Fulminant Acanthamoeba Encephalitis Diagnosed By Cell-Free DNA in a Renal Transplant Patient: A Case Report.

Granulomatous amoebic encephalitis (GAE) caused by Acanthamoeba spp. is a rare, nearly universally fatal central nervous system infection that predominantly affects immunocompromised individuals, including solid organ transplant (SOT) recipients. Early diagnosis is essential but often delayed due to nonspecific symptoms and the limited sensitivity of conventional diagnostic tests. We describe the case of a 58-year-old man who presented with fever and diarrhea one month after a simultaneous pancreas-kidney transplant. Despite broad-spectrum antimicrobial therapy, he developed status epilepticus

Epidemiology and Clinical Features of Balamuthia mandrillaris Infection, China.

Balamuthia mandrillaris is a highly lethal free-living ameba that primarily affects the skin and central nervous system, manifesting clinically as chronic granulomatous lesions and granulomatous encephalitis. Once the central nervous system is involved, the mortality rate exceeds 90%. No standardized treatment regimen has yet been established. In this review, we summarized 66 cases reported from China. The median patient age was 36 years (range 10 months-77 years); 62.12% patients were male and 37.88% female. Fifty-five (83.33%) patients were immunocompetent. For 42 (63.64%) patients, initial

Imaging monitoring of Balamuthia granulomatous amoebic encephalitis.

Balamuthia granulomatous amoebic encephalitis (GAE) is a rare and highly lethal infectious disease affecting the central nervous system, caused by the Balamuthia amoeba. This article reports the case of a child with normal immune function and no apparent epidemiological risk factors. In the initial stages, routine blood tests, cerebrospinal fluid cell tests, biochemical tests, and metagenomic next-generation sequencing (mNGS) were all normal. Surgical removal of the abscess and subsequent pathological diagnosis revealed a chronic granuloma. mNGS analysis of the brain abscess fluid identified B

Minimal Cerebrospinal Fluid Concentration of Miltefosine despite Therapeutic Plasma Levels during the Treatment of Amebic Encephalitis.

Miltefosine is an alkylphosphocholine compound that is used primarily for treatment of leishmaniasis and demonstrates in vitro and in vivo antiamebic activity against Acanthamoeba species. Recommendations for treatment of amebic encephalitis generally include miltefosine therapy. Data indicate that treatment with an amebicidal concentration of at least 16 μg/ml of miltefosine is required for most Acanthamoeba species. Although there is a high level of mortality associated with amebic encephalitis, a paucity of data regarding miltefosine levels in plasma and cerebrospinal fluid in vivo exists i

Prevalence of pathogenic free-living amoeba and other protozoa in natural and communal piped tap water from Queen Elizabeth protected area, Uganda.

BACKGROUND: Pathogenic water dwelling protozoa such as Acanthamoeba spp., Hartmannella spp., Naegleria spp., Cryptosporidium spp. and Giardia spp. are often responsible for devastating illnesses especially in children and immunocompromised individuals, yet their presence and prevalence in certain environment in sub-Saharan Africa is still unknown to most researchers, public health officials and medical practitioners. The objective of this study was to establish the presence and prevalence of pathogenic free-living amoeba (FLA), Cryptosporidium and Giardia in Queen Elizabeth Protected Area (QEP

Acanthamoeba castellanii Genotype T4 Stimulates the Production of Interleukin-10 as Well as Proinflammatory Cytokines in THP-1 Cells, Human Peripheral Blood Mononuclear Cells, and Human Monocyte-Derived Macrophages.

Free-living amoebae of the genus Acanthamoeba can cause severe and chronic infections in humans, mainly localized in immune privileged sites, such as the brain and the eye. Monocytes/macrophages are thought to be involved in Acanthamoeba infections, but little is known about how these facultative parasites influence their functions. The aim of this work was to investigate the effects of Acanthamoeba on human monocytes/macrophages during the early phase of infection. Here, THP-1 cells, primary human monocytes isolated from peripheral blood, and human monocyte-derived macrophages were either coi

Branch portal vein pyaemia secondary to amoebic liver abscess.

We describe a case of a young returning traveller who contracted amoebic dysentery while visiting India. She presented to a major London Hospital several months later with features suggestive of amoebic liver abscesses, a known sequelae of amoebiasis. MRI with intravenous contrast demonstrated an area of likely occlusion of the portal vein. The patient was treated with intravenous metronidazole for 10 days followed by diloxanide furoate, an intraluminal agent. The largest abscess was drained acutely under ultrasound guidance. The portal vein occlusion was treated medically without the use of a

Autopsy case of amebic granulomatous meningoencephalitis caused by Balamuthia mandrillaris in Japan.

