Modelling of potential risk areas of pertussis cases in the Philippines using bioclimatic envelopes.

OBJECTIVES: Pertussis remains a pressing public health concern in the Philippines despite being vaccine-preventable. The resurgence of the disease, driven by waning immunity, vaccine hesitancy and erratic outbreaks, underscores the need for innovative surveillance methods. Thus, this study intends to create a predictive model of potential risk areas for pertussis outbreaks in the Philippines using bioclimatic variables. METHODS: This study employs the maximum entropy algorithm to predict pertussis risk areas in the Philippines based on 19 bioclimatic variables. The occurrence data of reported

Pertussis immunisation strategies to optimise infant pertussis control: A narrative systematic review.

OBJECTIVE: Countries routinely offering acellular pertussis vaccine, where long-term protection is not sustained, have the challenge of selecting an optimal schedule to minimise disease among young infants. We conducted a narrative systematic review and synthesis of information to evaluate different pertussis immunisation strategies at controlling pertussis disease, hospitalisation, deaths, and vaccine effectiveness among young infants. METHODS: We conducted a review of the literature on studies about the primary, booster, and/or maternal vaccination series and synthesised findings narrativel

Global spatial dynamics and vaccine-induced fitness changes of Bordetella pertussis.

As with other pathogens, competitive interactions between Bordetella pertussis strains drive infection risk. Vaccines are thought to perturb strain diversity through shifts in immune pressures; however, this has rarely been measured because of inadequate data and analytical tools. We used 3344 sequences from 23 countries to show that, on average, there are 28.1 transmission chains circulating within a subnational region, with the number of chains strongly associated with host population size. It took 5 to 10 years for B. pertussis to be homogeneously distributed throughout Europe, with the sam

Pertussis vaccine effectiveness and duration of protection - A systematic review and meta-analysis.

A comprehensive review of observational pertussis vaccine effectiveness (VE) studies is needed to update gaps from previous reviews. We conducted a systematic review of VE and duration of protection studies for the whole-cell (wP) and acellular (aP) pertussis vaccines and conducted a formal meta-analysis using random effects models. Evidence continues to suggest that receipt of any pertussis vaccine confers protection in the short-term against disease although this protection wanes rapidly for aP vaccine. We detected significant heterogeneity in pooled estimates due, in part, to factors such a

Can Comprehensive Respiratory Pathogen Panels be Used to Exclude Pertussis Infection?

BACKGROUND: Pertussis is a serious public health concern and accurate diagnosis is imperative. Comprehensive, multiplex respiratory pathogen polymerase chain reaction (PCR) panels (RPPs) have recently become popular, but their utility in excluding pertussis infection has not been fully explored. OBJECTIVES: To determine RPP testing results for pertussis using frozen banked samples that previously tested positive on dedicated Bordetella pertussis PCR testing, and to describe positive test rates for other respiratory pathogens on these samples via RPP. METHODS: Our microbiology laboratory retr

Laparoscopic splenectomy for atraumatic splenic rupture.

A traumatic splenic rupture (ASR) is a rare clinical entity. Several underlying benign and malignant conditions have been described as a leading cause. We report on a case of ASR in a 41-year-old man treated with laparoscopic splenectomy. Considering ASR as a life-threatening condition, a prompt diagnosis can be life saving.

A new animal model of Bordetella pertussis infection and immunity.

Bordetella pertussis is a strictly human pathogen. Experimental infection of other animals can occur with large inoculating doses; rat, mice and primate models have been used to study pathogenesis and immunity. Recently, it was shown that newborn piglets are susceptible to B. pertussis. Lung pathophysiology of infected piglets was similar to that of human infants that develop bronchopneumonia. Piglets and infants are anatomically similar and maternal antibodies are transferred and secreted by a similar mechanism. This model could be valuable for studying the roles of passively and actively acq

The Battle Against Pertussis: Discovery of Endogenous Human Proteins and Peptides as Toxin-Inhibitors.

The life-threatening disease pertussis, also known as whooping cough, is caused by a complex interplay of several virulence factors produced by the bacterium Bordetella (B.) pertussis. These include the AB-type protein toxin pertussis toxin (PT), the main causative agent of pertussis. After infection with B. pertussis, PT is released and binds to its human target cells, which internalize PT. The enzyme subunit of PT is then taken up into the cytosol, where it catalyzes the ADP-ribosylation of the α-subunit of inhibitory GTP-binding proteins from the Gαi type. This ultimately leads to the devel…

[Current situation and research progress on severe pertussis and death in infants and young children].

Pertussis remains a global public health concern, with infants and young children representing the high-risk population for severe illness and death. In many countries, reported incidence levels of pertussis have rebounded, and passive surveillance systems often underreport severe cases and fatalities. Risk factors for severe pertussis include young age, comorbidities, and unvaccinated status, whereas elevated white blood cell count and pulmonary artery hypertension increase the risk of death among severe cases. Therefore, severe pertussis and related deaths in infants and young children warra

Variability in the Diagnostic Criteria for Severe Pertussis: A Systematic Review.

IMPORTANCE: Pertussis, or whooping cough, can be life threatening in infants, yet the concept of severe pertussis remains poorly defined in the literature. Lack of standardization may impair research and public health interventions. OBJECTIVE: To assess the variability in terminology and criteria used to define severe pertussis in the published literature. EVIDENCE REVIEW: A systematic review of the literature drawn from PubMed and Web of Science databases was performed. Studies involving infants with pertussis that provided a definition for severe pertussis and published from database incep…

Strengthening Bordetella pertussis genomic surveillance by direct sequencing of residual positive specimens.

Whole-genome sequencing (WGS) of microbial pathogens recovered from patients with infectious disease facilitates high-resolution strain characterization and molecular epidemiology. However, increasing reliance on culture-independent methods to diagnose infectious diseases has resulted in few isolates available for WGS. Here, we report a novel culture-independent approach to genome characterization of Bordetella pertussis, the causative agent of pertussis and a paradigm for insufficient genomic surveillance due to limited culture of clinical isolates. Sequencing libraries constructed directly f

Pertussis vaccines, epidemiology and evolution.

Pertussis, which is caused by Bordetella pertussis, has plagued humans for at least 800 years, is highly infectious and can be fatal in the unvaccinated, especially very young infants. Although the rollout of whole-cell pertussis (wP) vaccines in the 1940s and 1950s was associated with a drastic drop in incidence, concerns regarding the reactogenicity of wP vaccines led to the development of a new generation of safer, acellular (aP) vaccines that have been adopted mainly in high-income countries. Over the past 20 years, some countries that boast high aP coverage have experienced a resurgence i

Metoclopramide-induced acute dystonic reaction: A pediatric case report.

INTRODUCTION: This case report presents a unique acute dystonic reaction (ADR) induced by metoclopramide in a 6-year-old male patient with pertussis-associated vomiting. The rarity of such a reaction in pediatric patients underscores the significance of this case in contributing to the scientific literature. This report highlights the need for heightened awareness of the potential adverse effects of medications commonly used in pediatrics and emphasizes the importance of tailored interventions for this population. MAIN SYMPTOMS AND IMPORTANT CLINICAL FINDINGS: Following the administration of
