A 65-year-old woman with liver metastases from colon cancer and tumor thrombus extending from the right hepatic vein (HV) to the right atrium (RA) presented with marked lower-extremity edema and massive ascites. Computed tomography showed tumor thrombus completely occluding the inferior vena cava (IVC) and HV ostia. Recanalization of the IVC and HVs was performed. Metallic stents were placed in tandem from the superior vena cava to the IVC through the RA, and additional metallic stents were placed in the left HV. The patient's symptoms were relieved, and there was no recurrence of these sympto

We report a case of a complicated vascular access secondary to systemic venous defects, which have not been previously reported. Evaluation revealed an atypical coursing of a subclavian vein-persistent superior vena caval system into the hemizygos-azygos system and ultimately into the right (proper) superior vena cava. The clinical and surgical considerations of such an anomaly are discussed therein. We look at embryological origins of such a merger and share our experience in troubleshooting through the anomaly.

BACKGROUND: Autologous arteriovenous fistulas are frequently threatened by central venous obstruction. Although this is frequently ascribed to indwelling catheters and neointimal venous remodeling, we believe that extrinsic compression of the subclavian vein as it passes through the costoclavicular junction (CCJ) may play a significant role in a subset of dialysis patients.
METHODS: We reviewed our experience with CCJ decompression for arteriovenous fistula dysfunction at our institution. Decompression followed principles for venous thoracic outlet syndrome: bony decompression with thorough v

OBJECTIVE: Published case reports suggest that congenital portosystemic venous connections (PSVC) and other abdominal venous anomalies may be relatively frequent and potentially important in patients with polysplenia syndrome. Our objective was to investigate the frequency and range of portal and other abdominal systemic venous anomalies in patients with polysplenia and inferior vena cava (IVC) interruption who underwent a cavopulmonary anastomosis procedure at our center, and to review the published literature on this topic and the potential clinical importance of such anomalies.
DESIGN: Ret

OBJECTIVE: The purpose of this article is to illustrate the radiologic findings of various uncommon conditions that cause hepatic congestion in patients after living donor liver transplantation.
CONCLUSION: Radiologic findings of hepatic congestion may vary according to the cause and the level of outflow obstruction. It is important for radiologists to be aware of such variations in order to make an early diagnosis of hepatic congestion and to enable timely proper management in recipients after living donor liver transplantation.

We encountered a case of portal vein thrombosis (PVT) after treatment for portal hypertension due to pancreatic arteriovenous malformation (PAVM). A 75-year-old man was admitted for the treatment of esophageal varices. Diffuse PAVM and aneurysm in the celiac and superior mesenteric arteries were detected via abdominal computed tomography and angiography. Although endoscopical sclerotherapy was performed, PVT was identified after the treatment and variceal bleeding continued. Autopsy was performed and the thrombus and malformation were pathologically confirmed. This case indicates that PVT can

We describe imaging findings and interventional therapy of a 37-year old female patient suffering from chronic pelvic pain with associated vulvar varicosis. MR-angiography showed multiple varices in the left hemi-pelvis, a dilated draining vein from the left ovary as well as a left-sided vulvar varicosis, which was clinically evident. In addition, the MRI suggested insufficiency of the great saphenous vein with varicosis of its tributaries. After crossectomy and infragenual stripping of the great saphenous vein, we proceeded with a left phlebectomy and ligation of the supplying veins. Due to p

Although it is well known that outflow block is caused by stenosis or occlusion of hepatic vein anastomoses following living donor liver transplantation (LDLT), there have been few reports on inferior vena cava (IVC) stenosis following LDLT. In this paper, we report two cases of IVC stenosis and hepatic vein outflow block following right hepatic LDLT in the absence of stenosis of any of the anastomoses. Both patients presented with liver dysfunction, an ascitic fluid volume of approximately 2000 mL, and congestion in their biopsy specimens, and venocavography demonstrated IVC stenosis with gra

Cardiac resynchronisation therapy improves symptoms and reduces mortality in patients with chronic heart failure. In patients with previously implanted devices, particularly automatic defibrillators, central venous stenoses provide a challenge to upgrading to resynchronisation devices. We present a patient with central venous obstruction secondary to previously implanted defibrillator leads, in whom we achieved coronary sinus pacing through the ipsilateral internal jugular vein.

BACKGROUND AND PURPOSE: The pontomesencephalic veins (PMVs), especially the anterior PMV, are sometimes large enough that they could potentially affect the interpretation of CT angiograms of the circle of Willis. We investigated the frequency and anatomy of visible PMVs on 3D CT angiograms.
METHODS: CT angiograms of 211 consecutive patients who underwent CT angiography for a variety of clinical indications were evaluated retrospectively. Images evaluated by consensus between two neuroradiologists were maximum intensity projection and volume-rendered 3D CT angaiograms.
RESULTS: Visible PMVs w

PURPOSE: To describe the technical aspects of percutaneous balloon dilation and stenting for the treatment of venous outflow obstruction in chronic venous insufficiency.
METHODS: Between March 1997 and December 1998, 94 consecutive patients (median age 48 years, range 14 to 80) with suspected iliac vein obstruction in 102 limbs were studied prospectively with the intent to treat any venous occlusion or stenosis verified during femoral vein cannulation. Data from the history, clinical examination, procedure, and follow-up were recorded. Preoperative indicators of obstruction were venographic e

PURPOSE: To compare the nitinol occlusion plug with standard stainless steel coils for the occlusion of moderate-size peripheral veins.
MATERIALS AND METHODS: The nitinol plug is a braided multilayered vascular occlusion device filled with thrombogenic polyester fibers. It is self-expanding and can be recaptured into its 6-F introducing sheath for repositioning prior to detachment. Ten occlusion procedures were performed in five dogs from a retrograde transjugular venous approach. Five nitinol plugs (diameter: 7.4 mm +/- 0.5) were deployed in five femoropopliteal veins (diameter: 6.5 mm +/- 0

Computed tomography has revolutionized the diagnosis of ophthalmologic and neuro-ophthalmologic disease. High resolution CT with thin-section techniques has made available the depiction of finer anatomic orbital detail and permits radiologic detection of more subtle lesions than was previously available. The display of normal-sized superior ophthalmic veins by CT is further evidence of the resolving power of the currently available machines. A segment of the superior ophthalmic vein was identified in 18 of 64 orbits examined by CT. Virtually the entire superior ophthalmic vein was identified i
