INTRODUCTION: Parasitic infection can present with many different clinical manifestations.
EXEGESIS: A 77 year-old Russian woman, who's been living in France since 50 years was admitted for polyarthritis, myalgia, fever, abdominal pain, and eosinophilia simulating polyarterisis nodosa. Before admission, she was treated by steroids for polymyalgia rheumatica. The diagnosis of Strongyloides stercoralis was performed by parasitologic analysis of feces and colic biopsies. The outcome was favourable under treatment by ivermectine and steroid withdrawal.
CONCLUSION: S. stercoralis can be associate

Strongyloidosis is an parasitic disease, caused by an intestinal nematode endemic in tropic and subtropic regions. In Central Europe it occurs only sporadically. The infective larvae in the soil penetrate the human skin. Following circulation through the lungs the larvae settle in the small intestine and mature into adult worms. Chronic strongyloidosis recurring up to 15 years is possible through endogenous autoinfection. Clinical feature of the disease are gastrointestinal symptoms, hypereosinophilia and skin rashes. We describe the case of an 81-year-old woman who presented with scaly exanth

Disseminated strongyloidosis has been recognized with increasing frequency, often in patients who are immunocompromised or have received steroid therapy. In addition, disease due to cytomegalovirus (CMV) is noted in immunodeficient hosts. We report on a 55-year-old Puerto Rican man who received steroid treatment for orpharyngeal pemphigus vulgaris and developed abdominal symptoms with alternating constipation and diarrhea. The clinical work-up did not reveal specific abnormalities, but the patient died of cardiopulmonary failure. At the postmortem examination, the patient had evidence of stron

Nematode parasites of warm-blooded hosts use chemical and thermal signals in host-finding and in the subsequent resumption of development. The free-living nematode Caenorhabditis elegans is a useful model for investigating the chemo- and thermosensory neurons of such parasites, because the functions of its amphidial neurons are well known from laser microbeam ablation studies. The neurons found in the amphidial channel detect aqueous chemoattractants and repellants; the wing cells-flattened amphidial neurons-detect volatile odorants. The finger cells-digitiform amphidial neurons-are the primar

A 73-year-old woman was admitted to our hospital complaining of dyspnea, fever and general edema. Chest roentgenogram showed bilateral pleural effusion and cardiomegaly. Cardiovascular examination demonstrated atrial tachycardia and left ventricle dysfunction, suggesting congestive heart failure. She was sero-positive for human T-cell lymphoma virus I (HTLV-I). The dyspnea and general edema improved after therapy for heart failure. Because the pleural effusion persisted after therapy, thoracentesis was performed. The pleural effusion was an exudate, and Strongyloides sterocoralis was detected

In the present case report a 50-year-old woman is presented with a Strongyloides stercoralis infection after a one-month stay in Sri Lanka. She showed a severe involvement of the central nervous system without a correlation of an immunosuppression. Strongyloides stercoralis is regarded as the cause of the cerebral involvement. Other possible causes are also discussed.

Mongolian gerbils (Meriones unguiculatus) were infected with 100, 1,000 and 10,000 Strongyloides venezuelensis infective larvae (L3) to determine the duration of fecal egg output. Eggs per gram of feces (EPG) were counted chronologically, and adult worms in the small intestine were recovered on 100 days after infection. Changes of EPG in Mongolian gerbils infected with 10,000 L3 also were observed for 450 days. EPG was stable for each dose after the peak value noted 9 or 10 days after the infection. The EPG and number of worms recovered were dependent on the dose. The high EPG value continued

A 65-year-old man with steroid-dependent chronic airflow obstruction presented with progressive dyspnea and weight loss. Travel history included a military tour in southeast Asia. A chest roentgenogram revealed hyperexpanded lung fields with diffusely increased interstitial markings. The Papanicolaou stain of expectorated sputum demonstrated the rhabditiform larvae of Strongyloides stercoralis. Endemic areas of infection include the southeastern United States, Puerto Rico, Central America, the Pacific basin, and central Africa. In recent immigrant groups and veterans of the Vietnam conflict, r

Mast-cell growth factor (MCGF) activity in the media conditioned by mesenteric lymph node or spleen cells from Strongyloides ratti-infected C57BL/6 mice was examined by using factor-dependent cell line FDC-P2 or bone marrow-derived, cultured mast cells (BMMC) as indicators. Mesenteric lymph node cells from infected mice spontaneously released MCGF activity by culturing for 24 h, showing peak production on days 5-7. MCGF production by mesenteric lymph node cells was augmented after stimulation with adult worm antigen or with concanavalin A (con A). The peak of MCGF production by antigen-stimula

In order to assess the role of inflammation and its components in spontaneous cure of Strongyloides ratti infestation, rats were treated with non-steroid anti-inflammatory agents (indometacin; sodium salicylate) or with antagonists of certain mediators (dexchlorpheniramin; cyproheptadin, promethazin). Results were compared with those obtained in similar treatments of rats infested by other Nematoda which also give rise to spontaneous cure: especially Trichinella spiralis, and Nippostrongylus brasiliensis. Coordinating the various findings made it possible to devise a pattern accounting for the

In patients with compromised host defenses, diseases caused by protozoans and nematodes appeared, a few years ago, to be declining in importance. However, the outbreak of the acquired immunodeficiency syndrome (AIDS) among homosexual men, parenteral drug abusers, and other groups has made it necessary for physicians to familiarize themselves again with the manifestations of these diseases in abnormal hosts. The groups of patients at greatest risk, the usual clinical syndromes in abnormal hosts, and current approaches to diagnosis, treatment, and prevention of infections due to Pneumocystis car

The pharmacokinetics of thiabendazole and its metabolites were determined after the first dose and during hemodialysis and hemoperfusion in an anephric female patient treated for a strongyloides infection. The half-live, volume of distribution, and clearance for thiabendazole were 1.17 hour, 2.76 liters/kg, and 27.2 ml/min . kg, respectively. While thiabendazole and the 5-OH metabolite did not accumulate during multiple dosing, the glucuronide and sulfate esters accumulated extensively despite hemodialysis and hemoperfusion. Hemodialysis clearances for all compounds were poor. Hemoperfusion re
