Radiating pain to the lower extremities caused by lumbar disk rupture without spinal nerve root involvement.

PURPOSE: To locate the origin of the pain during lumbar diskography by means of a limited intradiskal injection of a local anesthetic. METHODS: Lumbar diskography by the direct central posterior approach was performed in 235 consecutive patients. In 17 patients, severe and persistent low back pain, with unilateral or bilateral radiation to the lower extremities, was provoked by contrast injection into only one disk. One milliliter of 1% lidocaine was then slowly injected in the center of these disks. RESULTS: A 75% to 100% reduction of the low back pain was experienced by 13 patients, and a

Cervical stabilization by plate and bone fusion.

Anterior stabilization with combined plate and bone fusion was performed after neural decompression on ten patients for spondylotic cervical myelopathy, and for radiculopathy or trauma in three patients. Medial corpectomy was performed at one to four levels. Iliac crest or fibular bone grafts were secured by plates anchored to the graft and adjacent vertebral bodies. All patients were placed in Minerva braces postoperatively. There was successful fusion in all cases, and no graft dislodgement or kyphosis. Early initiation of rehabilitation was achieved. Morbidity occurred in patients with seve

The Chiari II malformation: Part IV. The hindbrain deformity.

Computed tomography successfully delineates the multiple components of the Chiari II malformation at the craniocervical junction, the hindbrain, and the cervical spinal cord. These include wide foramen magnum and upper cervical spinal canal; incomplete fusions of the posterior arches of C1 and lower cervical vertebrae; cascading protrusions of vermis, fourth ventricle, medulla, and cervical cord into the spinal canal; cervicomedullary "kinking"; anterior displacement and sequential sagittal compression of each protrusion by the protrusions posterior to it; compression of all protrusions by the

Lateral extracavitary approach to traumatic lesions of the thoracic and lumbar spine.

The lateral extracavitary approach to the spine was used for resection of displaced bpne and disc located anterior to the dura in 62 patients with traumatic lesions of the thoracic and lumbar spine. Fifty-two patients had closed vertebral fractures and 10 had gunshot wounds. The spinal cord was involved in 44 patients, and the cauda equina in 18. A spinal subarachnoid block was demonstrated in 17 of 57 preoperative gas myelograms. Evoked potential recordings, although related to preception of joint rotation, tended to reflect the overall neurological condition and had some prognostic value. Si

Dynamic neutralization of the lumbar spine after microsurgical decompression in acquired lumbar spinal stenosis and segmental instability.

STUDY DESIGN: Prospective clinical study. OBJECTIVE: To report the outcome, radiologic findings, and complications in patients undergoing microsurgical radicular decompression and implantation of Dynesys (Zimmer Spine, Münsingen, Switzerland). SUMMARY OF BACKGROUND DATA: The currently available peer-reviewed English-language medical literature addressing the use of the dynamic stabilization systems is limited. Indications, clinical results, and implant failure of Dynesys after microsurgical decompression are still controversial. METHODS AND RESULTS: This study included a total of 37 consecu

Quadriplegia caused by push-up exercises.

This paper describes a case of a 52-year-old male patient with spinal cord injury caused by doing push-up exercises in the prone position. To our knowledge, no previous report has been published describing a case of spinal cord injury caused by push-up exercise. These days, sport is recommended not only to the young but also to older persons. However, to the older, who inevitably have some degenerative disease, moderation should be recommended.

[Giant ependymoma of the cauda equina. Long-term development apropos of 7 cases].

Seven cases of giant ependymomas of cauda equina, extending from the conus medullaris to the sacrum, are reported. Mean age of the patients was 36 years. The initial symptoms were not specific, including low back pain and radiculalgias. However, in two cases, the clinical presentation was related to hydrocephalus. At the time of the operation, 6 patients presented with pain, weakness and sphincter dysfunction. The diagnosis of tumor was made after myelogram in all cases. Metrizamide enhanced CT scan has been performed in one case. None patient had preoperative magnetic resonance imaging (MRI)

Probabilistic diagnosis of normal pressure hydrocephalus and other treatable cerebral lesions in dementia.

Clinical profiles of dementia patients, differing with respect to age, presence of gait abnormalities and urinary incontinence are discussed. Epidemiological data, subjective probabilities and clinical reasoning are used to predict a treatable cerebral lesion, i.e., an intracranial space occupying lesion or normal pressure hydrocephalus (NPH). Our calculations help in distinguishing clinically between demented patients who are, and who are not likely to benefit from computed tomography (CT) investigation for treatable lesions and, eventually, from cerebrospinal fluid shunting for NPH. Utility

Neuroimaging of the spine.

This article addresses the subject of available technology and techniques of neuroimaging of the spine. It also reviews broad areas of neuropathology such as congenital anomalies, degenerative joint disease, the postoperative spine, disorders of the spinal cord, neoplasia, trauma, and infection. A general approach to the radiographic diagnosis of spinal disorders is covered.

Spinal cord stimulators: typical positioning and postsurgical complications.

OBJECTIVE: Implantation of a spinal cord stimulator (SCS) is one option for pain control in individuals with chronic lumbosacral radicular or axial lumbar pain. The expected positioning of SCSs based on the location of pain, the types of electrodes (percutaneous vs surgical paddle), and the types of electrode arrays and the potential complications have not been described to date in the radiology literature. MATERIALS AND METHODS: A 5-year retrospective review of the radiology information system at our institution revealed 24 patients with images of 36 SCSs. Those images were reviewed to ident

Palsy of the C5 nerve root after midsagittal-splitting laminoplasty of the cervical spine.

STUDY DESIGN: The imaging characteristics of postoperative C5 nerve root palsy after midsagittal-splitting laminoplasty for cervical myelopathy, including those observed on plain radiography, computed tomography, and magnetic resonance imaging, were analyzed. OBJECTIVE: To investigate the imaging findings that predict occurrence of C5 nerve root palsy after midsagittal-splitting laminoplasty. SUMMARY OF BACKGROUND DATA: There have been several reports on imaging findings for postoperative nerve root palsy after open-door laminoplasty. However, there have been no detailed reports on imaging c

Measurement of the lateral recess angle as a possible alternative for evaluation of the lateral recess stenosis on a CT scan.

Stenosis of the lateral recess in the lumbar spinal canal is a clinical problem, especially in terms of surgical management. Criteria for the diagnosis and surgical treatment of lateral recess stenosis (LRS) are not clearly defined. Several authors have suggested measurement of the lateral recess height (LRH) on computed tomography (CT) scans as a helpful tool for making decisions in regard of management. The present study is based on the assumption that measurement of the lateral recess angle (LRA) may be useful in the clinical management of lateral recess stenosis. The reliability and signif
