery secondary to dissection of the subclavian artery--case report.

A 34-year-old man presented with occlusion of the left vertebral artery (VA) secondary to dissection of the left subclavian artery manifesting as vertigo, nausea, vomiting, and neck pain. On admission, he was alert with left limb and truncal ataxia. Magnetic resonance (MR) imaging and MR angiography showed left cerebellar infarction and occlusion of the left VA. Conventional angiography and three-dimensional computed tomography (3D-CT) angiography showed stenosis with thrombosed pseudo-lumen of the left subclavian artery, and occlusion of the left VA. Presumably the idiopathic dissection of th

Growth and regression of an intracranial vertebral artery dissecting aneurysm.

Intracranial vertebral artery dissecting aneurysms (VADAs) are rare vascular abnormalities with diverse presentations and unpredictable natural histories. Traditionally considered aggressive lesions with high mortality, emerging evidence has suggested some unruptured cases may undergo stabilization or even regression. This report details a 47-year-old patient presenting with ataxia and neck pain following a presumed traumatic dissection, leading to a diagnosis of a right vertebral artery dissection with mural hematoma formation. Serial imaging over two-years demonstrated progressive aneurysmal

Atraumatic Vertebral Artery Dissection in a Patient With a Migraine Headache.

This case discusses a 34-year-old active duty male who presented to the emergency department with a 2-week persistent headache. His initial review of symptoms was reassuring until a detailed neurologic examination on his second visit revealed a visual deficit in the left upper quadrant. Additionally, he complained of intermittent tension headaches for the last several years but had no history of diagnosed migraines until he was seen 4 days prior for empiric migraine therapy in the same emergency department and left without improvement in symptoms. On his return visit, computerized tomography s

[Endovascular management and classification of the dissecting aneurysms of the vertebral artery].

Objective: To summarize the classifications and optimize endovascular treatment of the dissecting aneurysms of the vertebral artery (DAVA). Methods: The clinical information of 39 cases of DAVA treated with endovascular methods in our department between January 2007 and September 2016 were analyzed retrospectively.According to the location of the aneurysm in relation to the posterior inferior cerebellar artery (PICA), the aneurysms were classified into three types: type pre-PICA, located proximally to the PICA; type in-PICA, located at the PICA origin; and type post-PICA located distally to th

A proposed dissection procedure for vertebral arteries in forensic pathology.

Vertebral artery removing constitutes a significant forensic pathology challenge. Dissection techniques during head-neck autopsy are based on anterior approach, a difficult method, which is unable to assess the transverse part of the artery. This work presents an original and simple method for dissecting vertebral arteries by a posterior approach, opening the vertebroarterial canal through the spinal canal without any special equipment. Once the spinal cord is removed, the transversarium foramens are opened by an internal cut at the pedicle and an external cut at the transverse process. This e

Dissecting vertebral artery aneurysm: diagnosis and coil embolization.

Dissecting aneurysm of the intradural vertebral artery, a rare cause of stroke and sub-arachnoid haemorrhage, is associated with a poor neurological outcome if the disease is unrecognized and remains untreated. Clinical manifestations may be delayed or overlooked and non-invasive radiological investigations demonstrate little or no abnormality. We describe the diagnosis and successful endovascular occlusion of a dissecting vertebral artery aneurysm using detachable microcoils; a novel technique that has advantages over conventional therapeutic procedures.

Dissection of the carotid and vertebral arteries: imaging with MR angiography.

Arterial dissection occurs when an intimal tear allows blood to enter the arterial wall, potentially compromising the lumen and reducing blood flow. Carotid and vertebral artery dissections typically occur after major trauma, although they also can arise spontaneously or after trivial injury. Arterial dissection has been associated with a variety of factors, including hypertension, fibromuscular dysplasia, Marfan syndrome, cystic medial necrosis, oral contraceptives, drug abuse (sympathomimetics), and infection [1-8]. It is important to recognize arterial dissection early so that prompt treatm
