Surgical Treatment of Subfibular Ossicle in Children: A Retrospective Study of 36 Patients with Functional Instability.

BACKGROUNDS: To evaluate the surgical management of a symptomatic subfibular ossicle after severe ankle sprain with functional instability and pain sequelae in children. METHODS: We analyzed 36 patients complaining of functional instability without laxity, 1 year after an ankle inversion trauma associated with the observation of a subfibular ossicle. We systematically suggested the open excision of the residual ossicles, followed by 6 weeks of immobilization and proprioceptive physiotherapy. Seventeen of them, constituting the "resection" group accepted this surgical approach. The remaining 1

Functional neuroimaging of visuo-vestibular interaction.

The brain combines visual, vestibular and proprioceptive information to distinguish between self- and world motion. Often these signals are complementary and indicate that the individual is moving or stationary with respect to the surroundings. However, conflicting visual motion and vestibular cues can lead to ambiguous or false sensations of motion. In this study, we used functional magnetic resonance imaging to explore human brain activation when visual and vestibular cues were either complementary or in conflict. We combined a horizontally moving optokinetic stimulus with caloric irrigation

Exercise and physical therapy help restore body and self in clients with severe anorexia nervosa.

Exercise in the context of anorexia nervosa is a multifaceted endeavour surrounded by controversy and uncertainty. A broader comprehension of this poorly understood phenomenon is required. Informed by the findings of a body examination of six individuals with anorexia nervosa, as well as exercise science, phenomenology and neurocognition, the purpose of this article is to elaborate on the potential role of exercise and physical therapy in the treatment of anorexia nervosa. The findings of the body assessment include constriction of posture, muscles and pattern of breathing. These bodily restra

Movement-related cortical magnetic fields associated with self-paced tongue protrusion in humans.

Sophisticated tongue movements are coordinated finely via cortical control. We elucidated the cortical processes associated with voluntary tongue movement. Movement-related cortical fields were investigated during self-paced repetitive tongue protrusion. Surface tongue electromyograms were recorded to determine movement onset. To identify the location of the primary somatosensory cortex (S1), tongue somatosensory evoked fields were measured. The readiness fields (RFs) over both hemispheres began prior to movement onset and culminated in the motor fields (MFs) around movement onset. These signa

Spinal Cord Stimulation and Augmentative Control Strategies for Leg Movement after Spinal Paralysis in Humans.

Severe spinal cord injury is a devastating condition, tearing apart long white matter tracts and causing paralysis and disability of body functions below the lesion. But caudal to most injuries, the majority of neurons forming the distributed propriospinal system, the localized gray matter spinal interneuronal circuitry, and spinal motoneuron populations are spared. Epidural spinal cord stimulation can gain access to this neural circuitry. This review focuses on the capability of the human lumbar spinal cord to generate stereotyped motor output underlying standing and stepping, as well as full

Using time to investigate space: a review of tactile temporal order judgments as a window onto spatial processing in touch.

To respond to a touch, it is often necessary to localize it in space, and not just on the skin. The computation of this external spatial location involves the integration of somatosensation with visual and proprioceptive information about current body posture. In the past years, the study of touch localization has received substantial attention and has become a central topic in the research field of multisensory integration. In this review, we will explore important findings from this research, zooming in on one specific experimental paradigm, the temporal order judgment (TOJ) task, which has

Restricted neural plasticity in vestibulospinal pathways after unilateral labyrinthectomy as the origin for scoliotic deformations.

Adolescent idiopathic scoliosis in humans is often associated with vestibulomotor deficits. Compatible with a vestibular origin, scoliotic deformations were provoked in adult Xenopus frogs by unilateral labyrinthectomy (UL) at larval stages. The aquatic ecophysiology and absence of body-weight-supporting limb proprioceptive signals in amphibian tadpoles as a potential sensory substitute after UL might be the cause for a persistent asymmetric descending vestibulospinal activity. Therefore, peripheral vestibular lesions in larval Xenopus were used to reveal the morphophysiological alterations at

The role of the caudal superior parietal lobule in updating hand location in peripheral vision: further evidence from optic ataxia.

Patients with optic ataxia (OA), who are missing the caudal portion of their superior parietal lobule (SPL), have difficulty performing visually-guided reaches towards extra-foveal targets. Such gaze and hand decoupling also occurs in commonly performed non-standard visuomotor transformations such as the use of a computer mouse. In this study, we test two unilateral OA patients in conditions of 1) a change in the physical location of the visual stimulus relative to the plane of the limb movement, 2) a cue that signals a required limb movement 180° opposite to the cued visual target location, o

Cross-modal plasticity preserves functional specialization in posterior parietal cortex.

In congenitally blind individuals, many regions of the brain that are typically heavily involved in visual processing are recruited for a variety of nonvisual sensory and cognitive tasks (Rauschecker 1995; Pascual-Leone et al. 2005). This phenomenon-cross-modal plasticity-has been widely documented, but the principles that determine where and how cross-modal changes occur remain poorly understood (Bavelier and Neville 2002). Here, we evaluate the hypothesis that cross-modal plasticity respects the type of computations performed by a region, even as it changes the modality of the inputs over wh

Disseminated protothecosis diagnosed by evaluation of CSF in a dog.

