OBJECTIVE: To describe a patient with severe thyrotoxicosis attributable to Graves' disease who had a thrombotic cerebrovascular accident and hyperhomocysteinuria, which resolved on correction of the thyrotoxicosis, and to present findings in a pilot study undertaken to investigate the relationship among thyrotoxicosis, homocysteine, folate, and vitamin B(12).
METHODS: We present a case report of the index case, with clinical and laboratory details. For the investigative analysis, 21 patients who were 18 to 50 years old and had newly diagnosed, untreated Graves' disease and 10 age-and sex-mat

OBJECTIVE: To investigate an observed epidemic of gynecomastia among Haitian refugees in US detention centers in 1981 and 1982.
METHODS: All identifiable environmental exposures were investigated for estrogenic and antiandrogenic activity.
RESULTS: A high incidence of gynecomastia was observed among Haitian refugees in five detention centers in the United States. Of 284 men screened, 20 (from 18 to 53 years old) demonstrated new-onset gynecomastia (Tanner stages 2 to 5) in June 1982. The mean onset of gynecomastia was 130 +/- 12 days after arrival in the United States. Other symptoms include

OBJECTIVE: To review one physician's management of glucocorticoid replacement therapy in adult patients with congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency and to determine the efficacy and the range of effective doses of dexamethasone in managing the clinical and biochemical consequences of this disorder.
METHODS: We conducted a retrospective clinical practice audit of nine patients treated by a single physician for 21-hydroxylase deficiency during the period from March 1998 to January 2002. We documented biochemical control and clinical features of glucocorticoid exce

OBJECTIVE: To discuss the rare association between primary hyperparathyroidism and chronic urticaria.
METHODS: A case report with clinical and laboratory details is presented, and the pertinent literature is reviewed.
RESULTS: A 29-year-old patient with chronic urticaria for more than 2 years, which was partially responsive to medical treatment, was found to have primary hyperparathyroidism. After parathyroidectomy, the urticaria resolved, and it has not recurred during a 1-year follow-up period.
CONCLUSION: Primary hyperparathyroidism can be a rare cause of chronic urticaria.

OBJECTIVE: To compare vitamin D status represented by serum 25-hydroxyvitamin D (25-OHD) levels in a group of patients with primary hyperparathyroidism, stratified by clinical manifestations.
METHODS: We studied 22 patients (18 women and 4 men) with primary hyperparathyroidism--5 patients with severe osteitis fibrosa cystica, 10 symptomatic patients with active renal stone disease without overt bone disease, and 7 asymptomatic patients. Bone mineral density (BMD) measurements (T-scores) were done at the lumbar spine, femoral neck, and distal radius, and laboratory data for the three subgroups

METHODS: We present a case report, with serial laboratory and immunohistochemical data, and summarize information about similar patients described in the medical literature.
RESULTS: A 48-year-old woman had irregular menstrual periods, decreased peripheral vision, headaches, and weight gain. Laboratory and radiologic investigation revealed a large nonfunctioning pituitary adenoma. Transsphenoidal subtotal resection of th

OBJECTIVE: To describe the molecular characterization of a kindred affected by the rare variant of multiple endocrine neoplasia type IIA (MEN IIA) associated with cutaneous lichen amyloidosis and to discuss the clinical implications in the management of this syndrome.
METHODS: A kindred with four affected family members was identified, and DNA analysis was performed by sequencing exon 11 of the RET proto-oncogene. Presymptomatic genetic screening was offered to all first-degree relatives.
RESULTS: Sequencing analysis of the RET proto-oncogene revealed a Cys634Trp (TGC->TGG) mutation in all c

OBJECTIVE: To report an unusual case of thyrotoxicosis caused by "silent thyroiditis" in a lithium-treated patient and to summarize all prior case reports of lithium-associated thyroiditis and compare them with our current case.
METHODS: In addition to reporting our case, we undertook a MEDLINE search of all case reports of lithium-associated thyroiditis from 1978 until the present. All reported cases of lithium-associated thyroiditis must have had documented low thyroid radioiodine uptake to be included.
RESULTS: A 52-year-old man with a history of bipolar disorder, who had been treated wit

OBJECTIVE: To describe a case of primary bacterial pituitary abscess manifesting as hypopituitarism.
METHODS: We present the case history, hormonal and bacteriologic data, and findings on imaging studies in a 34-year-old man.
RESULTS: The patient had an 8-month history of intermittent fever, headache, nausea, vomiting, and weight loss. Because a computed tomographic scan of the head showed a cystic sellar mass with ring enhancement, he was referred to our medical center. On physical examination, he showed signs of meningeal irritation and had mild hypotension. Hormonal evaluation revealed ev

OBJECTIVE: To describe a rare case of thyrotoxic hypokalemic periodic paralysis in a half Native American and half African American person.
METHODS: We present a case of a man of Native American descent with thyrotoxic periodic paralysis and review the pertinent literature.
RESULTS: In a 53-year-old man with untreated hyperthyroidism, bilateral lower extremity weakness developed as a result of hypokalemia. Two months before admission, the patient had thyroid function test results that were consistent with hyperthyroidism. He also had undergone a thyroid radioiodine uptake study and scan, whi

METHODS: We present the clinical, laboratory, radiologic, and pathologic findings in a patient with ACC and review the related literature.
RESULTS: A 52-year-old woman with a history of hypertension and hypokalemia was referred for evaluation of a 6-cm adrenal mass. Her biochemical studies revealed a serum aldosterone-to-renin ratio of 52 without evidence of cortisol, catecholamine, or androgen excess. She underwent surgical resection of thi
