OBJECT: The relationship between the waveform of intracranial pressure (ICP) and arterial blood pressure can be quantitatively characterized using a newly developed technique in systems analysis, the time-varying transfer function. This technique considers the arterial blood pressure as an input signal composed of multiple frequencies represented in the output ICP according to the transfer function imposed by the intracranial system on the input signal. The transfer function can change with time and with physiological manipulations. The authors examined data obtained from canine experiments in

The objective of this prospective 18-month study was to evaluate the clinical and laboratory effects of repeated intrathecal injections of chemotherapy in children with acute leukemia. All procedures were performed under general anesthesia, and complications were prospectively recorded. Laboratory measurements included lumbar puncture opening pressure, cerebrospinal fluid (CSF) chemistry, and cell count and morphology. Central venous pressure and ophthalmologic examinations were also performed. Forty-seven children underwent 247 intrathecal injections of chemotherapy. Adverse effects (13.7% of

Soluble glial fibrillary acidic protein (GFAP) was quantified in human cerebrospinal fluid (CSF) and amniotic fluid. A normal value in lumbar CSF of 4.3 +/- 0.7 ng GFAP/ml (mean +/- SEM) was obtained from 18 non-neurological patients. Increased GFAP concentrations in CSF were found in patients with intracranial tumours or with normal pressure hydrocephalus, while normal values were found in multiple sclerosis patients and in patients with degenerative dementia. In addition, a concentration gradient between ventricular and lumbar CSF was demonstrated, the GFAP content being significantly higher

The CSF flows in the aqueduct and at the foramen magnum were examined in 5 patients with communicating hydrocephalus (HC) and in 10 with benign intracranial hypertension (BIH) as well as in 5 healthy volunteers. As compared to normal individuals, the aqueductal flow in HC was about 10 times larger and the cervical flow was half as large. In BIH the CSF flows were not different from those of normal volunteers. The decreased arterial expansion as reflected in the reduced cervical flow in HC may be due to pathologic changes in the arteries and paravascular spaces. The large aqueductal flow in HC

When ICP measurement is used for a neuro-surgical patient we wonder about two questions: --which are the highest and the mean ICP value? --is there some spontaneous pathological variations like plateau wave or b wave? To answer these questions it is necessary to record ICP for extended periods of time. Numerical values are difficult and tedious to set up from ICP standard recordings. To resolve this practical problem, JANNY and KULLBERG had proposed a statistical automatic analysis of the different ICP values using a computer. The modal curve of the different classes of ICP amplitudes is then

Transmission of intracranial pressure (ICP) to the perilymph of the cochlea may occur via the cochlear aqueduct and possibly other routes. Indirect measurement of perilymphatic pressure may be investigated by observing tympanic membrane (TM) displacement during stapedial reflex contraction. In a previous study we investigated the effects of changes in ICP on perilymphatic fluid pressure in three patients who underwent ventriculo/lumbar-peritoneal shunt operations. The TM displacement technique proved extremely sensitive and revealed marked changes in cochlear fluid pressure brought about by ch

A 9-year-old boy with spina bifida, Chiari II malformation, and hydrocephalus presented with signs of increased intracranial pressure consistent with a shunt malfunction. Radiological investigations revealed an intracranial calcified lesion along the ventricular catheter. A shunt tap revealed a translucent milky white fluid. The patient underwent a ventriculostomy and, eventually, a shunt revision. Pathology findings were consistent with the formation of dystrophic calcification and a pseudocyst around the shunt catheter. Postoperatively, the patient returned to his neurological baseline. This

The case of a 30-year-old male patient is presented who was admitted with a 3-month history of elevated intracranial pressure and reduced-hearing on the right. CT revealed hydrocephalus due to displacement of the 4th ventricle by a large non-enhancing cerebellar mass with calcifications. MRI sequences showed a space-occupying lesion within the right cerebellar hemisphere with unusual septations. At operation, the poorly demarcated lesion was excised from the apparently normal surrounding cerebellar tissue and histologically proven to be a dysplastic gangliocytoma (Lhermitte-Duclos disease). PURPOSE: Although dopamine transporter (DAT) imaging has been reported to be useful for differentiating idiopathic Normal Pressure Hydrocephalus (iNPH) from its mimics, the radiological findings of DAT imaging in iNPH have not been established. We investigated [123I] N-ω-fluoropropyl-2β-carboxymethoxy-3β-(4-iodophenyl) nortropane (FP-CIT) single photon emission computed tomography (SPECT) images from patients with disproportionately enlarged subarachnoid-space hydrocephalus (DESH)-type iNPH to understand the characteristics of DAT images of iNPH.
METHODS: We retrospectively collected 11 DESH-

