Rapid transition from methadone to buprenorphine using naltrexone-induced withdrawal: A case report.

Background: Patients taking methadone for opioid use disorder may desire transition to buprenorphine for a number of reasons. However, the current recommended approach for this transition generally takes weeks to months as an outpatient, causing considerable discomfort to the patient and a heightened risk of relapse during the transition period. Case: We describe the case of a patient on methadone maintenance who was rapidly transitioned to buprenorphine because of her desire to not return to her methadone clinic. In order to rapidly transition the patient from methadone to buprenorphine, nalt

[Outcomes of comprehensive inpatient opiate detoxification in patients re-admitted to treatment within one year].

OBJECTIVE: Do patients readmitted to inpatient detoxification within a short time show favourable treatment outcomes? METHODS: Data were from a treatment facility providing for comprehensive opiate detoxification treatment. It was recorded whether consecutively admitted patients (n = 169) were re-admitted within 1 year after discharge. Outcome data were collected from patient files. RESULTS: There were 43 % readmissions within 1 year. Readmission was predicted by lack of transfer to drug-free follow-up treatment, longer duration of opiate dependence, and comorbid axis 1 disorders. Rates of r

Effects of THC and lofexidine in a human laboratory model of marijuana withdrawal and relapse.

INTRODUCTION: Individuals seeking treatment for their marijuana use rarely achieve sustained abstinence. OBJECTIVES: The objectives of the study are to determine if THC, a cannabinoid agonist, and lofexidine, an alpha(2)-adrenergic receptor agonist, given alone and in combination, decreased symptoms of marijuana withdrawal and relapse, defined as a return to marijuana use after a period of abstinence. MATERIALS AND METHODS: Nontreatment-seeking, male volunteers (n = 8), averaging 12 marijuana cigarettes/day, were maintained on each of four medication conditions for 7 days: placebo, tetrahydr

Step training-dependent plasticity in spinal cutaneous pathways.

Plasticity after spinal cord injury can be initiated by specific patterns of sensory feedback, leading to a reorganization of spinal networks. For example, proprioceptive feedback from limb loading during the stance phase is crucial for the recovery of stepping in spinal-injured animals and humans. Our recent results showed that step training modified transmission from group I afferents of extensors in spinal cats. However, cutaneous afferents are also activated during locomotion and are necessary for proper foot placement in spinal cats. We therefore hypothesized that step training would also

Short-term effect of apraclonidine on intraocular pressure in glaucoma patients receiving timolol and pilocarpine.

A placebo-controlled, prospective study was undertaken to determine whether apraclonidine has an additive effect in eyes treated with timolol and pilocarpine. Twelve patients with primary open-angle glaucoma while on continuing therapy with timolol 0.5% twice daily and pilocarpine 4% four times a day in both eyes for 3 months were included in the study. A single drop of apraclonidine 1% or placebo was added to treatment with timolol and pilocarpine. Intraocular pressure (IOP) was measured on both days before a single drop of test medications in both eyes as well as 2, 4, 6, 8 and 12 h later. T…

Potency of infused spinal antinociceptive agents is inversely related to magnitude of tolerance after continuous infusion.

By employing a constant-rate (1 microliter/hr) constant-dose intrathecal infusion pump in rats, groups received a 7-day infusion of one of three concentrations (in nmol/hr) of each of the following receptor preferring ligands: morphine (mu: 2, 6, 20), sufentanil (mu: 0.06, 0.2, 0.6), DAMGO (mu: 0.1, 0.3, 1.0), DADLE (delta: 2, 6, 20) or ST-91 (alpha-2: 3, 10, 30). On day 7 the magnitude of tolerance was assessed by establishing i.t. dose-response curves for the effect of the chronic drug given as a bolus. Each infused animal was used for one bolus injection. It was observed that for all drugs,

A Disproportionality Analysis of Drug-Drug Interactions of Tizanidine and CYP1A2 Inhibitors from the FDA Adverse Event Reporting System (FAERS).

INTRODUCTION: Tizanidine is primarily metabolized via cytochrome P450 (CYP) 1A2 and therefore medications that inhibit the enzyme will affect the clearance of tizanidine, leading to increased plasma concentrations of tizanidine and potentially serious adverse events. OBJECTIVES: Our aim was to study the occurrence of adverse events reported in the FDA Adverse Event Reporting System (FAERS) involving the combination of tizanidine and drugs that inhibit the metabolic activity of CYP1A2. METHODS: A disproportionality analysis of FAERS reports from 2004 quarter 1 through 2020 quarter 3 was condu

α2A- and α2C-Adrenoceptors as Potential Targets for Dopamine and Dopamine Receptor Ligands.

