Endoscopic sphincterotomy for acute pancreatitis: arguments in favour.

Pathogenetic factors of acute biliary pancreatitis include persistent or transient, absolute or relative, obstruction of Vater's ampulla or the main pancreatic duct and reflux of bile into the pancreatic ducts or of pancreatic juice into the bile ducts. There is an increasing incidence of persisting common bile duct stones in the more severe forms of acute biliary pancreatitis, in patients who die, and in patients with pancreatic necrosis. All these findings provide a rational basis for the use of early biliary decompression in patients with acute biliary pancreatitis. Endoscopic sphincterotom

Pruritus of chronic cholestasis.

Pruritus is a challenging clinical problem which often complicates chronic cholestatic liver disease. For practical purposes, cholestasis may be defined as impaired hepatocellular secretion of bile and is a feature of a wide variety of liver diseases. Cholestasis is usually suspected clinically when a patient presenting with jaundice or pruritus is found to have an elevation in serum alkaline phosphatase activity disproportionate to increases in serum aminotransferase levels. Early imaging by ultrasonography, computerized tomography, or cholangiography is important to address the possibility o

Value of bile acid determination for the diagnosis of neonatal jaundice.

Serum concentrations of bile acids and bilirubin, and activity of alanine transferase and alkaline phosphatase as well as bile acid and bilirubin levels in duodenal contents were determined in 90 infants aged 1-44 weeks (including 49 under 10 weeks of age) admitted to hospital for prolonged jaundice. Infants with extrahepatic cholestasis were found to have statistically higher serum bile acid and bilirubin concentrations. Oral administration of cholestyramine produced a statistically significant decrease in serum bile acids and bilirubin in infants with intrahepatic cholestasis under 10 weeks

Feline hepatic disease.

Species differences in anatomy, physiology, and biochemistry lead to many dissimilarities between the canine and feline liver. Major differences exist in the interpretation of liver function tests, the significance of biochemical jaundice, the consequences of anorexia, and the efficiency of hepatic metabolic systems. Biochemical alterations in total bilirubin, ALT, and SAP may indicate the presence of disease in the feline liver. It is, however, impossible to make accurate diagnoses without liver biopsy. A liver biopsy can provide a diagnosis and prognosis and can guide the therapeutic plan. T

Machine learning-based prediction model for post-ERCP cholangitis in patients with malignant biliary obstruction: a retrospective multicenter study.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is the preferred palliative treatment for patients with unresectable malignant biliary obstruction (MBO), which can relieve biliary obstruction and prolong survival. Post-ERCP cholangitis (PEC) affects the survival of MBO patients. Early prediction of PEC risk is crucial for developing individualized treatment plans and improving prognosis. Currently, no predictive models exist for clinical practice. This study aims to develop and validate an interpretable machine learning prediction model using multicenter cohorts to predict th

Endoscopic Retrograde Cholangiopancreatography for Choledocholithiasis: Clinical Outcomes and Predictors of Complications.

Background Common bile duct (CBD) stones are a frequent cause of biliary obstruction and may lead to life-threatening complications such as cholangitis, pancreatitis, and obstructive jaundice if not promptly managed. Endoscopic retrograde cholangiopancreatography (ERCP) has largely replaced open and laparoscopic bile duct exploration as the preferred therapeutic approach due to its minimally invasive nature, shorter hospital stay, and high efficacy. However, its success and safety depend on multiple clinical and procedural factors that require evaluation in local settings. Objective This study

Does adjunctive endoscopic biliary drainage improve outcomes beyond glucocorticoids alone in IgG4-related pancreatobiliary disease?

BACKGROUND: Immunoglobulin G4 (IgG4)-related pancreatobiliary disease typically manifests as obstructive jaundice, complicating therapeutic choices. Glucocorticoids (GCs) are the primary treatment with many challenges, including high recurrence rates and patient tolerance variability. Endoscopic biliary drainage (EBD) is used to alleviate obstruction, but its role during treatment in IgG4-related pancreatobiliary disease is still contested. There remains a paucity of research comparing the potential benefits of combination therapy with GCs and EBD versus GCs monotherapy. METHODS: This retrosp

Groove Pancreatitis: A Rare form of Chronic Pancreatitis.