Balamuthia mandrillaris is a free-living ameba that causes amebic encephalitis. Herein, we report an autopsy case of Balamuthia encephalitis proven with polymerase chain reaction (PCR) and immunohistochemistry from paraffin-embedded brain biopsy specimens. A 68-year-old Japanese male presented at a hospital with progressive right hemiparesis approximately 3 months before his death. An open-brain biopsy specimen showed diffuse meningitis with massive coagulative necrosis. The perivascular spaces contained numerous lymphocytes, histiocytes and giant cells, although the etiology was not determine

[Acanthamoeba meningoencephalitis: a case in an adolescent female patient with systemic lupus erythematosus].

Meningoencephalitis caused by Acanthamoeba spp . is a rare opportunistic infection, difficult to diagnose and difficult to treat, which causes death in almost all cases. We report the neuropathologic findings of a 16-year-old girl with systemic lupus erythematosus (SLE) treated with immunosuppression who died of fulminant Acanthamoeba meningoencephalitis. Neuropathologic examination revealed multiple supratentorial and infratentorial hemorrhagic necrotizing lesions with encephalitis and vasculitis with mixed inflammatory infiltrates, fibrinoid necrosis of vessel walls, and local leptomeningiti

Primary amebic meningoencephalitis--Arizona, Florida, and Texas, 2007.

Primary amebic meningoencephalitis (PAM) is a rare but nearly always fatal disease caused by infection with Naegleria fowleri, a thermophilic, free-living ameba found in freshwater environments. Infection results from water containing N. fowleri entering the nose, followed by migration of the amebae to the brain via the olfactory nerve. In 2007, six cases of PAM in the United States were reported to CDC; all six patients died. This report summarizes the investigations of the cases, which occurred in three southern tier states (Arizona, Florida, and Texas) during June-September and presents pre

Coinfection with HIV and tropical infectious diseases. I. Protozoal pathogens.

The brunt of the human immunodeficiency virus (HIV) pandemic has been borne disproportionately by resource-poor regions of the world, where tropical infectious diseases continue to hold greatest sway. As a result, our understanding of the epidemiological, biological, and clinical interactions between HIV and tropical pathogens has lagged, compared with our understanding of the interactions between HIV and pathogens that are common in the industrialized world. Because of the current rapid expansion of HIV care in the tropics, with increasing resources being made available, an overview of the av

Rapidly expanding brain mass.

Amebic encephalitis is an uncommon central nervous system infection, usually caused by Acanthamoeba spp., which generally occurs in immunocompromised individuals. The infection might start from a minor, slowly progressive, skin ulceration that can be present for weeks to months before neurologic changes occur. We present a 40-year-old male with multiple visceral allografts, on immunosuppression, 9 months after transplantation. He had an active skin rash previously diagnosed as graft-versus-host disease by biopsy. The condition of the patient declined rapidly.

The role of blood vessels and lungs in the dissemination of Naegleria fowleri following intranasal inoculation in mice.

Primary amoebic meningoencephalitis (PAM) was induced in mice by intranasal inoculation of Naegleria fowleri (Singh et Das, 1970) to study the role of the blood vessels and lungs in the early and later stages in this disease. Upon culturing blood and lung tissue obtained at 24-, 36-, 48-, 72-, 96-, and 120-hour time periods, it was found that amoebae grew only from blood and lung tissue obtained at the 96 and 120 hour time periods. Paraffin sections of the head revealed small foci of acute inflammation and amoebae within the olfactory bulb of the central nervous system (CNS) at 24 hours. Amoeb

Is the intestinal tract a portal of entry for Acanthamoeba infection?

Acanthamoeba culbertsoni isolated from a water sample of El-Mahmoudia canal in Alexandria, was orally inoculated into a mouse model (200-400 amoebae/mouse) under different conditions. One week postinfection (P.I.), 20% of infected normoacidic mice and all animals received cimetidine or tetracycline prior to infection passed the parasite in their stools. One month P.I., 70% of cimetidine and 100% of tetracycline pretreated mice showed marked erosion in the intestinal mucosa and areas of necrosis with congestion in the brains, with trophozoites and cysts in both tissues. It is concluded that, no

Ileal amoebiasis.

Whilst colonic amoebiasis and its complications are well-documented, ileal amoebiasis has not been described in the literature. We report the cases of two patients with ileal amoebiasis and outline their management and outcome. It is important that surgeons working in areas endemic for amoebiasis be mindful of this entity.

Differentiation of Naegleria fowleri from Acanthamoeba species by using monoclonal antibodies and flow cytometry.

Monoclonal antibodies to Naegleria fowleri and Acanthamoeba polyphaga were analyzed by enzyme-linked immunosorbent assay, indirect immunofluorescence microscopy, and fluorescence flow cytometry to assess specificity and cross-reactivity with axenically cultured N. fowleri and Acanthamoeba spp. Four monoclonal antibodies to N. fowleri were specific for N. fowleri and had no reactivity to A. polyphaga. Similarly, four monoclonal antibodies to A. polyphaga did not react with N. fowleri. Two of the four monoclonal antibodies to A. polyphaga did not react with other Acanthamoeba spp. tested, while
