PURPOSE: Moderate-energy zygoma fractures result frequently in a posteromedially displaced bone fragment. Closed reduction using a force vector directed in an anterolateral direction frequently produces stable repair of these injuries. Exploration of the orbital floor (OF) is not routinely undertaken. However, as the zygoma forms a significant portion of the OF, realignment may create an unrecognized OF defect. Routine OF exploration may be unnecessary and carries the risks of eyelid malposition, scarring, and extraocular muscle injury. Our goal was to quantitatively describe the effect of zyg

OBJECTIVE: To investigate the operative indication and clinical efficacy of combination of external fixator and volar approach fixation in treating distal radius fracture.
METHODS: From March 2000 to March 2005, 28 patients with distal radius fracture were treated with combination of external fixator and volar approach fixation. Dorsal external fixator was used to maintain wrist in functional position, combinated volar plate or Kirschner wire fixation after reduction was achieved. Bone graft was necessary if there were severe comminuted cortical bone or compress of cancellous bone. Of 28 pati

OBJECTIVES: To identify clinical and radiographic findings of pediatric scaphoid fractures that predict the need for surgical treatment.
METHODS: A retrospective review of pediatric patients (≤ 18 years) with scaphoid fractures, who underwent radiographic examination and treatment at our tertiary care pediatric hospital, between 2018 and 2024, identified all surgically treated patients. From the remaining conservatively treated patients, age matched comparisons were randomly selected. After randomization and blinded to outcome, skeletal age, fracture characteristics (location, displacement, c

INTRODUCTION: The purpose of this study was to identify risk factors present at the time of injury that predict poor functional outcomes and heterotopic ossification (HO) in open periarticular elbow fractures.
MATERIALS AND METHODS: We performed a retrospective review of 136 combat-related open elbow fractures from 2003 to 2010. Patient demographics, injury characteristics, treatment variables, and complications were recorded. Functional outcomes were analyzed to determine range of motion (ROM) and Mayo Elbow Performance Score (MEPS). Secondary outcome measures included the development of HO,

OBJECTIVE: The talus is an important component in the ankle, and its treatment after injury is crucial. However, complications and adverse events due to incomplete traditional classifications may still occur, and these classifications fail to analyze the patterns and distribution of fractures from a three-dimensional perspective. Therefore, in this study, we aimed to analyze the location and distribution of fracture lines in different types of talus fractures using three-dimensional (3D) and heat mapping techniques. Additionally, we aimed to determine the surface area of the talus that can be

This video article presents a 47-year-old male patient who presented to the emergency department after a cleaver bounced off the grinding wheel and lodged between his eyebrows. The patient complained of clear nasal drainage since the trauma. Computed tomography showed a comminuted displaced fracture starting from the right frontonasal recess and extending along the right ethmoid roof. The patient underwent surgery. The skull base defect was reconstructed with a free fascia lata graft in the first operation. In the second session, the reconstruction of the skull base was reinforced with a flap

One of the indications of the surgical approach of the temporomandibular joint is the presence of foreign body in its interior. However, joint handling offers risks, especially bleeding. In these cases, angiography and embolization of the arteries involved with the projectile play an important role in the prevention of complications. In this report, the authors describe a case of a 23-year-old male victim of firearm attack with comminuted fracture of the right mandibular condyle. The patient underwent angiography and prophylactic embolization of the arteries adjacent to the foreign body. A sur

We describe our experience with reconstruction of the mandibular condyle with a costrochondral graft (CCG). We retrospectively evaluated 122 patients with diagnoses including osteoarthrosis, ankylosis, tumours, idiopathic condylar resorption, comminuted condylar fracture, and chronic osteomyelitis of the temporomandibular joint (TMJ). We used a modified preauricular approach and an endoscopically assisted technique. The grafts were followed-up by computed tomograms (CTs) and magnetic resonance (MR) examinations. All patients had successful reconstruction of the mandibular condyle with a CCG. N

Massive trauma involving the foot was treated by external fixation in 33 cases and followed for 1 to 3 years. Amputations were performed in seven (21%) because of uncontrollable deep sepsis in two and massive injury in five. The other 26 (79%) had functionally acceptable feet at follow-up, but all had some stiffness and cosmetic deformity. Complications attributed to the Hoffman external fixation were limited to three pin tract infections. We recommend Hoffmann external fixation when it is necessary to: 1) stabilize major open fracture dislocations; 2) maintain length where bone is lost or ext

It is often the case that screws cannot be firmly anchored in small, marginal joint fragments. This is especially relevant in osteoporotic bone. By redesigning the last plate hole of a one-third tubular plate to function as a hook, these bone fragments can be securely grasped and stabilized. Placing two plates on top of one another further improves mechanical strength.

