OBJECTIVE: To determine the accuracy of common radiological indices for diagnosing ruptures of the scapholunate (SL) ligament, the most rele … surgical care.
KEY POINTS: • Scapholunate ligament (SL) lesions are the most relevant soft tissue wrist injuries. • Missed and untreated SL ruptures can cause painful and disabling post-traumatic wrist osteoarthritis. • Reliable threshold values of radiographic indices should prompt further imaging or surgical care. • Plain radiographs deliver conclusive clini …

Sanitary clamp wingnut manipulation is the fifth most frequent cause of injuries in the production areas of the Bayer Healthcare site in Berkeley, California. Task analysis showed that manual manipulation of clamp wingnuts had concurrent multiple risk factors, including: awkward postures, high forces, contact stresses, and high frequency. Task elimination was not an option. Several tools on the market were researched and tried but it was found that no existing tools would alleviate the associated ergonomics risk factors. An interim tool found on the market was used as a short term solution unt

The hands and wrists of 20 top-level rock climbers with sports injuries and overstress abnormalities were compared with the hands and wrists of 10 normal volunteers. They were all studied with MR imaging at 1.5 T. The imaging protocol included spin-echo and gradient-echo sequences with 1- to 5-mm-thick contiguous slices in the axial, coronal and/or sagittal planes, depending on the location and nature of the suspected injury. Typical hand and wrist lesions depicted with MRI in climbers consisted of anular ligament tears, lesions of the flexor tendons, tenosynovitis, ganglion cysts, joint effus

PURPOSE: Reconstruction of wrist extension in C5-C8 root brachial plexus injuries (BPIs) is challenging, because of limited nerve and tendon donors. The purpose of this study was to report outcomes relating to the pronator quadratus branch of the anterior interosseous nerve (AIN) to extensor carpi radialis brevis (ECRB) nerve transfer from three BP surgeons.
METHODS: Ten patients with C5-C8 BPI underwent AIN to ECRB nerve transfer. The mean age of the patients was 30.5 ± 15.9 years. All patients were men. The mean time to nerve surgery following initial injury was 4.5 ± 1.2 months. In all pat

BACKGROUND: Patient-reported outcome measures (PROMs) were developed to examine patients' perceptions of functional health. Most studies compare the responsiveness of each type of questionnaire. However, reports of patient preferences among PROMs commonly used with patients with hand/wrist injuries or disorders are limited. This study aimed to compare patient preferences, factors associated with those preferences and responsiveness among the Disability of the Arm, Shoulder, and Hand (DASH), Michigan Hand Outcomes Questionnaire (MHQ), Patient-Rated Wrist/Hand Evaluation (PRWHE) and EQ-5D in pat…

STUDY DESIGN: Retrospective cost-of-illness study.
INTRODUCTION: Injuries to the hand and wrist are common. Most uncomplicated and stable upper extremity injuries recover with conservative management; however, some require surgical intervention. The economic burden on the health care system from such injuries can be considerable.
PURPOSE OF THE STUDY: To estimate the economic implications of surgically managed acute hand and wrist injuries at one urban health care network.
METHODS: Using 33 primary diagnosis ICD-10 codes involving the hand and wrist, 453 consecutive patients from 2014 to 20…

BACKGROUND: Acute traumatic tendon injuries of the hand and wrist are commonly encountered in the emergency department. Despite the frequency, few studies have examined the true incidence of acute traumatic tendon injuries in the hand and wrist or compared the incidences of both extensor and flexor tendon injuries.
METHODS: We performed a retrospective population-based cohort study of all acute traumatic tendon injuries of the hand and wrist in a mixed urban and rural Midwest county in the United States between 2001-2010. A regional epidemiologic database and medical codes were used to identi

A system has been developed for the registration and combined display of the X-ray image and isotope bone scan. This system has been evaluated by prospectively studying 23 patients who were referred for suspected injury of the wrist. The registration system has an inherent precision for registration of about 1 mm. When patient positioning errors are included, this increases to about 4 mm. Two observers evaluated the sets of images prior to registration and after registration and combined display. They judged that in 16 out of 17 cases judged abnormal (observer 1) and 12 out of 18 cases judged

