CONTEXT: Survival rates exceed 90% in Wilms' tumor (WT). Actinomycin-D (ACT-D) which is indispensable in the management of WT is associated with the development of sinusoidal obstruction syndrome (SOS), a potentially fatal complication.
AIMS: The aim is to study the presentation, management, and outcome of SOS complicating ACT-D administration in WT.
SETTINGS AND DESIGN: Retrospective file review conducted in a Pediatric Hematology-Oncology unit.
MATERIALS AND METHODS: Patients diagnosed and treated for WT from January 2012 to December 2015 were analyzed. SOS was diagnosed clinically, based

BACKGROUND: Once it is established that a jaundiced infant has direct hyperbilirubinemia, the principal diagnostic concern is to differentiate hepatocellular from obstructive cholestasis. Traditional tests such as ultrasonography, percutaneous liver biopsy and technetium 99 m hepatobiliary iminodiacetic acid (HIDA) scan are often not sufficiently discriminating. Definitive exclusion of biliary atresia (BA) in the infant with cholestatic jaundice usually requires mini-laparotomy and intra-operative cholangiography. This approach has many drawbacks because those sick infants are subjected to a t

In patients undergoing percutaneous coronary intervention and in those with acute coronary syndromes, clopidogrel plus aspirin is the first-line antiplatelet therapy for reducing cardiovascular events. Although clopidogrel is generally well tolerated, with rash, indigestion, vomiting, diarrhea, and bleeding being the most common adverse effects, rare but serious complications may occur. We describe a 78-year-old woman who underwent percutaneous coronary intervention with drug-eluting stents; clopidogrel and aspirin were started as antiplatelet therapy. Three weeks later, the patient developed

We report a case of intrahepatic cholangiocarcinoma treated by extended right lobectomy and resection of the inferior vena cava (IVC) and portal vein. A 53-year-old man was referred with elevated serum alkaline phosphatase (ALP) and gamma-glutamyl transpeptidase (gamma-GTP) levels on April 23, 1999. He was not jaundiced and did not have any symptoms. Endoscopic retrograde cholangiopancreatography (ERCP) revealed irregular strictures in both the anterior and posterior segmental ducts. Contrast-enhanced computed tomography (CT) scan demonstrated a low-density tumor with an unclear margin in the

BACKGROUND AND AIMS: Partially covered metal stents have been extensively used for palliation of obstructive jaundice in malignant distal biliary strictures and can be removed in cases of malfunction or need for tissue diagnosis. We investigated independent predictors of mortality in patients undergoing partially covered metal stents revision (i.e., removal and replacement).
METHODS: Patients with a distal malignant biliary obstruction palliated with a partially covered metal stent were followed-up prospectively over 5 years until malfunction or death. All patients who required removal of the

Sinusoidal obstruction syndrome (SOS) is an uncommon hepatic vascular complication classically associated with haematopoietic stem cell transplantation, although it may also occur following oxaliplatin-based chemotherapy. We report the case of a 52-year-old woman with stage III colon adenocarcinoma treated with adjuvant CAPOX who developed jaundice, ascites, rapid weight gain, thrombocytopenia, and severe liver dysfunction three weeks after completing chemotherapy. After other causes of acute liver injury had been excluded, Doppler ultrasound demonstrated reversal of portal venous flow and sig

Postpartum autoimmune hepatitis (AIH) is a rare but serious condition triggered by immune rebound after delivery. Diagnosis is typically supported by elevated immunoglobulins and positive autoimmune markers; however, seronegative presentations remain diagnostically challenging. We present the fourth reported worldwide case of seronegative postpartum AIH in a 36-year-old woman who developed jaundice, pruritus, pale stools, and dark urine 4 months after delivery. Laboratory evaluation revealed severe hepatocellular injury with aspartate aminotransferase 1373 U/L, alanine aminotransferase 2137 U/

Haloperidol, a first-generation antipsychotic, is occasionally prescribed during pregnancy for the management of schizophrenia, but its hepatic safety profile is not well established, and drug-induced liver injury (DILI) remains a rare yet potentially serious complication. We report a 38-year-old pregnant woman at 14 weeks of gestation with a 3-year history of schizophrenia on Haloperidol who presented with progressive abdominal discomfort and jaundice. Laboratory evaluation demonstrated a hepatocellular pattern of liver injury with elevated AST and ALT and hyperbilirubinemia, while white bloo

INTRODUCTION: Spinal epidural abscess (SEA) is a rare but severe infection occurring in the epidural space of the spinal cord, increasingly associated with invasive spinal procedures and intravenous drug use. Recognizing and accurately diagnosing SEA is crucial due to its potential neurological impact.
PRESENTATION OF CASE: A 51-year-old male presented with urinary and bowel obstruction, right-sided hemiplegia, fever, and jaundice. Neurological examination revealed pronounced deficits, while imaging and cerebrospinal fluid analysis confirmed multiple epidural abscesses. Cultures identified St

