Hypothermia-rewarming: A Double-edged sword?

Hypothermia is a condition in which the body's core temperature drops below 35.0 °C. Hypothermia is the opposite of hyperthermia, which the metabolism and body functions are abnormal. Severe hypothermia is a life-threatening problem that may cause atrial and ventricular dysrhythmias, coagulopathy, cardiac, and central nervous system depression. What is worse, it is fatal when untreated or treated improperly. Accidental deaths due to hypothermia resulting from immersion in cold water, especially involving naval fighters and maritime victims have occurred continually in the past years. Currently

The mortality associated with pelvic fractures.

The mortality among 604 patients with pelvic fractures was 12%. Pedestrian accidents were the etiologic agent in 27% of the patients, but accounted for 49% of the deaths and for 73% of the deaths primarily due to pelvic fractures. Although 71 of the 72 patients who died sustained concomitant major injuries (mean, 3.1), 60% of the deaths (43 patients) were attributed entirely or in part to pelvic fractures. Of particular interest were the 26 patients in whom the pelvic fracture was the primary cause of death. Ninety-three percent were in shock or had clinical evidence of hypovolemia at the time

Heat injuries to the respiratory system.

A steam-tube of the main boiler exploded on a ship lying in the harbour of Hamburg. The steam temperature was 283 degrees C. Cutaneous and severe inhalational scalding occured in the 27 fatalities, the men dying after different intervals. This paper deals with the pathological findings in the respiratory passages and the lung, describing the topographical extent of direct thermal injury and the temporal course of tissue reactions. In cases of instantaneous death coagulation necrosis of the tracheal and bronchial wall was found to extend to alveolar ducts in central parts of the lung. The lung

[Observations on the course of leucocytes in cardiac surgery under acupuncture-analgesia (author's transl)].

In 11 cardiosurgical patients leucocyte and differential counts were made performed during and after surgery. In some cases Lymphocyte transformation and migration inhibitiontest was studied. Comparing cases under conventional neurolept anaesthesia with those who had acupuncture Analgesia (electrostimulation) it appeared that the number of leucocytes was rising significantly higher in the latter, although the total number of lymphocytes remained constant. In lymphocyte transformation and mirgration inhibitiontest the observed decrease seemed to be less pronounced. Because a trial applying acup

Validation of the Termination of Resuscitation Rules in Detroit.

Background and objective The termination of resuscitation (TOR) criteria - which recommends termination when a non-traumatic arrest in an adult is unwitnessed by emergency medical services (EMS), no shocks are administered, and no return of spontaneous circulation (ROSC) occurs - guide physicians in determining the viability of continuing cardiopulmonary resuscitation (CPR) and transporting patients to the hospital. We examined the level of compliance with the current basic life support (BLS) TOR rule and assessed alternative sets of rules to retrospectively derive improved TOR guidelines for

Causes of mortality in military working dog from traumatic injuries.

INTRODUCTION: This study aimed to identify the pathophysiologic causes of death following traumatic injuries in military working dogs (MWDs) and determine the risk factors associated with mortality in MWD following traumatic injuries. The results of this study will allow for better targeting of interventions to ameliorate these pathophysiologic causes of death and inform research priorities directed at the pathophysiology that leads to the death of MWDs. METHODS: The final dataset for this study was compiled by using two previously established datasets. Based on review of available data and s

Time-limited association between plasma transfusion and mortality in pediatric traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) is a predominant cause of pediatric mortality. While prehospital plasma administration has been associated with lower mortality in adults with TBI, the impact of early plasma in children is unknown. METHODS: In this retrospective cohort study, we examined the impact of plasma transfusion within 4 hours of arrival on 4-hour, 24-hour, and 30-day mortality in children aged 1 to 17 years with severe TBI (head Abbreviated Injury Scale scores 4 and 5) using the National Trauma Data Bank from 2020 to 2022. We excluded subjects with mild-moderate or nonsurviva

A survey on perioperative red blood cell transfusion trigger strategies for pediatric patients in the Nordic countries.

BACKGROUND: Transfusion of red blood cells (RBC) to rapidly increase hemoglobin levels have been associated with increased risks and worse outcomes in critically ill children. The international TAXI consensus from 2018 (pediatric critical care transfusion and anemia expertise initiative) recommended restrictive RBC transfusion strategies in pediatric patients. OBJECTIVE: To elucidate physicians perioperative RBC transfusion trigger strategies for pediatric patients in the Nordic countries and to investigate what factors influence the decision to transfuse this group of patients. METHODS: An

Closed-loop automated critical care as proof-of-concept study for resuscitation in a swine model of ischemia-reperfusion injury.

BACKGROUND: Volume expansion and vasopressors for the treatment of shock is an intensive process that requires frequent assessments and adjustments. Strict blood pressure goals in multiple physiologic states of shock (traumatic brain injury, sepsis, and hemorrhagic) have been associated with improved outcomes. The availability of continuous physiologic data is amenable to closed-loop automated critical care to improve goal-directed resuscitation. METHODS: Five adult swine were anesthetized and subjected to a controlled 30% estimated total blood volume hemorrhage followed by 30 min of complete

Multicentre observational study on practice of prehospital management of hypotensive trauma patients: the SPITFIRE study protocol.

