How good is end of life care for patients dying with cystic fibrosis?

BACKGROUND: Cystic Fibrosis (CF) is a life-limiting illness. Audit of the care of patients dying of CF has not been published to date. METHODS: Newcastle and Oxford teams adapted the National Audit of Care at the End of Life and agreed additional questions that were particularly pertinent for patients dying as a consequence of their CF. Data were extracted and analysed for 15 patients. RESULTS: On recognition that the patient was dying, the CF teams were less good at reviewing the need for physiological observations (50% vs national 70%) but better at reviewing the need for capillary blood g

Identifying task-general effects of stimulus familiarity in the parietal memory network.

Studies of human memory have implicated a "parietal memory network" in the recognition of familiar stimuli. However, the automatic vs. top-down nature of information processing within this network is not yet understood. If the network processes stimuli automatically, one can expect repetition-related changes both when familiarity is central to an ongoing task and when it is task-irrelevant. Here, we tested this prediction in a group of 40 human subjects using fMRI. Subjects initially named 100 objects aloud in the scanner. They then repeated the same task with novel and previously-named object

Automatic recognizing of vocal fold disorders from glottis images.

The laryngeal video stroboscope is an important instrument to test glottal diseases and read vocal fold images and voice quality for physician clinical diagnosis. This study is aimed to develop a medical system with functionality of automatic intelligent recognition of dynamic images. The static images of glottis opening to the largest extent and closing to the smallest extent were screened automatically using color space transformation and image preprocessing. The glottal area was also quantized. As the tongue base movements affected the position of laryngoscope and saliva would result in unc

Albinism and human rights: From marginalisation to global advocacy.

BACKGROUND: Persons with albinism (PWAs) face significant biological, social and structural challenges, particularly in Africa, where entrenched cultural myths and systemic discrimination expose them to violence, stigma and exclusion. Despite increasing recognition of albinism as a disability under international human rights frameworks, PWAs remain marginalised across multiple domains. OBJECTIVES: This study examines the historical and contemporary experiences of PWAs, tracing the trajectory from marginalisation to advocacy. It seeks to contextualise persistent discrimination, analyse the eme

From where to what: a neuroanatomically based evolutionary model of the emergence of speech in humans.

In the brain of primates, the auditory cortex connects with the frontal lobe via the temporal pole (auditory ventral stream; AVS) and via the inferior parietal lobe (auditory dorsal stream; ADS). The AVS is responsible for sound recognition, and the ADS for sound-localization, voice detection and integration of calls with faces. I propose that the primary role of the ADS in non-human primates is the detection and response to contact calls. These calls are exchanged between tribe members (e.g., mother-offspring) and are used for monitoring location. Detection of contact calls occurs by the ADS

The impact of long-term botulinum toxin injections on symptom severity in patients with spasmodic dysphonia.

Injection of botulinum toxin (BT) into the muscles of the larynx is an effective treatment for the symptoms of spasmodic dysphonia (SD). To date, however, there have been no studies that have used blinded raters to assess the efficacy of BT injections on overall symptom severity in a cohort of SD patients followed over a prolonged period. In this study, 102 subjects with SD were treated with serial BT injections for up to 2 years. Voice recordings were obtained at several time points during treatment and were played to a panel of expert and novice listeners who rated them based on overall symp

Complications in the management of large glomus jugulare tumors.

This study analyzes the complications encountered in the surgical treatment of 17 patients with large glomus jugulare tumors. All 17 patients sustained either new cranial nerve palsies or exacerbation of preexisting palsies. These involved, in descending order of frequency, the facial nerve, the vagus and glossopharyngeal nerves, and the hypoglossal nerve. Postoperative palsies of the facial nerve involved 17 patients, as compared to 5 preoperatively. Fifteen patients had postoperative partial or complete paralysis of the vagus nerve as compared to eight preoperatively. Ten patients had postop

Wheeze Recognition Algorithm for Remote Medical Care Device in Children: Validation Study.

BACKGROUND: Since 2020, peoples' lifestyles have been largely changed due to the COVID-19 pandemic worldwide. In the medical field, although many patients prefer remote medical care, this prevents the physician from examining the patient directly; thus, it is important for patients to accurately convey their condition to the physician. Accordingly, remote medical care should be implemented and adaptable home medical devices are required. However, only a few highly accurate home medical devices are available for automatic wheeze detection as an exacerbation sign. OBJECTIVE: We developed a new

Automatic intraoperative optical coherence tomography positioning.

PURPOSE: Intraoperative optical coherence tomography (iOCT) was recently introduced as a new modality for ophthalmic surgeries. It provides real-time cross-sectional information at a very high resolution. However, properly positioning the scan location during surgery is cumbersome and time-consuming, as a surgeon needs both his hands for surgery. The goal of the present study is to present a method to automatically position an iOCT scan on an anatomy of interest in the context of anterior segment surgeries. METHODS: First, a voice recognition algorithm using a context-free grammar is used to

[Organic personality disorder: conceptual review and research strategies].

