Vestibular nerve disorder (VND) in cases suffering from sudden deafness with vertigo was studied. Between 1987 and 1991, we observed 46 patients suffering from sudden deafness with or without vertigo, who had undergone the caloric test and galvanic body sway test (GBST). Twenty-seven patients had vertigo and canal paresis (CP) upon the caloric test, 6 vertigo without CP and 13 CP without vertigo. VND was detected in 12 of the 46 patients. All those with VND had vertigo and CP upon the caloric test, 12/27 (44%). The degree of hearing loss in those with VND was significantly (p < 0.05) more seve

UNLABELLED: Recently, vestibular anomalies have been described as a frequent feature in children with coloboma-heart-atresia-retarded-genital-ear (CHARGE) syndrome. They are likely to play an important role in the psychomotor retardation affecting these children. In order to test this hypothesis, we prospectively performed complete vestibular investigations in a series of 17 CHARGE syndrome patients including inner ear CT scan and functional vestibular evaluation of both canal and otolith functions. These results were correlated with the postural anomalies observed during the children's develo

The incidence of inferior vestibular nerve disorders in patients suffering from unilateral vestibular neuritis and the recovery of these disorders were evaluated by monitoring the vestibular-evoked myogenic potential (VEMP). Eight patients ranged from 21 to 73 years that suffered from unilateral vestibular neuritis underwent VEMP and caloric testing. Abnormal VEMP was observed in two of the eight patients with unilateral vestibular neuritis. Two patients were diagnosed as having an inferior vestibular nerve disorder. One of these patients showed recovery of the inferior vestibular nerve functi

The compensatory process of vestibular neuronitis in 7 patients was followed up and evaluated using the Combined Galvanic Test (CGT) and other neuro-otological data. CGT is the simultaneous recording of both galvanic eye movement (GEM) and galvanic body sway (GBS), and has proved to be reliable in the diagnosis of retrolabyrinthine disorders. The results were as follows: The subjective symptoms remained in 3 cases from 1 month to 1 year. The recovery of the GBS response was seen earlier than that of both GEM and caloric responses. The recovery of GEM and caloric responses was observed at almos

A survey of 265 Japanese patients with Meniere's disease revealed bilateral involvement in approximately 29% of all these patients. Somatic and psychiatric aspects of bilateral Meniere's disease were as follows. (1) The duration of the disease in cases of bilateral involvement was significantly longer than cases of unilateral involvement. (2) An abnormal general condition, a more extensive degree of hearing loss and neurotic type were frequently diagnosed in cases of bilateral involvement. Based on these results, it was concluded that psychiatric management and prevention of aggravation of dea

Testing vestibular evoked myogenic potentials (VEMPs) may be the most important new clinical test for evaluation of vestibular function developed during the past 100 years since the introduction of the caloric test. VEMPs are easily recordable and therefore suitable for everyday testing in clinical neurotology. VEMPs in response to air-conducted sound stimulation using surface electrodes over the sternocleidomastoid muscles reveal saccular function, inferior vestibular nerve function, and vestibulocollic connections. At present, VEMPs are of clinical importance for estimating the severity of p

PURPOSE: Body balance involves the vestibular, visual, and proprioceptive systems. IGF-I is a GH-dependent key factor in the development and postnatal differentiation of the inner ear in mice and men, but its role in the vestibular function in adult humans is unknown. We have previously described a cohort of individuals with severe isolated GH deficiency (IGHD) caused by a mutation in the GHRH receptor (GHRHR) gene. These individuals complain of dizziness, exhibit mild sensorineural loss, but have normal postural balance, without increase in falls risk. The aim of this study was to evaluate th

BACKGROUND: Differential diagnosis of acute vertigo syndrome is challenging given the similarities between clinical presentations of posterior circulation stroke and peripheral vestibular dysfunction. The Head Impulse, Nystagmus, Test of Skew ('HINTS') assessment is a clinical bedside test used to aid diagnosis.
METHODS: Comprehensive training on use of the Head Impulse, Nystagmus, Test of Skew assessment was provided to one stroke consultant, and the effectiveness of the test in that setting was assessed. Further education was completed with more members of the stroke and emergency departmen

We report on the preoperative findings in 147 patients with surgically confirmed acoustic neuroma (AN). Gaze nystagmus, spontaneous nystagmus and positional nystagmus were present in 23.1%, 47.5% and 63.0% of the patient, respectively. Pathological findings in ETT, OKP and bithermal caloric test were found in 44.3%, 45.3% and 77.0%, respectively. The ratio of pathological findings in ETT and OKP was larger in the large tumor group than in the small tumor group. Gaze nystagmus to both directions and direction-changing positional nystagmus were also found in the large tumor group. While the corr

We describe the treatment of two patients with peripheral vestibular dysfunction using a novel, staged exercise program. Response to treatment was documented. The first patient, a 62-year-old woman with unilateral vestibular dysfunction (UVD) and a 6-month history of disequilibrium following herpes zoster oticus resulting in damage to the right inner ear, was treated with an 8-week course of vestibular physical therapy. During the 8 weeks, the patient attended weekly physical therapy sessions and was trained to perform vestibular adaptation exercises on a daily basis at home. The second patien

Vestibular evoked myogenic potential (VEMP) has been applied for patients with vestibulo-cochlear disorders. The impairment of the sound transmission due to middle ear pathology affects VEMP results. In children, otitis media with effusion (OME) is well documented and it is difficult to apply conventional VEMP in such cases. To overcome the attenuation of stimulation due to middle ear pathology, VEMP by bone-conducted sound has been developed. We report a 3-year-old girl with vestibular neuritis and OME as a representative case of clinical application of VEMP by bone-conducted sound. VEMP by b

OBJECTIVE: Otolith is a critical component of the peripheral vestibular system responsible for detecting linear acceleration. Dysfunction of the otolith organs can lead to vertigo, balance disturbances, and gaze instability. By elucidating the physiological foundations and evaluating both current and emerging methods for assessing otolith function, this review aims to provide a comprehensive resource for researchers and clinicians in the field of vestibular science, contributing to improved diagnostic accuracy and the development of more effective therapeutic strategies.

Caloric testing is the only method to examine the vestibular function of each labyrinth separately. When a perforated drum is present, caloric stimulation has to be achieved by air injection. In this study we examine the applicability and reliability of a simplified stimulator with Fréon. This comparative study proves it to be a reliable method for daily ENT practice.