A 5-year-old female spayed Shetland Sheepdog Mix dog was evaluated for a history of recent seizure activity, progressive hind limb ataxia, polyuria, and polydipsia and no history of gastrointestinal signs. Physical examination findings included conscious proprioceptive deficits, ataxia, and anterior uveitis along with a hypermature cataract in the right eye. Results of a CBC, serum biochemical profile, urinalysis, and computed tomography scan of the brain were unremarkable. Cerebrospinal fluid (CSF) analysis revealed marked eosinophilic pleocytosis and rare organisms consistent with Prototheca

Movement observation activates lower limb motor networks in chronic complete paraplegia.

BACKGROUND: In healthy subjects, observation of actions activates a motor network similar to that involved in the performance of the observed actions. Movement observation could perhaps be applied to functionally sustain brain motor functions when efferent motor commands and proprioceptive feedbacks are disconnected. OBJECTIVE: The authors examined whether observation-induced brain activation is preserved in people with chronic complete spinal cord injury (SCI). METHODS: Nine patients and 12 healthy subjects underwent behavioral assessment and functional magnetic resonance imaging. The SCI p

Spatial perspective and coordinate systems in autoscopy: a case report of a "fantome de profil" in occipital brain damage.

Autoscopic phenomena refer to complex experiences involving the illusory reduplication of one's own body. Here we report the third long-lasting case of autoscopy in a patient with right occipital lesion. Instead of the commonly reported frontal mirror view (fantôme spéculaire), the patient saw her head and upper trunk laterally in side view (fantôme de profil). We found that the visual appearance and completeness of the autoscopic image could be selectively modulated by active and passive movements, without being influenced by imagining the same movements or by tactile and auditory stimulation

Management of the patient with chronic dizziness.

In this review we present a pragmatic approach to the patient with chronic vestibular symptoms. Even in the chronic patient a retrospective diagnosis should be attempted, in order to establish how the patient reached the current situation. Simple questions are likely to establish if the chronic dizzy symptoms started as benign paroxysmal positional vertigo (BPPV), vestibular neuritis, vestibular migraine, Meniere's disease or as a brainstem stroke. Then it is important to establish if the original symptoms are still present, in which case they need to be treated (e.g. repositioning maenouvres

Motion parallax contribution to perception of self-motion and depth.

The object of this study is to mathematically specify important characteristics of visual flow during translation of the eye for the perception of depth and self-motion. We address various strategies by which the central nervous system may estimate self-motion and depth from motion parallax, using equations for the visual velocity field generated by translation of the eye through space. Our results focus on information provided by the movement and deformation of three-dimensional objects and on local flow behavior around a fixated point. All of these issues are addressed mathematically in term

Magnetic resonance imaging and marsupialization of a hemorrhagic intramedullary vascular anomaly in the cervical portion of the spinal cord of a dog.

CASE DESCRIPTION: A 1-year-old female spayed Labrador Retriever was admitted for evaluation of a progressive gait disturbance characterized by tetraparesis and general proprioceptive ataxia in all limbs. CLINICAL FINDINGS: Neurologic examination suggested a dysfunction of the C6-T2 spinal cord segments, which was slightly worse on the right side. Discomfort was suspected upon lateral flexion of the neck. Two magnetic resonance imaging (MRI) examinations at a 3-week interval revealed an intramedullary fluid-filled cavitary lesion adjacent to C7, containing a blood clot. TREATMENT AND OUTCOME:

A fronto-parietal network is mediating improvement of motor function related to repetitive peripheral magnetic stimulation: A PET-H2O15 study.

Repetitive peripheral magnetic stimulation (RPMS) is a focused and painless stimulation method, in which muscle contractions are elicited by depolarization of the terminal motor branches. Clinical-experimental investigations on different disorders of sensorimotor integration in the last decade have shown that RPMS can be used for the rehabilitation of motor functions after stroke. It is supposed that this therapeutic effect is based on the RPMS-induced proprioceptive inflow to the CNS. To analyze the conditioning effects of RPMS on reorganization of the motor system on cortical level positron

Postural control in otolith disorders.

It was the aim of the present paper to investigate the influence of otolith disorders on human postural control by different methods. The 33 patients of our study had undergone a minor head injury and suffered subsequently from an utricular or sacculo-utricular disorder as evidenced by vestibular evoked myogenic potential recordings and eccentric rotation recordings of the otolith-ocular responses. Postural control was assessed by performing stance/gait tests (standard balance deficit test, SBDT) and by evaluating trunk sway (using angular velocity sensors). Moreover, classical tests of the po

Runx3 and Runx1 are required for CD8 T cell development during thymopoiesis.

The RUNX transcription factors are important regulators of lineage-specific gene expression. RUNX are bifunctional, acting both as activators and repressors of tissue-specific target genes. Recently, we have demonstrated that Runx3 is a neurogenic transcription factor, which regulates development and survival of proprioceptive neurons in dorsal root ganglia. Here we report that Runx3 and Runx1 are highly expressed in thymic medulla and cortex, respectively, and function in development of CD8 T cells during thymopoiesis. Runx3-deficient (Runx3 KO) mice display abnormalities in CD4 expression du
