Complications of endoscopic third ventriculostomy: a systematic review.

INTRODUCTION: Endoscopic third ventriculostomy (ETV) is an established treatment for hydrocephalus. Most studies focus on success rates, and complications are insufficiently charted. The aim of this study was to perform a systematic review of ETV complications. METHODS: A Medline search discovered 24 series of ETV (seven in children, five in adults, and 12 in a mixed-age group) with detailed complications reports. RESULTS: The analysis included 2,672 ETVs performed on 2,617 patients. The cause of hydrocephalus was aqueductal stenosis in 25.9%, tumor 37.0%, meningomyelocele-Chiari II 6.1%, po

First Clinical Evaluation of a Modular Lumboperitoneal Shunt System With a Gravitational Unit for Communicating Hydrocephalus: A Retrospective Case Series.

BACKGROUND AND OBJECTIVES: In Europe and North America, communicating hydrocephalus (HC) is commonly treated with ventriculoperitoneal shunts, whereas lumboperitoneal shunts (LPS) are preferred in Japan and other Asian countries. However, no LPS valve with an integrated gravitational unit had been available until recently, leaving overdrainage as a persistent complication. The objective of this study was to report initial clinical experiences with a novel modular LPS valve system that incorporates a gravitational unit and to assess its safety, feasibility, and early clinical outcomes across va

Shunting for hydrocephalus: analysis of techniques and failure patterns.

BACKGROUND: Hydrocephalus is characterized by ventricular dilatation because of progressive accumulation of cerebrospinal fluid. Normal pressure hydrocephalus (NPH) affects a subset of patients representing a reversible clinical triad of gait disturbance, urinary incontinence, and dementia with normal cerebrospinal fluid pressure and composition. Various shunting procedures have been used for treatment, but techniques and outcomes remain under debate. The objective of this study was to evaluate the clinical outcomes of 232 patients with and without NPH after the first-time Ventriculoperitoneal

Brain Ischemia in Alzheimer's Disease May Partly Counteract the Disruption of the Blood-Brain Barrier.

BACKGROUND: In normal pressure hydrocephalus (NPH) there is blood-brain barrier (BBB) disruption, which should increase the CSF formation rate (CSFfr) and, therefore, also increase the intracranial pressure (ICP). However, the ICP is normal in NPH. A lumped parameter study was performed to look at the interrelation between the ICP, cerebral blood flow (CBF), and the degree of BBB disruption in NPH. The model suggested that the CSFfr could be reduced in this condition if the BBB disruption was moderated by a reduction in the capillary transmural pressure (TMP) secondary to arteriolar constricti

Non-invasive intracranial pressure monitoring in management of slit ventricle syndrome-case report.

Slit ventricle syndrome (SVS) is a condition of severe headaches, usually intermittent, suffered by patients after ventricular shunting for hydrocephalus with normal or smaller ventricle size; although in some cases, the correct diagnosis and management can be a significant challenge. This case report aims to highlight the application of non-invasive ICP monitoring (nICP) technology in the diagnosis and management of SVS, providing insights into its benefits and limitations. A 6-year-old boy with a history of ventriculoperitoneal (VP) shunt placement at 11 months of age for communicating hydro

The Cerebral Aqueduct Compliance: A Simple Morphometric Model.

BACKGROUND AND OBJECTIVES: This work aimed to identify different configurations of the adytum of the cerebral aqueduct suggesting its safe n … e pathology (colloid cyst/normal anatomy, intraventricular hemorrhage, tetraventricular obstructive hydrocephalus, normal pressure hydrocephalus, and distal membranous aqueductal stenosis). A simple geometrical scheme was applied to the endoscopic anatomy of the aqueductal adytum in relation to the posterior commissure to measure its pathologic deformations. Eventual damages to …

Cerebral hemodynamic monitoring combined with infusion test in hydrocephalus.

INTRODUCTION: Disturbance in cerebrospinal fluid (CSF) circulation may overlap with abnormality of cerebral blood flow (CBF) in hydrocephalus. Transcranial Doppler (TCD) ultrasonography is a non-invasive technique able to assess CBF velocity (CBFv) dynamics in response to a controlled rise in ICP during CSF infusion tests. RESEARCH QUESTION: Which TCD-derived cerebral hemodynamic parameters change during controlled rise of ICP, and in which direction? MATERIAL AND METHODS: Infusion tests combined with TCD monitoring and non-invasive monitoring of arterial blood pressure (ABP) were conducted

Persisting Embryonal Infundibular Recess (PEIR): Two Case Reports and Systematic Literature Review.

