[Long-term visual prognosis after successful vitrectomy for proliferative diabetic retinopathy].

We evaluated visual prognosis in 50 eyes of 47 patients which underwent successful vitrectomy for proliferative diabetic retinopathy and were followed for more than five years. It was found that the latest visual acuity was worse than the best visual acuity by more than three lines after vitrectomy in 14 eyes (28%) except for the eyes with decreased vision due to cataract. The causes of decrease in visual acuity were optic atrophy in seven eyes, macular degeneration in five eyes, branch retinal vein occlusion in one eye, and submacular hemorrhage in one eye. Risk factors for postoperative decr…

Triptolide alleviates pathological neovascularisation in oxygen-induced retinopathy via inhibiting the inflammation mediated by NF-κB pathway.

BACKGROUND: Retinopathy of prematurity (ROP), an oxygen-induced retinopathy (OIR), triggers a series of vascular lesions and inflammatory responses and results in visual impairment or even blindness. Triptolide (TP) possesses many pharmacological properties, including immunosuppressive and anti-tumour effects. However, the effects of TP on ROP and its underlying mechanisms remain unclear. PURPOSE: To investigate whether TP could inhibit the progression of OIR and to elucidate its underlying mechanisms. METHODS: The 7-day-old mice (P7) were kept in a 75% hyperoxia incubator for 5 days to indu

Aqueous humour extracellular vesicle membrane protein profiling reveals pathological features of refractory macular edema.

BACKGROUND: Both diabetic macular edema (DME) and retinal vein occlusion-related macular edema (RVO-ME) can become refractory to anti-vascular endothelial growth factor (anti-VEGF) therapy, but the underlying mechanisms are unclear. Molecular discrimination of refractory disease could guide personalized treatment. This study examined whether aqueous humor-derived extracellular vesicle (EV) membrane proteins can characterize refractoriness and reveal etiology-specific pathways. METHODS: This prospective cohort study included 28 patients with DME or RVO-ME (14 each), further divided into treatm…

N6-methyladenosine modification of mRNAs in retinal ischemia-reperfusion in mice.

Retinal ischemia-reperfusion injury (RIR) is the main pathogenic mechanisms of acute glaucoma, diabetic retinopathy, central retinal vein occlusion. As a common post-transcriptional modification of eukaryotic RNAs, N6-methyladenosine (m6A) is associated with the pathogenesis of different diseases, including angiogenesis, through the regulation of RNA metabolism and functions. The aim of this study was to identify the potential relevance of m6A RNA methylation in pathogenesis of RIR. A total of 10,851 mRNAs and 23,270 associated m6A methylation modified peaks were identified in the RIR group. S…

Real-world cost evaluation of conbercept vs. ranibizumab for retinal and choroidal vascular diseases from 2021 to 2024: evidence from societal and medical insurance perspectives.

BACKGROUND: Retinal and choroidal vascular diseases including age-related macular degeneration (AMD), branch and central retinal vein occlusion (BRVO, CRVO), diabetic macular edema (DME), and choroidal neovascularization secondary to pathologic myopia (PM) are important causes of blindness. Their treatments by anti-VEGF agents imply heavy economic impact. METHODS: The Markov model was constructed based on best-corrected visual acuity, real-world injection frequencies and medical costs of 15,266 patients in Tianjin (2021–2024), utility values, and transition probabilities derived from clinical …

Deep learning-based classification of multiple fundus diseases using ultra-widefield images.

PURPOSE: This study aimed to develop a hybrid deep learning model for classifying multiple fundus diseases using ultra-widefield (UWF) image … s. RESULTS: The model demonstrated exceptional diagnostic performance. For common diseases such as retinal vein occlusion, age-related macular degeneration, and diabetic retinopathy, AUC values exceeded 0.975, with accuracy rates above 0.980. For rare diseases, AUC values were above 0.970, and accuracy rates surpassed 0.998. Grad-CAM visualizations confirmed …

Real-world effectiveness of intravitreal dexamethasone implants - Comparison between eyes eligible and ineligible for clinical trials and their associated outcomes.

BACKGROUND: Concerns about the generalizability of pivotal randomized controlled trials (pRCTs) findings have been raised. We aimed to compare intravitreal dexamethasone implants' (IDIs) effectiveness for diabetic macular edema (DME) and central retinal vein occlusion (CRVO), between eyes eligible and ineligible for pRCTs. METHODS: This retrospective cohort study analyzed Taiwan's Chang Gung Research Database, including DME or CRVO eyes initiating IDIs during 2015-2020. We classified all treated eyes as eligible or ineligible for pRCTs following major selection criteria of the MEAD and GENEVA…

Peripheral Chorioretinal Imaging Through a Front Prism on Optical Coherence Tomography Angiography.

PURPOSE: To evaluate the clinical feasibility of peripheral chorioretinal imaging through a front prism on swept-source optical coherence tomography angiography (SS-OCTA). METHODS: We prospectively obtained en face OCTA images using SS-OCTA in 10 eyes of 10 healthy volunteers. For the peripheral chorioretinal imaging, the scanning laser passed and refracted through a 45°-90°-45° right-angle prism. We evaluated the qualitative and quantitative characteristics of chorioretinal vessels in the periphery. RESULTS: Using peripheral chorioretinal imaging through a prism, the retinal vasculature was

Experimental and clinical evidence of neuroprotection by nerve growth factor eye drops: Implications for glaucoma.

Elevated intraocular pressure (IOP) in glaucoma causes loss of retinal ganglion cells (RGCs) and damage to the optic nerve. Although IOP is controlled pharmacologically, no treatment is available to restore retinal and optic nerve function. We evaluated the effects of NGF eye drops in a rat model of glaucoma. We also treated 3 patients with progressive visual field defects despite IOP control. Glaucoma was induced in rats through injection of hypertonic saline into the episcleral vein. Initially, 2 doses of NGF (100 and 200 mug/mL) were tested on 24 rats, and the higher dose was found to be mo

Three-Dimensional High-Resolution Temporal Bone Histopathology Identifies Areas of Vascular Vulnerability in the Inner Ear.

OBJECTIVES: Hypothesized causes of vestibular neuritis/labyrinthitis include neuroinflammatory or vascular disorders, yet vascular disorders of the inner ear are poorly understood. Guided by known microvascular diseases of the retina, we developed 2 hypotheses: (1) there exist vascular vulnerabilities of artery channels in cases of hypothetical nerve swelling for the superior, inferior, and vestibulocochlear artery and (2) there are arteriovenous crossings that could compromise vascular flow in disease states. METHODS: Two fully mounted and stained temporal bones were used to render three-dim