The poor norepinephrine innervation and high density of Gi/o-coupled α2A- and α2C-adrenoceptors in the striatum and the dense striatal dopamine innervation have prompted the possibility that dopamine could be an effective adrenoceptor ligand. Nevertheless, the reported adrenoceptor agonistic properties of dopamine are still inconclusive. In this study, we analyzed the binding of norepinephrine, dopamine, and several compounds reported as selective dopamine D2-like receptor ligands, such as the D3 receptor agonist 7-OH-PIPAT and the D4 receptor agonist RO-105824, to α2-adrenoceptors in cortical

But it's not all there.

A 44-year-old man presented with acute onset headache and isolated anisocoria. Dilation lag was noted in the smaller pupil and Horner syndrome was suspected despite the lack of eyelid ptosis. Cocaine testing confirmed the clinical diagnosis, and urgent neuroimaging found an internal carotid artery dissection. The patient was managed with systemic anticoagulation with a good final outcome. Horner syndrome should be suspected in any patient with aniscoria and dilation lag, despite the absence of eyelid ptosis or other associated findings.

Botulinum neurotoxin versus tizanidine in upper limb spasticity: a placebo-controlled study.

BACKGROUND: While spasticity is commonly treated with oral agents or botulinum neurotoxin (BoNT) injection, these treatments have not been systematically compared. METHODS: This study performed a randomised, double-blind, placebo-controlled trial to compare injection of BoNT-Type A into spastic upper limb muscles versus oral tizanidine (TZD), or placebo, in 60 subjects with upper-limb spasticity due to stroke or traumatic brain injury (TBI). Wrist flexors were systematically injected, while other upper limb muscles were injected as per investigator judgement. Participants were randomised into

[Effects of induction of anesthesia on hemodynamics in children with deformities of the spinal cord when turning to the position lying on the belly].

The investigation was devoted to assessment of the reaction of blood circulation to turning to the position "lying on the belly" in different variants of induction of anesthesia to children aged from 7 to 17 years having deformities of the spinal cord. The following combinations were compared: Ketamin, Fentanyl, and inhalation with nitrous oxide; Propofol and Fentanyl; Thiopental, Clofelin, Fentanyl and inhalation with nitrous oxide; Propofol, Clofelin and Fentanyl. The minimal changes in blood circulation were found to occur when using the combination of Thiopental, Clofelin and Fentanyl.

Circulating growth hormone isoforms in girls with Turner's syndrome.

To evaluate the presence of different GH isoforms in serum of girls with Turner's syndrome, we measured the serum GH content using RIAs with three different site-specific monoclonal antibodies (MAbs). We compared the results to those obtained with authentic GH and GH isoforms. Compared to pituitary GH (mol wt, 22K daltons) as the standard for all three MAbs, serum from girls with Turner's syndrome did not displace tracer [125I]GH equally with all three MAbs. The relative amounts of GH-immunoreactive material found in Turner's syndrome were different from the amounts observed in normal adults a

Effects of tizanidine on morphine physical dependence: attenuation and intensification.

It has previously been shown that subchronic and acute administration of L-asparaginase and glutaminase inhibitors D-Aspartic acid (D-ASP) and prolyl-leucyl-glycinamide (PLG) intensifies and attenuates morphine (M) physical dependence, respectively, by the inhibition of ASP and glutamic acid (GLU) production, and subsequently their normal releases. Tizanidine (TIZ) has long been known to be an alpha 2-adrenoceptor agonist and inhibitor of ASP and GLU release. Therefore, in this study TIZ has been administered subchronically during the development of M physical dependence to rats in which M-con

Comparative Analysis of the Postoperative Analgesic Effects of Caudal Epidural Injection of Ropivacaine Combined With Fentanyl Versus Ropivacaine Alone in Lumbosacral Spine Surgeries: A Randomized Double-Blinded Study.

INTRODUCTION: A single injection of local anesthetic as a caudal epidural block provides pain relief for 2-4 hours. This duration can be extended by adding adjuvants such as opioids (morphine, fentanyl, buprenorphine, tramadol), ketamine, α2 agonists (dexmedetomidine, clonidine), and adrenaline. Caudal analgesia also reduces the need for intravenous opioids during and after surgery, which helps avoid the systemic side effects of opioids. Additionally, adjuncts such as opioids synergistically augment the analgesic properties of caudal epidural anesthetics without escalating motor block. Combini…
