Endoscopic Papillectomy May Be a Feasible Treatment Option for Elderly Patients With Ampullary Tumors: A Multicenter Retrospective Study.

OBJECTIVES: Endoscopic papillectomy (EP) is a minimally invasive treatment for ampullary tumors; however, its safety in elderly patients remains unclear. We aimed to evaluate the feasibility and clinical outcomes of EP in this population. METHODS: This multicenter retrospective study included 255 patients with ampullary tumors who underwent EP between April 2008 and September 2025 at three collaborating institutions. Patients aged ≥ 75 years were defined as elderly; complication rates and clinical outcomes were compared with the non-elderly group. RESULTS: The elderly group had a significant

Sequential endoscopic and laparoscopic treatment for choledocholithiasis: a 30-year experience in the era of evolving techniques.

BACKGROUND: Sequential endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy (EST) followed by laparoscopic cholecystectomy (LC) is a standard minimally invasive approach for treating concomitant gallbladder and common bile duct stones. This study presents a 30-year experience with this strategy and compares its outcomes with emerging single-session techniques, including laparoscopic common bile duct exploration (LCBDE) and laparoendoscopic rendezvous (LERV). METHODS: Between October 1991 and December 2020, a total of 350 patients (mean age 65 years, range 35-80) underwent

ONE-STEP TACTICS OF SURGICAL TREATMENT OF ACUTE CHOLECYSTITIS IN COMBINATION WITH CHOLEDOCHOLITHIASIS.

INTRODUCTION: Concrements of the common bile duct (choledocholithiasis) occur from 8% to 20% in patients with acute cholecystitis. This pathology requires simultaneous correction of the pathology of the gallbladder and the common bile duct. METHODS: We retrospectively analyzed the case histories of 135 patients who underwent endoscopic retrograde cholangiopancreatography, endoscopic papillosphincterotomy, choledocholithoextraction followed by laparoscopic cholecystectomy+CHEST+LCE) for acute cholecystitis in combination with choledocholithiasis in the period from January 2016 to March 2021. Primary Needle-Knife Fistulotomy Versus Standard Transpapillary Technique for Cannulation of Long-Size Papilla: A Randomized Clinical Trial.

INTRODUCTION: The morphology of the major papilla plays a crucial role in the selection of the cannulation method for the common bile duct during endoscopic retrograde cholangiopancreatography. Nevertheless, there is limited evidence available that compares the efficacy and safety of cannulation approaches in certain papilla morphologies. The aim of this study was to assess the safety and effectiveness of 2 cannulation methods, including primary needle-knife fistulotomy (pNKF) and standard transpapillary (STP), in patients with long-size papilla. METHODS: A total of 260 patients with intact l

Evaluation of the outcomes of fistulotomy with primary sphincter reconstruction in the management of high trans-sphincteric fistula and supra-sphincteric fistula-in-ano : A single-center prospective cohort study.

BACKGROUND: Supra-sphincteric and high trans-sphincteric fistula are very challenging procedures for both the patient and the surgeon. We aimed to evaluate the outcomes of anal sphincter repair in the management of supra-sphincteric and high trans-sphincteric fistula-in-ano in terms of postoperative wound infection, bleeding, incontinence to flatus or stool, and recurrence within 1 year. PATIENTS AND METHODS: This single-center prospective cohort trial conducted from June 2020 to December 2023 at the Ain Shams University Hospitals included 20 patients who presented with supra-sphincteric or h

Peroral endoscopic myotomy versus pneumatic dilation - result from a retrospective study with 1-year follow-up.

BACKGROUND AND AIMS: On the basis of lesser rates of major adverse events and a short-term efficacy as Heller's myotomy, there is a growing enthusiasm in favor of peroral endoscopic myotomy (POEM), whereas study comparing POEM and pneumatic dilatation (PD) is quite rare. The aim of this study was to evaluate the efficacy of POEM and PD in Chinese achalasia patients in a retrospectively designed study. METHODS: Patients with achalasia, who underwent either PD (n = 26) or POEM (n = 40) were retrospectively recruited from September 2010 through March 2016 at a single tertiary center. During the

Expanding the indication of endoscopic papillectomy for T1a ampullary carcinoma.

