Three-dimensional quadratic modeling and quantitative evaluation of the diaphragm on a volumetric CT scan in patients with chronic obstructive pulmonary disease.

PURPOSE: In patients with chronic obstructive pulmonary disease (COPD), diaphragm function may deteriorate due to reduced muscle fiber length. Quantitative analysis of the morphology of the diaphragm is therefore important. In the authors current study, they propose a diaphragm segmentation method for COPD patients that uses volumetric chest computed tomography (CT) data, and they provide a quantitative analysis of the diaphragmatic dimensions. METHODS: Volumetric CT data were obtained from 30 COPD patients and 10 normal control patients using a 16-row multidetector CT scanner (Siemens Sensat

[Endovascular treatment of paraprosthetic false aneurysm of the proximal anastomosis after aortobifemoral repair for an abdominal aortic aneurysm].

Presented herein is a clinical case concerning endovascular surgical treatment of a male patient having developed a paraprosthetic pseudoaneurysm of the proximal anastomosis 2 years after previously performed aortofemoral bifurcation repair for an infrarenal abdominal aortic aneurysm. The preoperative preparation was followed by endovascular prosthetic repair of the false aneurysm of the proximal anastomosis in the juxtarenal portion of the aorta. The postoperative period was uneventful with no complications, and the patient was discharged in a satisfactory condition on POD 9. During the perio

Preprocedural imaging to guide transcoronary ethanol ablation for refractory septal ventricular tachycardia.

BACKGROUND: Radiofrequency ablation (RF) of ventricular tachycardia (VT) due to intramural foci has a high recurrence rate. Several techniques, such as bipolar ablation, irrigated needle ablation catheter, and retrograde coronary venous ethanol ablation have been suggested. Transarterial coronary ethanol ablation (TCEA) can also be effective. We present a case series of TCEA guided with preprocedural imaging to correlated coronary arteries and the intramural substrate. METHODS AND RESULTS: We present three consecutive patients with previous RF of septal VT (100% male; age, 72.6 ± 11.01 years;

[Application possibilities and initial experience with digital volume tomography in hand and wrist imaging].

During the last decade, DVT (digital volume tomography) imaging has become a widely used standard technique in head and neck imaging. Lower radiation exposure compared to conventional computed tomography (MDCT) has been described. Recently, DVT has been developed as an extremity scanner and as such represents a new imaging technique for hand surgery. We here describe the first 24 months experience with this new imaging modality in hand and wrist imaging by presenting representative cases and by describing the technical background. Furthermore, the method's advantages and disadvantages are disc

Features on contrast-enhanced computed tomography for predicting recurrent esophageal variceal bleeding in patients with hepatocellular carcinoma.

BACKGROUND: Esophageal varices (EV) bleeding in patients with hepatocellular carcinoma (HCC) and liver cirrhosis is a life-threatening complication. We investigated whether features on multi-detector computed tomography (MDCT) could predict recurrent EV hemorrhage. MATERIALS AND METHODS: In this cross-sectional study, we retrospectively reviewed 72 patients with HCC and liver cirrhosis who received endoscopic therapy for EV bleeding between 2017 and 2022 at a single institution. Patients with both available contrast-enhanced CT scans taken before the bleeding episode within 6 months and upcom

Improved diagnostic accuracy of vessel-specific myocardial ischemia by coronary computed tomography angiography (CCTA).

BACKGROUND: Discrepancies between stenosis severity assessed at coronary computed tomography angiography (CCTA) and ischemia might depend on vessel type. Coronary plaque features are associated with ischemia. Thus, we evaluated the vessel-specific correlation of CCTA-derived diameter stenosis (DS) and invasive fractional flow reserve (FFR) and explored whether integrating morphological plaque features stratified by vessel might increase the predictive yield in identifying vessel-specific ischemia. METHODS: Observational cohort study including patients undergoing CCTA for suspected coronary ar

Percutaneous coronary intervention of an anomalous coronary chronic total occlusion: The added value of three-dimensional printing.

Three-dimensional (3D) printing has had an evolving role in cardiology, although has been largely reserved for planning of structural heart disease interventions. We present a case whereby multimodality imaging, including 3D printing, played a pivotal role in planning a technically feasible approach for complex percutaneous coronary intervention of a chronically occluded anomalous right coronary artery, with creation of a customized guide catheter.

Percutaneous transluminal angioplasty for central venous stenosis or occlusion in hemodialysis patients.

The objectives of central venous percutaneous transluminal angioplasty are to dilate the venous lesion and to extend the life of arteriovenous fistula for hemodialysis. It is reasonable to perform percutaneous transluminal angioplasty for central venous lesions if this interventional therapy is required to maintain stable dialysis therapy. However, the presence of large fresh thrombus at central venous lesion site represents a contraindication to percutaneous transluminal angioplasty unless the thrombus can first be removed by thrombectomy. Balloon angioplasty is a basic treatment for central

Single coronary ostium in a patient with quadricuspid aortic valve combined with aneurysmal ascending aortic dilatation.

BACKGROUND: The presence of a fourth aortic valve cusp (quadricupsid aortic valve) is a rare congenital malformation and is often accompanied by other anomalies of the adjacent cardiovascular structures. Among these concomitant anomalies, simultaneous association of both a single coronary ostium and aneurysmal ascending aortic dilation in combination with the quadricupsid aortic valve has not been reported yet. CASE PRESENTATION: We experienced the case of a 56-year-old female patient presenting as aortic regurgitation resulted from malcoaptation of quadricupsid aortic valve. The patient had

Coronary artery calcium and physical performance as determinants of mortality in older age: the AGES-Reykjavik Study.

BACKGROUND: Coronary artery calcium (CAC) and physical performance have been shown to be associated with mortality, but it is not clear whether one of them modifies the association. We investigated the association between the extent of CAC and physical performance among older individuals and explored these individual and combined effects on cardiovascular disease (CVD) mortality and non-CVD mortality. METHODS: We studied 4074 participants of the AGES-Reykjavik Study who were free from coronary heart disease, had a CAC score calculated from computed tomography scans and had data on mobility li

The Mesopancreas remains at risk in primary resectable pancreatic cancer patients: time to reappraise resectability criteria?

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is associated with poor survival, and complete surgical resection with a negative circumferential resection margin (R0CRM-) remains the only potentially curative treatment. Current resectability criteria primarily stratify patients according to arterial and portomesenteric venous involvement. However, the impact of mesopancreatic infiltration on resection margin status within these classifications remains unclear. METHODS: A consecutive cohort of 271 patients undergoing upfront resection for primarily resectable or borderline resectable PDAC

Pancreaticoduodenectomy preserving aberrant gastroduodenal artery utilized in a previous coronary artery bypass grafting: A case report and review of literature.

RATIONALE: Pancreaticoduodenectomy (PD) is a technically demanding procedure with high rates of morbidity and mortality. Therefore, preoperative evaluation of anatomy is indispensable. Multi-detector row computed tomography (CT) enables us to precisely understand arterial anatomy. It is a well-known fact that anatomical variants are often present in the hepatic artery (HA) but rarely in the gastroduodenal artery (GDA). We present the case of a patient with ampullary cancer with a rare anatomical anomaly, "replaced GDA (rGDA) " arising from the superior mesenteric artery, along with a history o
