Wrist Arthroscopy in Children and Adolescent With Chronic Wrist Pain: Arthroscopic Findings Compared With MRI.

BACKGROUND: Wrist arthroscopy is a dynamic diagnostic procedure and its indications are growing as a treatment modality in the adult population. The aim of the study was to retrospectively report our series of pediatric and adolescence with chronic wrist pain, with or without wrist instability who underwent wrist arthroscopy after failing at least 4 months of conservative management. Our secondary aim was to report the sensitivity and specificity of clinical examination and magnetic resonance imaging evaluation for various injury subgroups against the gold standard of the arthroscopic findings…

Wrist arthrodesis with the AO titanium wrist fusion plate: a consecutive series of 42 cases.

In a 4 year period (1996-1999), 42 total wrist fusions in 25 men and 17 women were performed using the AO/ASIF Titanium wrist fusion plate. The median age of the patients at the time of surgery was 41 (range, 19-72) years. The indication for fusion was post-traumatic arthritis in 29 wrists, Kienböck's disease in eight, rheumatoid arthritis in three, mono-arthritis in one and Volkmann's contracture in one. All patients were reviewed at a median follow-up of 23 (range, 6-50) months. The Buck-Gramcko and Lohmann score for functional evaluation was excellent in 35, good in 5 and satisfactory in 2 …

Wrist arthroscopy without distraction. A technique to visualise instability of the wrist after a ligamentous tear.

We describe a technique for arthroscopy of the wrist which is carried out without traction and with the arm lying horizontally on the operating table. The wrist is not immobilised, which makes it possible to assess the extent of instability after a ligamentous tear. In a prospective study of 30 patients we compared this technique with conventional wrist arthroscopy, performing the new method first followed by conventional arthroscopy. The advantages are that the horizontal position of the arm allows the surgeon to proceed directly from arthroscopic diagnosis to treatment, and that no change of…

[Load transmission through the wrist joint: a biomechanical study comparing the normal and pathological wrist].

This study was undertaken to elucidate the correlation, if any, between pressure loading and the progress of degenerative osteoarthritis in the human wrist. Nine fresh amputated forearms were used to produce i) a pathological model of a malunion after a distal radius fracture, ii) a model of a triangular fibrocartilage (TFC) tear, and iii) a model of a scapholunate dissociated wrist (involving three specimens for each model). On pressure loading, changes in the radioulno-carpal joint and in the midcarpal joint were measured using a pressure sensor. In the malunion of the distal radial end, we …

Rheumatoid wrists treated with synovectomy of the extensor tendons and the wrist joint combined with a Darrach procedure.

Forty-three rheumatoid wrists in 43 patients with bilateral wrist involvement were treated with synovectomy of the extensor tendons and wrist joint combined with a Darrach procedure in the period from 1966 to 1986. Clinical and radiologic assessment of the wrists was carried out after an average follow-up period of 11 years, with comparison of the treated and the opposite untreated wrists. The authors confirmed what others have concluded regarding the operation: pain was generally decreased, forearm rotation increased, and wrist extension and palmar flexion changed little. Radiologically, carp…

Production time and practicability of 3D-Printed wrist orthoses versus low temperature thermoplastic wrist orthoses.

INTRODUCTION: In recent years, three-dimensional (3D) printing has emerged as a new manufacturing technique for orthoses, showing comparable stability and wearing comfort to traditional orthoses. However, the lengthy designing and printing process is assumed to take more time than the common practice of manufacturing low-temperature thermoplastic orthoses (LTTOs). The aim of this prospective cross-sectional study was to compare the production time of 3D-printed orthoses (3DPO) to LTTOs. METHODS: The active and passive time needed to manufacture the orthoses was measured in a clinical setting.

Wrist posture unpredictably affects perception of targeted transcutaneous electrical nerve stimulation with wrist-placed electrodes.

OBJECTIVE: Targeted transcutaneous electrical nerve stimulation (tTENS) is a non-invasive neural stimulation technique that involves activating sensory nerve fibers to elicit tactile sensations in a distal, or referred, location. Though tTENS is a promising approach for delivering haptic feedback in virtual reality or for use by those with somatosensory deficits, it was not known how the perception of tTENS might be influenced by changing wrist position during sensorimotor tasks. APPROACH: We worked with 12 able-bodied individuals and delivered tTENS by placing electrodes on the wrist, thus t…

[The application and prospect of wrist arthroscopy].

Since its introduction in the 1970s, wrist arthroscopy has evolved from its nascent stages to a mature technology, aiding hand surgeons in achieving clear diagnoses and effective treatments for complex wrist injuries. Wrist arthroscopy shows important effects on the diagnostic and therapeutic value in conditions such as triangular fibrocartilage complex injuries, scapholunate ligament injuries, ganglion cysts, scaphoid fracture nonunion, distal radius fractures, and lunate avascular necrosis. Through systematic arthroscopic evaluation of the wrist, the technique has significantly enhanced the …

[Sport Injuries to Wrist and Fingers].

About one fifth of all sports accidents involve the hand. Many injuries can be diagnosed and treated easily. However, some of them such as fractures of the scaphoid or the hamate of the hamulus are difficult to detect with conventional radiographs and are often missed without a CT scan. Ligament injuries such as the skier's thumb must be recognized and treated properly, often surgically. There are also sport-specific injuries, such as the closed rupture of a flexor tendon pulley in climbers, which otherwise occur rarely and are little known. These topics, pitfalls and tricks will be discussed.

Is Wrist Arthroscopy Safer with the Nanoscope?

Background  Nanoscope, given its smaller size, may be safer when establishing dorsal wrist arthroscopy portals compared with the traditional 2.7 mm arthroscope. Case Description  Ten fresh frozen cadaver specimens were utilized. Dorsal radiocarpal portals were established with the Nanoscope and calipers were used to measure the distance between the portals and the surrounding anatomical structures. The only structure that was pierced during portal placement was the dorsal sensory branch of the ulnar nerve (DSUN) in one specimen when establishing the 6U portal. Our study did not note any tendon…
