A balanced autosomal translocation (3;9) associated with primary hypogonadism and dorsal spine stenosis.

A man with a balanced translocation between chromosomes 3 and 9 associated with primary hypogonadism and dorsal spine stenosis is reported. The possible significance of this chromosomal abnormality is discussed.

Spinal meningeal diverticula in spontaneous intracranial hypotension: analysis of prevalence and myelographic appearance.

BACKGROUND AND PURPOSE: Spinal meningeal diverticula have been implicated in the pathogenesis of spontaneous intracranial hypotension and have been proposed as both diagnostic features of and therapeutic targets for the condition. We compared the prevalence and myelographic appearance of spinal diverticula in patients with SIH and healthy controls. MATERIALS AND METHODS: Patients satisfying the ICHD-2 criteria for SIH were retrospectively identified. CT myelograms of 19 patients with SIH were compared with CT myelograms of 18 control patients. Images were reviewed by 2 blinded neuroradiologis

Pigmented villonodular synovitis originating from the lumbar facet joint: a case report.

The authors successfully treated a rare case of pigmented villonodular synovitis (PVNS) that originated from the lumbar facet joint (L4-5). A 43-year-old man presented with a complaint of left severe sciatica causing difficulty in walking. Magnetic resonance imaging (MRI) demonstrated an extradural mass on the left side at L4 and the mass compressed the dural tube and was continuous with the left L4-5 facet joint. A computed tomography myelogram revealed an extradural defect of contrast medium at the L4 level and an erosion of the L4 lamina. A total synovectomy with unilateral osteoplastic lam

[Sacral nerve root cysts--a rare cause of bladder dysfunction. Case report and review of the literature].

INTRODUCTION: Asymptomatic cysts of the sacral nerve roots display a prevalence of 5 % and are occasionally demonstrated by MRI of the spine. Depending on their size and localization, arachnoid cysts may cause sacral or perineal pain, radicular sensomotory symptoms and neurogenic bladder and bowel dysfunction by compression of the nerve roots. CASE REPORT: We report on a case of a recently developed neurogenic bladder dysfunction. MRI of the spine demonstrated two large, liquor-filled cysts of 2.5 and 3 cm diameter, bilaterally localized at the nerve roots S2/3. Neurological and urological ex

Hematogenous pyogenic facet joint infection of the subaxial cervical spine. A report of two cases and review of the literature.

Two cases of hematogenous, pyogenic, subaxial cervical facet joint infection are reported, and the literature is reviewed. Infection of the cervical facet joint is a rarely diagnosed condition; only one case has been reported in the literature. Lumbar facet joint infections are also rare but more commonly reported. Approximately one fourth of facet joint infections in the lumbar spine are complicated by epidural abscess formation, which can lead to a neurological deficit. Because of the paucity of reports on cervical facet joint infections, the clinical characteristics of this entity are not w

Early rod-sleeve stabilization of the injured thoracic and lumbar spine.

The rod-sleeve method provides adjustable corrective forces in all directions so as to accomplish anatomic alignment and three-dimensional rigid fixation for acute spinal injuries. The authors studied a prospective series of 135 consecutive cases treated with this new technique. Results showed improved indirect canal decompression and neurologic recovery, few complications, and greater maintenance of correction than previously reported.

Evaluation and Management of Cauda Equina Syndrome.

Cauda equina syndrome is a potentially devastating spinal condition. The diagnosis of cauda equina syndrome lacks sensitivity and specificity, sometimes occurring after irreparable neurological damage has happened. Timely diagnosis and treatment is imperative for optimal outcomes and for avoiding medicolegal ramifications. Cauda equina syndrome results from conditions that compress the nerves in the lumbosacral spinal canal. Although no consensus definition exists, it generally presents with varying degrees of sensory loss, motor weakness, and bowel and bladder dysfunction (the latter of which

JOURNAL CLUB: Incidence of Complications Following Fluoroscopically Guided Lumbar Punctures and Myelograms.

OBJECTIVE: Headaches due to CSF leak are a well-described complication of dural puncture. It is uncertain how long patients should be observed after dural puncture to reduce the risk of headache. Most of the literature has focused on dural punctures performed without fluoroscopic guidance. The purpose of this study was to determine the incidence of complications from fluoroscopically guided dural punctures, with attention to predictive factors such as the length of bed rest after the procedure. MATERIALS AND METHODS: We retrospectively reviewed 2141 fluoroscopically guided dural punctures per

Tethered cord syndrome in low motor level children with myelomeningocele.

The clinical presentation of tethered spinal cord and the results of tethered cord release were examined in a group of 30 low motor level (L3 and below) children with a history of myelomeningocele without concomitant CNS complications. Changes in orthopedic and/or neurologic status formed the basis of consideration for tethered cord release. Clinically, these patients presented with a new onset or recently progressing scoliosis, spasticity with or without contractures, decrease in motor function and low back pain at the site of closure. One or more of these findings was present in all cases an

Computed tomography of sellar and parasellar lesions: indications for metrizamide cisternography.

Sixty-four patients with intrasellar and suprasellar lesions on conventional computed tomography (CT) have been studied retrospectively to determine the indications for metrizamide CT cisternography (MCTC). Enhancing or calcified lesions, like a craniopharyngioma, pituitary adenoma, meningioma or aneurysm are adequately evaluated by conventional CT and MCTC is usually not needed. Determination of the exact suprasellar extent of an enhancing pituitary adenoma is best accomplished with coronal thin-section conventional CT alone. However, for isodense or hypodense suprasellar and intrasellar lesi

Cervical spinal stenosis.

Cervical spinal stenosis occurs at the craniovertebral junction, usually incident to a congenital malformation, or it appears as a developmental defect with diffuse narrowing of the cervical canal. In its acquired form the lesion may be limited to one or two levels, or it may be more extensive and affect three or more segments.