CONTEXT: Groove pancreatitis is a rare form of chronic pancreatitis affecting the "groove" of the pancreas among the pancreatic head, duodenum, and common bile duct. The exact cause is unknown, although there are associations with long-term alcohol abuse, smoking, peptic ulcer disease, heterotopic pancreas, gastric resection, biliary disease, and anatomical or functional obstruction of the minor papilla. The diagnosis can be challenging. Endoscopic ultrasound (EUS) and magnetic resonance cholangiopancreatography are the preferred imaging modalities. The treatment of choice is conservative alth

Transpancreatic precut sphincterotomy for cannulation of inaccessible common bile duct: a safe and successful technique.

OBJECTIVE: Obstruction of the biliary tract can lead to severe complications. The common treatment of patients with biliary tract obstruction is the decompression by endoscopic procedures. However, cannulation of the common bile duct can be difficult under certain instances because of anatomical variations or obstruction at the biliary entrance level. Needle-knife or precut sphincterotomy has been described as technique to facilitate biliary access in patients with difficult bile duct cannulation. In the current study, we evaluated success and complication rates of a wire-guided transpancreati

[Dilatation of the common bile duct without visible obstruction at endoscopic retrograde cholangiography. Description and development].

The aim of this study was to specify the signs and course of patients with a dilated common bile duct without obstruction. We included patients with a dilated common bile duct of more than 12 mm on endoscopic retrograde cholangiography, and we excluded patients with stones, tumor or other visible obstruction. Two hundred and seven patients (8.4 p. 100 of endoscopic retrograde cholangiography) were included. One hundred and nineteen (57.5 p. 100) had undergone cholecystectomy. Sixty-five p. 100 of patients had signs suggesting biliary tract disease, and 78 p. 100 had biological signs of cholest

Biliary Drainage During Neoadjuvant Chemotherapy in Pancreatic Cancer: Evidence and Practical Recommendations.

Pancreatic cancer frequently presents with obstructive jaundice resulting from distal malignant biliary obstruction. Neoadjuvant chemotherapy (NAC) is increasingly applied in resectable and borderline resectable disease. In this context, uncontrolled cholestasis or cholangitis may hinder timely chemotherapy initiation and cause unplanned hospitalizations and treatment delays; therefore, preoperative biliary drainage is essential to ensure safe and uninterrupted NAC. This review summarizes current biliary drainage strategies during NAC, focusing on key clinical goals, maintaining durable patenc

Exploring the Role of Percutaneous Transhepatic Biliary Drainage in the Interim Management of Paediatric Choledochal Cysts with Cholangitis: A Pilot Study.

BACKGROUND: Choledochal cysts are congenital anomalies of the bile ducts that can lead to significant complications, such as cholangitis, liver fibrosis, and biliary obstruction. This study aims to evaluate the effectiveness and safety of percutaneous transhepatic biliary drainage (PTBD) in managing choledochal cysts with cholangitis in children. METHODS: This non-randomized interventional study included twelve paediatric patients with choledochal cysts and cholangitis treated over two years. Percutaneous transhepatic biliary drainage was performed to relieve biliary obstruction and stabilize

Acute cholangitis due to a Type IV Mirizzi syndrome management strategy of a rare condition: a case report.

INTRODUCTION: Mirizzi syndrome (MS) is a rare complication of chronic cholelithiasis, resulting from an impacted gallstone in the cystic duct or gallbladder neck, leading to common hepatic duct obstruction. Advanced cases, such as Type IV, involve cholecystobiliary fistula formation, posing significant diagnostic and therapeutic challenges. CASE PRESENTATION: We report a case of a 69-year-old woman presenting with jaundice, fever, and right upper quadrant pain. Laboratory tests revealed cholestasis and hepatic cytolysis. Imaging, including ultrasonography computed tomography, and magnetic res

Assessment of safety and patency of 7-mm covered metal stents for preoperative biliary drainage in pancreatic cancer: Prospective multicenter study.

