Combined PGI implantation and pars plana vitrectomy (PPV) provided effective intraocular pressure (IOP) control with an acceptable safety profile. These findings suggest that this combined approach represents a viable surgical option for challenging glaucoma cases requiring simultaneous vitreoretinal intervention. In complex cases of secondary glaucoma following vitreoretinal surgery (VR) where patients require both IOP control and treatment of coexisting vitreoretinal (VR) pathology, a combined surgical approach with VR surgery and glaucoma drainage devices (GDD) may be necessary. However, the

Michael T. Trese, MD (1946-2022), a vitreoretinal surgeon, made significant contributions to the field of retina. Although most known for his work in pediatric retina surgery, he was a pioneer in areas such as medical retina, translational research, and telemedicine. This article reviews his major contributions to spread his knowledge more widely to vitreoretinal trainees and specialists. We discuss six areas where Trese made a lasting impact: lens-sparing vitrectomy, familial exudative vitreoretinopathy, congenital X-linked retinoschisis, autologous plasmin enzyme, regenerative medicine, and

Vitreoretinal diseases involving age-related macular degeneration (AMD) are refractory to most topical or systemic drugs. The retina and the vitreous cavity have a unique position regarding pharmacokinetics in that the inner and outer blood retinal barriers separate the retina and vitreous from the systemic circulation. Eye drops achieve minimal therapeutic concentrations in the vitreoretinal tissue. Drug delivery systems are a strategy to address this. Intraocular sustained drug release using implantable devices has been investigated to treat vitreoretinal diseases. Possible targeted diseases

PURPOSE: When severe retinal and corneal injury occur together, a temporary keratoprosthesis (TKP) is often a last resort to allow posterior segment visualization to enable vitreoretinal surgery, followed by a penetrating keratoplasty (PKP) which can restore corneal clarity in a single operation. We aimed to assess visual outcomes following combined PKP and vitreoretinal surgery with the use of a TKP for cases of ocular trauma.
DESIGN: A systematic literature review was performed following PRISMA guidelines (PROSPERO registration number: CRD42023423518).
METHODS: CENTRAL, MEDLINE, Embase, IS

In this case report, we discuss the closure of a macular hole over a perfluorocarbon liquid (PFCL) globule in a silicone oil-filled eye. PFCL is an important adjunct in the armamentarium of a vitreoretinal surgeon. The use of PFCL has changed the management of various retinal pathologies. PFCL has increased the success rates of vitrectomies for retinal detachment associated with giant retinal tear and proliferative vitreoretinopathy. Recent literature has reported the use of PFCL in the management of macular hole with or without concurrent retinal detachment. A 60-year-old woman presented with

PURPOSE: To describe the case of a young boy affected by a rhegmatogenous retinal redetachment in whom the use of peripheral optical coherence tomography led the vitreoretinal surgeon to treat the patient with an additional surgery instead of a "watchful waiting" strategy.
METHODS: A 13-year-old boy was referred to the Vitreoretinal Department of the study institution affected by a macula-off rhegmatogenous retinal redetachment in his right eye secondary to a second blunt trauma. Three months before, he was treated with episcleral surgery and cryopexy as a result of a first traumatic rhegmato

Pathological changes in the diabetic macula are a frequent cause of visual loss. They can be classified as intraretinal or vitreoretinal maculopathies. The intraretinal ones are due to hyperpermeable retinal capillaries or retinal vascular shut-down. They appear clinically as macular edema, exudative maculopathy, fluorescein dye leak maculopathy, or ischemic maculopathy. When edema and exudation cause or threaten to cause significant visual loss they are treated by photocoagulation. The vitreoretinal maculopathies are due to traction resulting from (1) the formation of vitreoretinal adhesions

OBJECTIVE: To evaluate the surgical techniques and the efficacy for massive suprachoroidal hemorrhage (MSH).
METHODS: Secondary surgery performed on 11 cases of MSH occurring during or after intraocular surgery was delayed for 11 to 28 days (mean, 15.4 days). All eyes underwent posterior drainage sclerotomies under constantly maintained limbal fluid line pressure, followed by pars plana infusion and vitreoretinal surgery. The perfluorocarbon liquid was used intraoperatively in 6 cases.
RESULTS: The drainage of the choroidal hemorrhage was successful in all cases. The blood drained from supra

An 81-year-old man was taken to Accident & Emergency after a fall. CT brain imaging demonstrated a well-defined, homogenous, hyperdense mass in the posterior segment of the left eye. Reported as vitreous haemorrhage, an urgent ophthalmological assessment was instigated.Detailed history revealed previous vitreoretinal procedures for multiple retinal detachments. Ophthalmological examination confirmed presence of silicone oil in this eye.The appearance of silicone oil in the eye has become an increasingly prevalent finding given its continued use in complex vitreoretinal procedures since 1962. PURPOSE: Visual results following vitreoretinal surgery for stages 4 and 5 retinopathy of prematurity are often disappointing, even when anatomic results are good. This poses the question whether the surgery or the post-operative care causes the optic atrophy. A hypothesis is proposed that ocular perfusion pressure (mean blood pressure minus intraocular pressure) during or after surgery may be too low to provide adequate ocular blood flow.
METHODS: This report analyses the published results of retinopathy of prematurity surgery, the techniques used, as well as data about blood pressure and in

BACKGROUND AND OBJECTIVE: Corneal decompensation and complications are a frequent cause of visual loss after vitreoretinal surgery. This paper presents data regarding endothelial cell loss in aphakic and pseudophakic silicone oil filled eyes when oil was retained for many months. This study updates our previous investigation on the subject.
PATIENTS AND METHODS: The corneal endothelial cell count of 10 eyes of 10 consecutive patients who had undergone vitreoretinal surgery, including fluid-gas exchange and ultimately silicone oil placement, were obtained. The patients underwent an average of

INTRODUCTION: Asteroid hyalosis (AH) is characterized by mild vitreous-body liquefaction and a reduced likelihood of posterior vitreous detachment (PVD). Here, we report the clinical features of 2 cases of macular hole (MH) in eyes with AH. The extent of retinal vitreous adhesion at the time of vitreous surgery, the presence or absence of iatrogenic retinal breaks, and the postoperative course was examined in regard to the association with AH.
CASE PRESENTATION: Case 1 involved a 67-year-old female with decreased visual acuity in her left eye. Although preoperative optical coherence tomograph

The term "chromovitrectomy" has been coined to define the use of vital dyes in vitreoretinal surgery. The basic concept for the application of vital dyes during vitreoretinal surgery is to assist in highlighting preretinal membranes and tissues which are very thin and semitransparent and thus difficult to detect. Various dyes are currently being used in routine clinical procedures, however, the ideal staining agent has not yet been found. The vital dyes indocyanine green, infracyanine green, and brilliant blue stain the internal limiting membrane, trypan blue and triamcinolone acetonide help t