CONTEXT: The persisting embryonal infundibular recess (PEIR) is a rare anomaly of the floor of the third ventricle with a debated pathogenesis. It can be a cause of misdiagnosis in the case of cystic lesions of the sellar and suprasellar area. OBJECTIVE: To describe two recently evaluated cases and provide a systematic literature review. EVIDENCE ACQUISITION AND CASE DESCRIPTIONS: PEIR has been previously reported in six adult patients. Because in some cases it was associated with hydrocephalus and/or empty sella, a possible role of altered intracranial pressure in PEIR formation has been po

Intracranial compartment volumes in patients with enlarged ventricles assessed by magnetic resonance-based image processing.

Magnetic resonance image-based computerized segmentation was used to measure the volumes of the brain, gray and white matter components, and to identify regions with prolonged enhancement on T2-weighted imaging, such as periventricular or deep white matter hyperintensities. The authors also determined the volumes of the ventricles and subarachnoid space in control subjects and in patients with: 1) aqueductal stenosis (AS); 2) other causes of obstructive hydrocephalus (OH); 3) Alzheimer's disease (AD); and 4) normal-pressure hydrocephalus (NPH). In AS the volume of the brain was smaller, wherea

CSF CXCL13 is elevated in patients with CIDP and may reflect higher disease activity.

To evaluate B-cell involvement in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), 11 patients with CIDP, 8 patients with Guillain-Barré syndrome and 13 patients with idiopathic normal pressure hydrocephalus (iNPH) were studied. CSF cytokine and chemokine (IL-10, IL-15, TNF-α, TGF-β1, GM-CSF, BAFF, CXCL10, and CXCL13) levels were measured by ELISA. The CSF CXCL13 level was significantly higher in patients with CIDP than in those with iNPH. The CSF CXCL13 level was significantly higher in CIDP patients with higher annualized relapse rates and higher modified Rankin scale scores

Variably protease-sensitive prionopathy: a novel disease of the prion protein.

Variably protease-sensitive prionopathy (VPSPr) is a novel disease involving the prion protein (PrP) that has clinical similarities with non-Alzheimer's dementias especially frontotemporal dementia, diffuse Lewis body disease, and normal pressure hydrocephalus. VPSPr can be distinguished from sporadic Creutzfeldt-Jakob disease (sCJD) especially for the characteristics of the abnormal PrP. Furthermore, although VPSPr like sCJD affects patients with the three PrP genotypes as determined by the common methionine/valine polymorphism, the allelic prevalence is very different in the two diseases. [Dementias--diagnosis, differential diagnosis and therapy].

The most common cause of primary (degenerative) dementia in old age is Alzheimer's disease, with vascular forms of dementia taking second place. Endocrinopathies, normal pressure hydrocephalus and space-occupying lesions are frequent causes of secondary dementia. A multitude of further, more rare clinical presentations results in a wide differential diagnostic spectrum necessitating a careful diagnostic work-up of the demential syndrome before considering treatment. This work-up must include clinical, laboratory and equipment-based investigations. The multimodal therapeutic approach to dementi

Primary intracranial arachnoid cyst in the elderly: a survey on 39 cases.

Thirty-nine cases of primary intracranial arachnoid cyst in the elderly, including one case of our own, were analyzed. Clinical characteristics in these patients were as follows. (1) The number of patients decreased with age, but there were no differences according to sex. (2) Clinical manifestations were similar to those of chronic subdural haematoma or normal pressure hydrocephalus including dementia, urinary incontinence, and hemiparesis. General symptoms such as headache and seizures were also present. (3) Surgery was performed in most patients with generally good outcome regardless of ope

Cerebrospinal fluid levels of cyclic nucleotides in non-neoplastic hydrocephalus: preliminary report.

Cerebrospinal fluid levels of cyclic nucleotides in children with hypertensive communicating hydrocephalus secondary to leptomeningitis were investigated. 3':5'-cAMP and 3':5'-cGMP values were uniformly increased in pre-operative findings, suggesting a "passive" behaviour of the blood brain barrier after the inflammatory lesion. After ventriculo-peritoneal shunt, the mean ratio between the two nucleotides gradually became negative, with a marked increase of cGMP levels, probably related to selective barrier damage. In patients with shunt obstruction, the altered cAMP/cGMP ratio showed a gradua

FSI simulation of CSF hydrodynamic changes in a large population of non-communicating hydrocephalus patients during treatment process with regard to their clinical symptoms.

3D fluid-structure interaction modelling was utilized for simulation of 13 normal subjects, 11 non-communicating hydrocephalus (NCH) patients at pre-treatment phase, and 3 patients at five post-treatment phases. Evaluation of ventricles volume and maximum CSF pressure (before shunting) following results validation indicated that these parameters were the most proper hydrodynamic indices and the NCH type doesn't have any significant effect on changes in two indices. The results confirmed an appropriate correlation between these indices although the correlation decreased slightly after the occur