BACKGROUND AND STUDY AIMS: For preoperative biliary drainage of pancreatic cancer (PC), a 10-mm diameter metal stent (MS) is commonly used; however, the rate of pancreatitis is high. It is hypothesized that smaller-diameter MS may reduce the rate of pancreatitis. Therefore, we conducted a multicenter prospective study to evaluate the efficacy and safety of 7-mm MS. PATIENTS AND METHODS: Patients requiring initial biliary drainage for obstructive jaundice caused by PC and scheduled for surgery from six facilities were included. After endoscopic retrograde cholangiography, a 7-mm MS was placed

Comparing the efficacy and complications of Endoscopic Biliary Drainage (EBD) and Percutaneous Transhepatic Biliary Drainage (PTBD) in patients with perihilar cholangiocarcinoma.

INTRODUCTION AND OBJECTIVE: Hilar cholangiocarcinoma (HCCA) is a rare malignancy in patients with biliary disease. If jaundice and obstruction before surgery are left untreated, then they can cause side effects such as cholangitis, delayed tumor treatment, decreased quality of life, and increased mortality. Surgery is the main treatment for HCCA. Therefore, this study was performed to compare the efficacy and complications of percutaneous transhepatic biliary drainage (PTBD) and endoscopic biliary drainage (EBD). METHODS: This cohort study was conducted on 20 patients with biliary obstruction

[Design and application of a new type of biliary-intestinal nutrient tube].

The establishment of a nutritional pathway is the premise and basis of nutritional therapy for patients with malignant tumor. The nasogastric tube, nasoenteric tube, and percutaneous endoscopic gastric/jejunostomy are commonly used clinical pathways for enteral nutrition (EN) therapy. However, these EN pathways are often difficult to establish in patients with malignant obstructive jaundice (MOJ) with pyloric or duodenum primary obstruction. For this reason, a new type biliary-intestinal nutrient tube placed through percutaneous transhepatic cholangiography drainage (PTCD) pathway was designed

A non-klatskin tumor: A case report and review of intrabiliary hydatid cyst rupture.

INTRODUCTION: A 64-year-old lady was diagnosed with having a klatskin type 3A tumor based on imaging, however, an alternative diagnosis was achieved during surgery. PRESENTATION OF CASE: We present a case of a 64-year-old lady who presented for new-onset jaundice and was diagnosed with type 3A klatskin tumor based on MRCP findings. During surgery, it was revealed that the obstruction was caused by a frank intrabiliary hydatid cyst perforation. Choledocoscopy with irrigation, cholangiography, and removal of the mother cyst were performed, and an end-to-end biliary anastomosis over a t-tube was

Foregut atresias and bile duct anomalies: rare, infrequent or common?!

The association of foregut atresias and bile duct anomalies is reportedly rare. We encountered five referrals within 2 years where the secondary diagnosis was missed at operation. Four patients initially presented on antenatal scans as a foregut atresia whereas the fifth presented at nine years with abdominal pain due to a choledochal cyst. The biliary anomalies (cholecysto-hepatic duct, liver cyst and choledochal cysts) in the first four presented as postoperative jaundice during infancy whereas the fifth patient developed subacute intestinal obstruction due to congenital duodenal stenosis at

Epidemiology of typhoid carriers among blood donors and patients with biliary, gastrointestinal and other related diseases.

Enteric fever due to Salmonella Typhi is a major public health problem. Typhoid carriers have high titres of Vi agglutinins in their sera. We worked out the baseline data for Vi agglutinins from 705 healthy blood donors (controls) by ELISA and compared it with 446 patients with biliary, gastrointestinal and other related diseases (cases). The samples were divided into five groups based on the disease condition of the patients from whom they were collected. Group A (n=196) consisted of patients with stones in the gall bladder/common bile duct and Group B (n=27) with gall bladder carcinoma. Grou