Budd-Chiari syndrome (BCS) is a rare disorder caused by hepatic venous outflow obstruction, with clinical presentation ranging from asymptomatic disease to acute liver failure. We report a case of secondary BCS due to hyperhomocysteinemia in a 52-year-old male patient with a history of alcohol use. He presented with jaundice, vomiting, and melena. Examination showed icterus and tender hepatomegaly, while laboratory investigations revealed elevated liver enzymes. Doppler ultrasound and contrast-enhanced CT of the abdomen demonstrated caudate lobe hypertrophy, intrahepatic IVC and portal vein th

Budd-Chiari syndrome (BCS) is a scarce but severe condition characterized by the obstruction of the hepatic veins, liver congestion, and consequent damage. This series brings up one unusual presentation of BCS associated with autoimmune hemolytic anemia (AIHA), immune thrombocytopenic purpura (ITP), and lupus nephritis (LN), which collectively complicate the clinical scenario. This is a 19-year-old woman who was admitted for abdominal pain, hepatomegaly, ascites, and jaundice. Her history included the diagnosis of systemic lupus erythematosus. Laboratory findings revealed hemolytic anemia, thr

Xanthogranulomatous inflammation (XGI) is a rare, benign condition that can affect several organs, including the gallbladder, kidney, skin, gastrointestinal tract, lymph nodes, and soft tissues. It is often misdiagnosed as a malignancy. In this report, we present the case of a 79-year-old male who presented with persistent jaundice for 11 months. Computed tomography and magnetic resonance imaging revealed pancreatic head enlargement, gallbladder thickening, and common bile duct thickening, leading to a preoperative diagnosis of malignant neoplasm of the pancreatic head. During surgery, dense a

BACKGROUND: Congenital pulmonary hypoplasia (CPH) is a rare pulmonary disease featured by incomplete development of pulmonary tissues. Its diagnosis is still a challenge as patients are usually misdiagnosed as atelectasis.
CASE PRESENTATION: A female neonate was admitted to our hospital due to post-birth jaundice for 12 hrs. Physical examination showed accelerated breathing. There was no respiratory sound in the left lung. Chest film indicated decline of lucency in the left lung. Chest CT scan indicated absence of left lung and primary bronchus of the left lung. The boundary between left medi

Chronic pancreatitis is characterized by irreversible destruction of pancreatic parenchyma and its ductal system resulting from longstanding inflammation, leading to fibrosis and scarring due to genetic, environmental, and other risk factors. The diagnosis of chronic pancreatitis is made based on a combination of clinical features and characteristic findings on computed tomography or magnetic resonance imaging. Abdominal pain is the most common symptom of chronic pancreatitis. The main aim of treatment is to relieve symptoms, prevent disease progression, and manage complications related to chr

A 74-year-old man with advanced colon cancer was admitted to our hospital with jaundice and ascites. Four weeks before admission, he had started treatment with regorafenib because other chemotherapies had failed. Blood tests showed a characteristic increase in his serum lactate dehydrogenase level, which indicated intrahepatic hypoxia. The liver was not cirrhotic, but Doppler ultrasonography (US) showed that the portal flow was markedly decreased. These findings suggested that his liver failure could be caused by sinusoidal obstruction syndrome (SOS). We therefore started treatment with antico

Development of jaundice is an ominous prognostic sign, whether it occurs early or late in the months following hematopoietic cell transplant. In the first weeks after transplant, the dominant causes of liver injury are Sinusoidal Obstruction Syndrome (toxic damage resulting from myeloablative conditioning regimens) and cholangitis lenta (cholestasis of sepsis). Later after transplant, cholestasis is more commonly caused by acute graft-vs.-host disease and drugs. Hepatic infections have become uncommon because of the use of prophylactic anti-fungal and anti-viral drugs. Treatment of severe live

Small intestinal neoplasms are uncommonly encountered in clinical practice. They may occur sporadically, in association with genetic diseases (e.g., familial adenomatous polyposis coli or Peutz-Jeghers syndrome), or in association with chronic intestinal inflammatory disorders (e.g., Crohn's disease or celiac sprue). Benign small intestinal tumors (e.g., leiomyoma, lipoma, hamartoma, or desmoid tumor) usually are asymptomatic but may present with intussusception. Primary malignancies of the small intestine-including adenocarcinoma, leiomyosarcoma, carcinoid, and lymphoma-may present with intes

This report describes the surgical management of 2 children with fusiform choledochal cysts who had accessory hepatic ducts (AHD) identified during excisional surgery for fusiform choledochal cysts (CC). Two children presenting with a triad of recurrent jaundice, fever, and abdominal pain were investigated and found to have type 1 choledochal cyst. Preoperative imaging and intraoperative cholangiography missed the AHD in both cases. In one of the patients, the main and the accessory ducts were separated by the right hepatic artery. In both the patients the accessory ducts were reconstructed su
