Prevalence of Rotator Cuff Tears Among Older Tennis Players and Its Impact on Clinical Findings and Shoulder Function.

CONTEXT: Tennis is a noncontact sport playable at an individual's own pace. Thus, it is popular among the older adults. It is known that older adults often suffer from rotator cuff tears (RCTs), which may affect tennis activity. However, the prevalence and influence of RCTs on tennis play among older players are not well-known. This study aimed to evaluate the prevalence of RCTs in older tennis players and its impact on physical findings and shoulder function. DESIGN: A cross-sectional study. METHODS: Fifty-three tennis players (mean age: 70.7 y; range: 60-83 y) participating in a recreation

Pneumothorax, an Uncommon but Devastating Complication following Shoulder Arthroscopy: Case Reports.

Shoulder arthroscopy is a mature and widely used treatment to deal with various shoulder disorders. It enables faster recovery and decreases postoperative complications. However, some complications related to shoulder arthroscopy cannot be neglected because they could be life threatening. We presented three cases of various clinical manifestations of pneumothorax after shoulder arthroscopy. The first case was a 65-year-old female who underwent arthroscopic rotator cuff repair under general anesthesia and interscalene nerve block in the beach-chair position. The second case was a 58-year-old ma

Bilateral Erecta Luxation: A Case Report and Literature Review.

CASE: A 69-year-old man fell from a height, resulting in direct axial loading while both shoulders were fully abducted. He was referred to the surgical ward, and both shoulders were reduced by closed reduction using the traction-countertraction maneuver. After little improvement of complaints of pain in the right shoulder over 45 days, magnetic resonance demonstrated traumatic rupture of the supraspinatus. CONCLUSION: Inferior dislocation (luxation erecta) is an uncommon event, with bilateral dislocation being even rarer. Treatment should be initiated urgently via closed reduction or, if this

Lose-Dose Administration of Dexamethasone Is Beneficial in Preventing Secondary Tendon Damage in a Stress-Deprived Joint Injury Explant Model.

Secondary joint damage is the process by which a single injury can lead to detrimental changes in adjacent tissue structures, typically through the spread of inflammatory responses. We recently developed an in vitro model of secondary joint damage using a murine rotator cuff explant system, in which injuries to muscle and bone cause massive cell death in otherwise uninjured tendon. The purpose of the present study was to test the ability cytokine-targeted and broad-spectrum therapeutics to prevent cell death and tissue degeneration associated with secondary joint damage. We treated injured bon

Anatomic study of pedicled bipolar teres major transfer for irreparable posterosuperior rotator cuff tears.

BACKGROUND: Treatment of rotator cuff (RC) tears has not included bipolar muscle-tendon transfers to date. The objective of this study was to verify the feasibility of pedicled bipolar teres major (TM) transfer over and under the long head of the triceps brachii (LHT) and compare its versatility with monopolar transfer in a model of supraspinatus (SS) tears in cadavers. METHODS: In 6 shoulders of cryopreserved cadavers, we re-created complete SS tears, conducting monopolar and bipolar TM transfers over and under LHT. We compared the morphology of the SS and TM, defect coverage, angle between

Rotator cuff tear healing process with graft augmentation of fascia lata in a rabbit model.

BACKGROUND: Fascia lata augmentation of massive rotator cuff tears has shown good clinical results. However, its biological effect during the early healing process is not clearly understood. The purpose of the study was to evaluate the biological efficacy of fascia lata augmentation during the early healing process of rotator cuff tears using a rabbit rotator cuff defect model. METHODS: The infraspinatus tendon was resected from the greater tuberosity of a rabbit to create a rotator cuff tear. The tendon edge was directly sutured to the humeral head. The rotator cuff repaired site was augment

Fatty replacement of rotator cuff in brain-injured patients is associated with hemiplegic arm function, but not with tendon tear: A multicenter study.

BACKGROUND: The association between fatty replacement of rotator cuff and hemiplegic upper extremity function has not been defined yet. Moreover, the relationships among rotator cuff tears, shoulder pain, spasticity, and fatty replacement were not clearly studied. OBJECTIVE: We investigated the association of fatty replacement of the supraspinatus and infraspinatus muscles with tendon tear in stroke or brain-injured patients. METHODS: A total of 72 hemiplegic patients were enrolled in 3 hospitals, and ultrasonography of both shoulders was performed once. Fatty replacement of the supraspinatu

Fluid temperatures during arthroscopic subacromial decompression using a radiofrequency probe.

