Extracardial migration of an atrial septal defect occluder device.

Several complications have been reported regarding the percutaneous closure of secondary atrial septal defects such as erosion, thromboembolic events, arrhythmias, and endocarditis. In this report, we describe the case of a 75-year-old woman who underwent percutaneous closure of a secondary atrial septal defect with a 12 mm Amplatzer septal occluder. Six months after the uneventful implantation of the device, we manifested an asymptomatic late embolization of the device in the abdominal aorta. The device was surgically retrieved.

Transcatheter retrieval of embolized AMPLATZER Septal Occluder.

In selected patients, transcatheter closure of atrial septal defects with the AMPLATZER Septal Occluder has yielded excellent results. However, there is a slight risk of device embolization after deployment. We report the case of a 26-year-old woman in whom an embolized AMPLATZER device was retrieved percutaneously from the right pulmonary artery. We also discuss important technical principles for managing this uncommon but potentially severe complication.

Implantation of a right ventricular implantable cardioverter-defibrillator lead in the right ventricular outflow tract in a patient with Ebstein anomaly and right ventricular lead dislocation.

An 80-year-old patient with Ebstein anomaly and prior implantable cardioverter-defibrillator (ICD) implantation was sent to our heart center because of right ventricular (RV)-ICD lead dislocation. Positioning of the new RV lead was difficult, no stable position could be found, and sensing and threshold measurements showed weak results in the RV basis, apex, and septum. So, we conducted positioning of the lead in the RV outflow tract where a stable position with good sensing and threshold parameters was found.

Subacute presentation of right ventricular perforation after pacemaker implantation.

We report an 87-year-old woman with right ventricular perforation due to a permanent pacemaker lead detected 4-days after implantation. The pacemaker lead was seen to perforate through the myocardium and pericardium and to reach the left pleural cavity. We removed the wire surgically by median sternotomy. The pericardial effusion was cloudy and yellowish, suggesting infection. However, no bacteria were detected by bacterial cultures of the pericardial effusion and pacing wire. The patient developed neither mediastinitis nor sepsis after the operation, and a new pacemaker was implanted safely o

Management of non-infectious central venous access device complications.

OBJECTIVES: To review the central venous access device (VAD) complications of inadvertent device damage, device malfunction, and various causes of mechanical occlusion and to describe the limitations of VAD complication data. DATA SOURCES: Journal articles, legal cases, case reports, manufacturers' product information, personal experience. CONCLUSION: The available data on VAD complications are difficult to interpret and apply in practice because of the many limitations of the studies that have been conducted (eg, single-institution, mixed patient populations, inconsistent measurement of com

Surgical removal of an embolized patent ductus arteriosus coil from pulmonary artery without cardiopulmonary bypass.

We report the case of a 4-month-old girl who underwent patent ductus arteriosus (PDA) coil embolization. The coil had migrated into the left pulmonary artery and attempts to retrieve it with interventional techniques were unsuccessful. To prevent late effects of a foreign object in pulmonary artery, the patient was referred to surgery. The operation was done without cardiopulmonary bypass (CPB) and with the aid of a Fogarty catheter. Recovery was uneventful after the procedure. As far as we know this is the first patient in whom CPB was not used for coil removal.

Caval migration of a ureteral J-stent after simultaneous ureter and iliac vein perforation during its placement for obstructive pyelonephritis.

A variety of indications have made the use of double pigtail ureteral catheters routine in urological practice. Although side effects are frequent they are usually mild. We report a case of intravenous stent migration after simultaneous perforation of the left ureter and left common iliac vein during retrograde J-stent placement. To the authors' knowledge such a potentially severe complication of a J-stent has not been previously described. A postprocedural plain abdominal radiograph is a helpful tool which may suggest possible misplacement of the pigtail stent especially if urine fails to ret

Posterior epidural mass: can a posteriorly migrated lumbar disc fragment mimic tumour, haematoma or abscess?

A 60-year-old woman complained of low back pain radiating to both buttocks and to the anterior aspect of the left thigh. MRI showed a left posterolateral epidural mass at the L1-L2 level. An epidural abscess was suspected, but the biochemistry was normal. Excision yielded complete relief of symptoms. Pathological examination demonstrated that the specimen was a migrated disc fragment. The authors found 29 other cases of disc migration to the posterior epidural space; two of these were at the thoracic level. Eleven of the 27 lumbar cases (40%) were complicated with Cauda Equina Syndrome (CES).

Percutaneous, minimally invasive retrieval of a dislodged and impacted device from the abdominal aorta, four months after closure of an atrial septal defect.

Four months after the successful implantation of a closure device in a 67-year-old woman with an atrial septal defect, the device was found to be impacted in the abdominal aorta. Severe bouts of only partially containable coughing episodes had occurred some days after implantation. Despite unknown endothelialization status and extent of adhesion to the aorta, percutaneous retrieval was favored over conventional surgical retrieval. In conclusion, this case confirms that bouts of elevated intrathoracic pressure can lead to device dislocation and that it may be possible to successfully retrieve a

A life-threatening complication of the Nuss procedure for pectus excavatum.

We describe a delayed, life-threatening complication in a boy operated on using the Nuss-procedure 2 months earlier. On admittance he was in shock with cardiac tamponade. An immediate needle aspiration of blood from the pericardium was done before a median sternotomy. The Nuss-bar was removed, and we identified a 1.5-cm laceration in the adventitial layer of the ascending aorta as the source of bleeding. The tear was closed, and the patient had an uneventful recovery. Careful positioning of the bar is necessary to avoid complications. Measures must be taken postoperatively to confirm that the

Gunshot wounds to the chest with arterial bullet embolization.

This report describes two cases involving small-caliber gunshot wounds to the chest with embolization of the bullet (.22 long rifle). In the first patient, hemodynamic status permitted additional imaging before surgery. Findings demonstrated embolization of the bullet into the right external carotid artery after penetration into the left ventricle. In the second patient, emergency thoracotomy was required to perform hemostasis following perforation of the pulmonary artery with massive hemothorax. Bullet embolization into the left hypogastric artery through an acquired aortopulmonary fistula su

Free orbital fat graft to prevent porous polyethylene orbital implant exposure in patients with retinoblastoma.

PURPOSE: To determine if porous polyethylene orbital implant (Medpor) exposure can be prevented in retinoblastoma patients when the implant is placed in combination with a free orbital fat graft over the anterior surface of the implant. METHODS: Free orbital fat grafts were performed after enucleation and Medpor implantation, and results were compared with patients who underwent conventional enucleation and Medpor implantation without an orbital fat graft. RESULTS: Although implant exposure occurred in 13 of 39 eyes (33.3%) that had conventional enucleation and Medpor implantation, exposure

Repositioning of a dislodged and fibrosed ventricular lead.

UNLABELLED: This is a case of a 61 year old African American female with history of end stage renal disease on hemodialysis. She had a dual chamber permanent pacemaker implanted in 2003 for symptomatic bradycardia. Over the past several months the patient had problems with access for dialysis. The right arm AV fistula was occluded. The patient had multiple temporary central catheter placed for dialysis over the past 3 months. The patient was admitted for superior vena-cava syndrome. CT scan of the thorax and neck showed occluded right and left subclavian system. An incidental finding was the r