BACKGROUND AND AIM: Endoscopic papillectomy (EP) has been attempted not only for benign lesions but also for early ampullary carcinoma (AC). However, there is still no sufficient evidence or consensus regarding the effectiveness of EP for early AC. Herein, we evaluated the expanding indication of EP for early AC. METHODS: Between May 1999 and December 2016, 177 patients were diagnosed with ampullary tumor before undergoing EP, and their clinical and histopathological data were analyzed retrospectively. RESULTS: There were 27 Tis-T1a AC patients and four T1b AC patients who underwent EP. Mean

Tips and tricks in endoscopic papillectomy of ampullary tumors: single-center experience with large case series (with videos).

Endoscopic papillectomy (EP) has been recognized as a safe and reliable treatment for ampullary adenomas. In this article, we describe important tips and tricks in EP of ampullary tumors on the basis of our large case series outcome. Between May 1999 and April 2014, 115 patients underwent EP at Tokyo Medical University hospital. The mean tumor diameter was 16.2 (6-35) mm. The final pathological diagnoses were adenoma, cancer in adenoma, cancer, and hyperplasia in 85, 13, 10, and seven patients, respectively. "En bloc" or "piecemeal" EP was performed with a polypectomy snare forceps using "Endo

Long-term outcome of endoscopic papillotomy for choledocholithiasis with cholecystolithiasis.

BACKGROUND: Although endoscopic papillotomy is now considered established treatment for choledocholithiasis, therapeutic results of endoscopic papillotomy alone without subsequent cholecystectomy in patients with cholecystolithiasis have not been well evaluated. The aim of this study was to assess the long-term outcome of endoscopic papillotomy for these patients. METHODS: Patients admitted with choledocholithiasis and cholecystolithiasis from 1976 to 1993 were studied retrospectively. Of 385 patients in whom the bile duct was cleared by endoscopic papillotomy and endoscopic stone extraction,

Pancreatic and ampullary carcinoma.

Surgery should be the first therapeutic modality considered in patients with pancreatic and ampullary carcinoma. Surgery is the only potentially curative therapy and offers the best form of palliation in patients with impending or overt duodenal obstruction. Patients with clearly unresectable tumors or those considered unfit for surgery should be offered palliative therapy, preferably endoscopically. The difficulty, however, arises in patients who undergo laparotomy and who subsequently are found to have unresectable tumors. The problem of stent occlusion and frequent associated hospital visit

[Spontaneous rupture of aberrant bile ducts in choledocholithiasis].

It is reported on 4 patients aged 69 to 79 years suffering from rupture of aberrant subserosal bile ducts. In the first patient the gallbladder did not obtain stones, the common duct stones causing increased pressure in the intrahepatic ducts and rupture were not diagnosed, the supposed "cyst" was excised, followed by suture of the liver tissue. Due to peritonitis relaparotomy had to be done. Since now intrahepatic obstruction was supposed a hepatojejunostomy was performed. The patient died from peritonitis following insufficiency of the anastomosis and bronchopneumonia. The other patients wer

[Combined non-operative intervention for retained bile duct stones].

Basket lithotriptor, piezoelectric shock wave lithotripsy, electrohydraulic shock wave lithotripsy, and endoscopic papillosphincterotomy were used to manage respectively 10, 15, 14, and 15 patients with retained intra and extrahepatic bile duct stones that could not be removed with a Dormia basket. In these four group patients the stones ranged from 5 to 25 mm in diameter. A residual stone was fragmented with a basket lithotriptor if its diameter exceeded that of the T tube tract. Shock-wave was used when mechanical lithotripsy was unsuccessful. Endoscopic papillosphincterotomy was carried out