Our aim was to investigate the temperatures reached in the subacromial space during radiofrequency ablation of the subacromial bursa, in order to see if the probes generate sufficiently high temperatures to cause chondrolysis of the articular surfaces in the glenohumeral joint. We recorded the maximum temperatures in the subacromial space during arthroscopic subacromial decompression on 30 consecutive patients using a sterile digital temperature probe (Series 400, DeRoyal, TN, USA). Both the mean (27.8 degrees C) and maximum (41.8 degrees C) temperatures reached were below the experimental thr

Out of the ring and into a sling: acute latissimus dorsi avulsion in a professional wrestler: a case report and review of the literature.

We present a 29-year-old professional wrestler who sustained a traumatic latissimus dorsi tendon rupture from its humeral insertion. To our knowledge, this report is the first to describe the use of two small anterior axillary incisions to repair a traumatic avulsion of the latissimus dorsi. Our new surgical approach is an alternative treatment for highly competitive, muscular athletes, while taking the associated nerves and cosmetic appearance into consideration.

The rotator crescent and rotator cable: an anatomic description of the shoulder's "suspension bridge".

Twenty fresh frozen cadaver shoulders were dissected in order to study the rotator cable-crescent complex. The rotator crescent is a term that we have used to describe the thin, crescent-shaped sheet of rotator cuff comprising the distal portions of the supraspinatus and infraspinatus insertions. The crescent was found to be bounded on its proximal margin by a thick bundle of fibers that we have called the rotator cable. This cable-crescent configuration was found to consistently span the insertions of supraspinatus and infraspinatus tendons. The dimensions of the rotator cable and crescent we

Pathomechanics of the throwing shoulder.

Many anatomic, physiological, and biomechanical alterations have been observed in overhead athletes who present with painful shoulders. This is probably due to the complex kinetic chain mechanics required in the overhead throwing or serving motion. Any alteration along the kinetic chain can result in deficits in force production or increase in joint loads in other parts of the chain. The "disabled throwing shoulder" (DTS) is a general term that describes the limitations in function that exist in symptomatic overhead athletes. DTS typically results from a "cascade to injury" with alterations in

GLP-1 receptor agonist suppresses fatty infiltration while improving range of motion and electromyographic function in a chronic rotator cuff tear rat model.

HYPOTHESIS/BACKGROUND: Fatty infiltration (FI) and muscle atrophy following rotator cuff (RC) tears are largely irreversible and are major determinants of poor surgical outcomes, increased re-tear risk, and long-term functional disability. No pharmacologic therapies have been validated to prevent or reverse these degenerative changes. Glucagon-like peptide-1 receptor agonists (GLP-1RAs), including liraglutide, have demonstrated antiadipogenic and tissue-preserving effects in other organ systems, suggesting potential application in RC-related muscle degeneration. We hypothesized that systemic l

Prevalence and risk factors for stem subsidence following reverse shoulder arthroplasty using on-lay press-fit short stems.

INTRODUCTION: In reverse shoulder arthroplasty (RSA), press-fit short stems offer benefits such as reduced operative time and bone preservation. However, stem subsidence has been reported, although the risk factors remain unclear. This study aimed to determine the prevalence and risk factors for stem subsidence in RSA with on-lay press-fit short stems, hypothesizing that a lower filling ratio (FR) would correlate with subsidence. MATERIALS AND METHODS: This retrospective analysis included patients who underwent RSA using on-lay press-fit short stems between 2017 and 2023, with a minimum follo

3D Muscle Volume and 3D Fat Fraction After Successful and Failed Arthroscopic Rotator Cuff Repair at 5-Year Follow-up.

BACKGROUND: The results of rotator cuff (RC) repair surgery can be influenced by the presence of muscle atrophy and fatty infiltration. Three-dimensional (3D) quantitative data regarding those degenerative muscle changes after successful or failed RC repair are rare in the current literature. HYPOTHESIS/PURPOSE: The purpose of this study was to analyze muscle volume and fatty infiltration of the supraspinatus muscle after successful and failed arthroscopic RC tendon repair, with a minimum follow-up of 5 years. It was hypothesized that RC muscle volume and fatty infiltration would improve afte

Factors affecting the onset and progression of rotator cuff tears in the general population.

While previous studies have revealed factors affecting the progression of rotator cuff tear (RCT), none have yet described factors affecting its onset. The purpose of this longitudinal observational study was to analyze factors affecting the RCT onset and progression in the general population. The present study included 185 shoulders from 93 participants who completed all the examinations in both 2012 and 2017. Participants received a questionnaire with age, gender, arm dominance, and presence of pain at rest, in motion, and at night. The range of motion (ROM), simple shoulder test (SST) were