INTRODUCTION: The retrospective study was made to evaluate the fracture patterns at the proximal humeral shaft for which the long version of a standard proximal humeral nail (PHNLV) has been used. The indication has been decided by the individual surgeons.
PATIENTS AND METHODS: Over a five year period 72 consecutive PHNLV cases of an acute fracture were identified and were included in the study. Mean patient age was 68.9 years. Gender ratio was m/f=22/50. 86.1% of the patients fractured their humerus by a fall, the rest by a high velocity accident. We analysed patient comorbidity, ASA score,

Rapid prototyping (RP) comprises a variety of automated manufacturing techniques such as selective laser sintering (SLS), stereolithography, and three-dimensional printing (3DP), which use virtual 3D data sets to fabricate solid forms in a layer-by-layer technique. Despite a growing demand for (virtual) reconstruction models in daily forensic casework, maceration of the skull is frequently assigned to ensure haptic evidence presentation in the courtroom. Owing to the progress in the field of forensic radiology, 3D data sets of relevant cases are usually available to the forensic expert. Here,

The decision to salvage or amputate a severely injured limb is one of the most difficult an orthopaedic surgeon may face. The inclination to undertake heroic measures to save the limb should be tempered by the realization that doing so may lead to repeated hospitalizations, extensive complications, and a poor functional outcome. Several factors must be considered, including objective elements related to the patient's injury and physical condition and subjective considerations related to the patient's psychologic, social, and economic status. We present a framework, in the context of a case stu

Antegrade intramedullary nailing has become the gold standard to treat two-, three-, and four-part cephalo-tuberosity fractures. We report a retrospective study of 6 patients who have undergone centro-medullary nailing since January 2012. Mean follow-up was 12 months; the average age was 57 years. Clinical evaluation was based on gross and weighted Constant and Murley score, based on the age and sex, compared to the "normal" body surface area. Radiological assessment allowed us to evaluate bone healing, the occurrence of osteonecrosis of femoral head or of post-traumatic arthrosis. Radiologica

From 1991 to 1998, 23 humeral fractures has been treated using the Orthofix external fixator. Average age of the patient was 42 y and average follow-up 55.5 m. Initial trauma was: 13 traffic accidents, four falls at home, two devastating farming accidents, two sports accidents, one aggression and one gun accident. AO classification was used and location of the fracture was classified using Hackethal classification modified by de la Caffinière. Majority of fractures were located at the one-third distal humerus and the majority was also comminuted. At follow-up, elbow range of motion was 130 deg

Comminuted fractures of the long bones present problems of mechanical instability. We therefore used a plate and six screws as the main osteosynthesis combined with supplementary lag screws or cerclage wire. These two supplementary methods were compared. As a model, we used 60 plastic tibias with standard midshaft butterfly fractures. These models, depending on the osteosynthesis applied for the fixation of the butterfly onto the main fracture, were split into three groups of 20 bones. Group A1 with two lag screws, group A2 with two lag screws and two double cerclages, and group A3 with two do

The unique case of a rare 3-level extensor mechanism failure in a 28-year-old male, involving a tibial tubercle avulsion fracture, a patellar tendon avulsion off the tibial tubercle fragment, and a severely comminuted patella fracture, and the surgical technique required to repair such an injury is presented. Focus is spent on the unique repair of a tendon injury when both proximal and distal bony attachments are damaged. Trifocal knee extensor mechanism is a rare clinical entity with minimal literature available-to date, this injury has only been reported in a retrospective review of combat-r

Traction tables have been widely used by orthopaedic surgeons for the management of femoral shaft fractures. Although complications associated with their use are uncommon, they can still occur. Among neurological complications, sciatic and pudendal nerve palsies are the most frequently reported. Recognizing the potential causes and analyzing these complications are crucial for raising awareness among surgeons and implementing preventive strategies to enhance patient safety during surgical procedures. A 22-year-old male sustained a comminuted fracture of the left femoral shaft following a road