The historical review of "psychiatric personality disorders" reveals the lack of convergence of those disorders with the organic personality disorder (OPD). Only the psychopathy concept has been used as a psychopathological phenotype for one of the groups of OPD, the so-called "pseudopsychopaths". These patients have been described from the beginning of the XXth century under the heading of "frontal lobe syndrome". It was only with the development of the psychiatric nosologies, towards the middle of the XXth century, that the term "organic personality disorder" started to be used. The accumula

Amnesia can facilitate memory performance: evidence from a patient with dissociated retrograde amnesia.

A case is reported of a patient who experienced numerous episodes of transient global amnesia (TGA) during which anterograde amnesia was less prominent than usual, and who developed a permanent selective retrograde amnesia. On formal testing, he performed well on traditional verbal memory tests, but showed marked retrograde amnesia for verbal material, including items on a famous voices recognition test. He was administered a paired-associate learning test where the names of famous personalities for which he was amnesic were associated with incongruous activities (e.g. John Newcombe-singing).

Enhancing clinical documentation with voice processing and large language models: a study on the LAOS system.

The growing volume of Electronic Health Records (EHRs) has enhanced patient care quality but significantly increased the cognitive workload on clinicians, particularly in ophthalmology where specialists handle 1.6 times more patient consultations than other specialties. This study introduces the "LLM-based Auxiliary Ophthalmic System (LAOS)," an integrated framework leveraging Large Language Models (LLMs) and audio processing to improve clinical documentation accuracy and efficiency. LAOS combines voice recognition with Retrieval-Augmented Generation (RAG) and Low-Rank Adaptation (LoRA) to con

Crossmodal benefits to vocal emotion perception in cochlear implant users.

Speech comprehension counts as a benchmark outcome of cochlear implants (CIs)-disregarding the communicative importance of efficient integration of audiovisual (AV) socio-emotional information. We investigated effects of time-synchronized facial information on vocal emotion recognition (VER). In Experiment 1, 26 CI users and normal-hearing (NH) individuals classified emotions for auditory-only, AV congruent, or AV incongruent utterances. In Experiment 2, we compared crossmodal effects between groups with adaptive testing, calibrating auditory difficulty via voice morphs from emotional caricatu

Immediate and partial neural dysfunction after thyroid and parathyroid surgery: Need for recognition, laryngeal exam, and early treatment.

BACKGROUND: Laryngeal dysfunction after thyroid and parathyroid surgery requires early recognition and a standardized approach for patients that present with voice, swallowing, and breathing issues. The Endocrine Committee of the American Head and Neck Society (AHNS) convened a panel to define the terms "immediate vocal fold paralysis" and "partial neural dysfunction" and to provide clinical consensus statements based on review of the literature, integrated with expert opinion of the group. METHODS: A multidisciplinary expert panel constructed the manuscript and recommendations for laryngeal

A network analysis of body satisfaction of people with gender dysphoria.

In gender dysphoria (GD), much of the experienced distress results from body dissatisfaction. The current study analyzed the configuration of body satisfaction in trans men and women using network analysis. In total, 485 individuals diagnosed with GD from four European countries, applying for medical treatment, filled out the Body Image Scale for Transsexuals. A six-factor model reflecting different body areas was confirmed via confirmatory factor analysis. A further configuration of body satisfaction was modelled using correlation network analysis techniques in R. Genital dissatisfaction show

Task specificity in adductor spasmodic dysphonia versus muscle tension dysphonia.

OBJECTIVES: Adductor spasmodic dysphonia (ADSD) has been characterized as a "task specific" laryngeal dystonia, meaning that the severity of dysphonia varies depending on the demands of the vocal task. Voice produced in connected speech as compared with sustained vowels is said to provoke more frequent and severe laryngeal spasms. This study examined the diagnostic value of "task specificity" as a marker of ADSD and its potential to differentiate ADSD from muscle tension dysphonia (MTD), a functional voice disorder that can often masquerade as ADSD. STUDY DESIGN: Case-control study. METHODS:

A Two-Stage Cepstral Analysis Procedure for the Classification of Rough Voices.

OBJECTIVES: The objective of this study was to investigate the ability of a two-stage method of cepstral peak identification to effectively discriminate rough vs breathy vs typical voice in sustained vowel productions. It was hypothesized that a dual-stage search for cepstral peak prominences (CPP's) above and below specified quefrency/F0 cutoffs would result in a CPP difference that would be characteristic of the rough, diplophonic voice type. METHODOLOGY: Central one-second portions of sustained vowel /a/ productions were obtained from 90 subjects (rough, breathy, and normophonic voices). A