Regional cerebral blood flow (rCBF) was measured as fast flow clearance (F1) and the initial slope index (ISI2) after inhalation of 133Xe in 30 patients with subarachnoid hemorrhage (SAH). Vasomotor responsiveness to reduction in end-tibal PECO2 was examined in those patients who could carry out this procedure satisfactorily as a test for the presence or absence of vasospasm. F1 and ISI2 were significantly reduced in patients with recent SAH compared to 35 age-matched normal volunteers. The degree of reduction of F1 and ISI2 correlated directly with severity of the neurological deficit graded

Shunt operations have conventionally been performed to deal with normal-pressure hydrocephalus after subarachnoid hemorrhage. The indication and timing of shunt operations are often based on clinical symptoms and head computed tomography findings, and the early identification of the need for such surgery would be advantageous. The present study examined whether this need could be predicted solely on the basis of data collected on patient admission. A total of 120 consecutive patients with subarachnoid hemorrhage who underwent radical surgery for aneurysm were analyzed for potential risk factor

OBJECT: Endoscopic third ventriculostomy (ETV) is an established treatment for hydrocephalus. Most studies focus on success rate, and complications are insufficiently documented. The aim of this study was to perform a systematic review of ETV complications.
METHODS: A Medline search discovered 34 series of ETV with detailed complications reports (17 series involving exclusively pediatric patient populations, 6 series involving exclusively adults, and 11 series involving mixed adult and pediatric populations).
RESULTS: The analysis included 2985 ETVs performed in 2884 patients. The cause of h

The hypothalamus, a vital regulator of multiple physiologic functions, is the principal source of the neuropeptide orexin, which is thought to regulate the sleep-wake cycle. As hypothalamic damage may result from aneurysmal subarachnoid haemorrhage (SAH) and be associated with a depressed conscious level, we sought to investigate whether orexin levels reflected the severity of the ictus and were of any prognostic value in SAH. CSF orexin levels from 15 patients with aneurysmal SAH were analysed for up to 14 days. The correlation between orexin and GCS, WFNS grade, Fisher grade, GOS at 6 months

BACKGROUND: Normal pressure hydrocephalus (NPH) is an etiology of dementia that is reversible following cerebrospinal fluid shunt placement, however, surgical intervention not always clinically effective and the respons to shunt therapy is poorly understood. Furthermore, NPH is a source of comorbidity in diseases with neurodegenerative pathology, such as Alzheimer's disease (AD).
CASE REPORT: A 61-year-old woman presented to the neurology clinic with progressive gait difficulties and cognitive impairment over five years. Nine years after ventriculoperitoneal (VP) shunt treatment, the patient

Water not only exists in the ventricles and cisterns but is also contained in the cerebral parenchyma. Water movement in most tissues has been described to be mediated by water channels since the discovery of these channels(aquaporins: AQPs). As to the intracranial situation, AQP-1 is abundantly expressed in the epithelial cells of the choroid plexus. AQP-4 is also enriched in the ependyma, endfeet of astrocytes at the blood-brain barrier, and glia limitans. AQP-4 is involved in cerebrospinal fluid(CSF)absorption, while AQP-1 is involved in CSF production. Impairment of CSF absorption through

One of the first steps toward the correct diagnosis of dementia is to segregate out the nondegenerative dementias from possible degenerative dementias. Nondegenerative dementias could be due to traumatic, endocrine, metabolic, nutritional, toxic, infective, and immunological causes. They could also be caused by tumors, subdural hematomas, and normal pressure hydrocephalus. Many of the nondegenerative dementias occur at an earlier age and often progress quickly compared to Alzheimer's disease and other degenerative dementias. Many are treatable or preventable with simple measures. This review a

Although lower urinary tract dysfunction and dementia are classically associated, some patients with advanced dementia do not experience incontinence. Management of these patients must be based on careful assessment of the respective roles of ageing of the urinary tract and dementia in the pathogenesis of incontinence. It is also important to distinguish those dementia syndromes that are inevitably accompanied by urinary incontinence (such as normal pressure hydrocephalus) and those in which urinary incontinence is a late phenomenon (such as Alzheimer's disease). The diagnostic and therapeutic