BACKGROUND/PURPOSE: Diagnosing wrist ligament injuries is challenging; early detection and treatment are important to prevent osteoarthritis progression. Interosseous proximity maps, a proxy measure for joint space, can be generated from volumetric imaging data and may provide important information about wrist health. Artificial intelligence (AI) could enhance accuracy of noninvasive diagnosis based on imaging-derived metrics. This work demonstrates feasibility of AI training using synthetic proximity map data generated from finite element models (FEMs).
METHODS: Personalized wrist FEMs for t

OBJECTIVE: The aim of direct distal selective nerve transfer close to the end organ with high radial nerve injury is restoration of the paretic function before irreversible atrophy of the target muscle. Simultaneous tendon transfer enables direct functional correction of wrist drop.
INDICATIONS: Selective nerve and tendon transfer of the lower arm is indicated if a) the primary nerve lesion is located proximally distant and reinnervation by direct nerve repair would take too long to reach a paretic muscle because of the long distance involved, b) direct repair of the nerve lesion is impossibl

BACKGROUND: The hand and wrist are most often in contact with electrical currents and thus most vulnerable to severe electrical burns. The treatment of such severe injury via surgical intervention remains a big challenge because of the vast tissue necrosis and the segmental vascular injury. The flow-through lateral-thigh free flap has been used effectively to resurface these defects and to reconstruct segmental vascular defects.
METHODS: Between January 2014 and June 2017, 11 male patients aged 19 to 53 years were admitted to the burn unit of our institution. Each presented with severe electr

A systematic review of the literature related to effective occupational therapy interventions in rehabilitation of individuals with work-related forearm, wrist, and hand injuries and illnesses was conducted as part of the Evidence-Based Literature Review Project of the American Occupational Therapy Association. This review provides a comprehensive overview and analysis of 36 studies that addressed many of the interventions commonly used in hand rehabilitation. Findings reveal that the use of occupation-based activities has reasonable yet limited evidence to support its effectiveness. This revi

Athletic injuries to the wrist and hand are common in clinical practice. When indicated, the use of a protective playing splint enables athletes to continue sports participation. The purpose of this paper is to discuss the splinting material properties that determine the materials' suitability for use in fabrication of a protective playing splint. A second aim is to present currently available splinting materials. The clinical reasoning process for material application and selection is based on understanding material properties, the type and severity of the injury, phase of treatment, and type

Turner syndrome is a chromosomal disorder associated with short stature, webbed neck, gonadal dysgenesis, and congenital cardiac anomalies. Subtle skeletal findings may provide diagnostic clues. We report a 12-year-old girl in whom an incidental short fourth metacarpal identified after wrist trauma led to the diagnosis of Turner syndrome.

Bony avulsion injuries of the triangular fibrocartilage complex (TFCC) caused by penetrating wrist trauma are rare, and there is limited literature available regarding their treatment. This case report describes a 54-year-old male who sustained a penetrating injury to his right wrist, resulting in "double lesion" of TFCC injury, which are ulnar avulsion (Palmer 1B) and radial avulsion (Palmer 1D) lesion with a sigmoid notch fracture. The patient underwent an arthroscopic examination and a novel arthroscopic technique for fixation and repair of the TFCC injury. The procedure involved debridemen

Perilunate dislocations are uncommon high energy injuries. The combination of fractures resulting in a trans-styloid, trans-scaphoid, and trans-triquetral perilunate fracture dislocation is extremely rare. We describe a 20 year old male who suffered this injury after a fall from height while parkouring. He underwent open reduction and internal fixation with ligamentous repair and carpal tunnel decompression. After 3 months, he was noted to have a radiographic evidence of scapholunate dissociation and he returned to the operative room for definitive fixation. Despite this complication, the fina