INTRODUCTION: Major haemorrhage after injury is the leading cause of preventable death for trauma patients. Recent advancements in trauma care suggest damage control resuscitation (DCR) should start in the prehospital phase following major trauma. In Italy, Helicopter Emergency Medical Services (HEMS) assist the most complex injuries and deliver the most advanced interventions including DCR. The effect size of DCR delivered prehospitally on survival remains however unclear. METHODS AND ANALYSIS: This is an investigator-initiated, large, national, prospective, observational cohort study aiming

The old man and the C-spine fracture: Impact of halo vest stabilization in patients with blunt cervical spine fractures.

BACKGROUND: Placement of a halo vest for cervical spine fractures is presumed to be less morbid than operative fixation. However, restrictions imposed by the halo vest can be detrimental, especially in older patients. The purpose of this study was to evaluate the impact of halo vest placement on outcomes by age in patients with cervical spine fractures without spinal cord injury. METHODS: All patients with blunt cervical spine fractures managed over an 18-year period were identified. Those with spinal cord injury and severe traumatic brain injury were excluded. Patients were stratified by age

Platelet activation and dysfunction in a large-animal model of traumatic brain injury and hemorrhage.

BACKGROUND: Traumatic brain injury (TBI) and hemorrhage are the leading causes of trauma-related mortality. Both TBI and hemorrhage are associated with coagulation disturbances, including platelet dysfunction. We hypothesized that platelet dysfunction could be detected early after injury, and that this dysfunction would be associated with early activation, as measured by circulating levels of platelet activation markers. METHODS: A total of 33 swine were allocated to TBI and hypotension (n = 27, TBI and volume-controlled 40% blood loss) or controls (n = 6, anesthesia and instrumentation only)

Maintaining survivors' values of left ventricular power output during shock resuscitation: a prospective pilot study.

OBJECTIVE: Maintaining left ventricular power output (LVP) > 320 mm Hg x L/min/m2 during resuscitation has been retrospectively associated with faster resolution of acidosis and survival after posttraumatic shock. The purpose of this prospective study was to evaluate the effects of maintaining LVP above this threshold during resuscitation on base deficit clearance, organ failure, and survival. METHODS: This was a study of a consecutive series of critically injured patients (PWR) monitored with a pulmonary artery catheter during initial resuscitation. LVP, calculated as cardiac index-(mean art

Endogenous ethanol production in trauma victims associated with medical treatment.

Four cases of trauma, where endogenous ethanol production was suspected to have been occurred in association with medical treatment, are reported. To discriminate endogenous ethanol produced de novo by bacteria from exogenous ethanol by drinking, various tissues and body fluids, such as brain and cerebrospinal fluid, together with blood obtained from various locations, were subjected to analysis for both ethanol and n-propanol. The first individual was a 40-year-old man who had been stabbed in the abdomen with a knife and had died of bleeding about 12 h after peritoneotomy, and autopsied 12 h

Exploring Long-Term Outcomes in Sepsis Survivors: A Prospective Cohort Study Across Acute Hospitals.

INTRODUCTION: While sepsis survival rates have improved, long-term consequences remain under-recognised and support services limited. Survivors often face persistent physical, cognitive and psychological impairments that diminish their quality of life. AIM: To investigate the long-term health outcomes and sequelae experienced by survivors of critical care-treated sepsis and septic shock. DESIGN/METHODS: Adapting the Mid-German Sepsis Cohort protocol to the Irish context, this prospective longitudinal study spanned five acute hospitals. Participants were adults surviving critical care-treated

Bridging the gap: Acute amantadine augments the neurorehabilitative efficacy of delayed and abbreviated environmental enrichment in a pediatric traumatic brain injury model.

Environmental enrichment (EE) promotes neurobehavioral recovery after traumatic brain injury (TBI). However, most preclinical studies initiate EE immediately after injury, which contrasts with delayed rehabilitation in the clinic. To better model clinical practice, we delayed the onset of EE and administered amantadine (AMT), which exhibits indirect dopaminergic effects, acutely as a therapeutic bridge. We hypothesized that this temporally sequenced combination therapy would improve neurobehavioral outcomes more than either treatment alone. Post-natal day 21 male rats received a controlled cor

Suvorexant improves mitochondrial dynamics with the regulation of orexinergic and mTOR activation in rats exhibiting PTSD-like symptoms.

BACKGROUND: Increasing evidence suggests that mitochondrial dysfunction plays a significant role in PTSD. However, the exact mechanism is still unclear. Mitochondrial dynamics could be one of the mechanisms, as it is crucial for mitochondrial homeostasis and is widely affected in traumatic situations. Mitochondrial dynamics regulate mitochondrial homeostasis via orexinergic receptors, and it is shown that antagonism of orexinergic receptors attenuates PTSD-like symptoms. Therefore, the present study aimed to determine how orexin antagonists affect mitochondrial dynamics in rats exhibiting PTSD
