[Long-term outcome after Ilizarov corrective fixation for severe foot deformity].

PURPOSE OF THE STUDY: Much work has focused on the short-term outcome after external fixation with the Ilizarov apparatus for treatment of severe recurrent or neglected foot deformity. No series has provided sufficient follow-up to allow an analysis of long-term stabilization. MATERIAL AND METHODS: We reviewed a series of 17 patients (21 feet) treated with the Ilizarov apparatus between 1988 and 1998 and reexamined after a mean 6-year follow-up. We assessed the risk of degradation of the initial result and quantified the need for secondary arthrodesis surgery. This series included 18 congenit

Was the pedestrian hit in an erect position before being run over?

If a pedestrian was run over by a car, the question can arise whether there was a preceding collision while the pedestrian was in an erect position. From a total of 53 selected autopsy reports, the findings associated with accidents known to involve running over in isolation (n=32) were compared to findings associated with a combined mechanism of a primary impact in an erect position and subsequent running over (n=21). Findings exclusively present in the combined group were wedge-shaped bone fractures ("Messerer"-wedges, 38%), glass fragment injuries (24%), traumatic amputations (10%), traces

[Conservative management of hemophilic arthropathy].

Intraarticular bleeding and muscle bleeding occur spontaneously or as a result of trauma in hemophilia A or B. The most common sites for hemarthrosis are the knees joints, elbows and ankles joints. Resorption of intraarticular blood induces reactive hemophilic synovitis Hyperplasia of the synovium can be followed by recurrent bleedings. These early reactions can change to a chronic synovitis and cause cartilage damage, finally resulting in a complete destruction of the joint. Since 1981 158 adults and 61 children with a hemophilia A or B were treated at the Center for hemophilic disorders Fran

Foot asymmetry in healthy adults: elliptic fourier analysis of standardized footprints.

The size and shape of paired structures differ in the left and right sides of the body. Shape characteristics should be analyzed separately from size to supply information about the normal variations of human organs. In the present study, the within-subject normal symmetry of footprint shape and size was analyzed from a mathematical standpoint. On the standardized left and right footprints of 46 healthy adults (23 women and 23 men; 19-26 years old), the outline of each foot excluding the toes was identified and its shape was quantified independently from size by the elliptic Fourier analysis.

Stress fractures at the base of the second metatarsal in ballet dancers.

Stress fractures are a frequent injury in ballet companies and the most common location is at the base of the second metatarsal. While previous reports have focused on risk factors for this injury (overtraining, delayed menarche, poor nutrition), there is no published series describing the natural history and outcome following this fracture. We reviewed the office records of the senior author and identified 51 professional dancers (64 fractures) who sustained a stress fracture at the base of the second metatarsal. History of a previous stress fracture in the lower extremity was seen in 19 pati

Tibial torque generation in a flexed weight-bearing stance.

Internal and external torque generated about the long axis of the lower extremity was measured in 18 male subjects who were instructed to twist with maximal effort against a fixed footplate containing an instrumented torque cell. Mean torque values ranged from 30 to 71 newton meters (Nm) depending upon the test conditions. Torques recorded during the flexed single-leg stance were 19% to 49% higher than those measured while seated. Values at 45 degrees of knee flexion were 11% to 16% greater than those at 20 degrees. Torques generated while wearing a ski boot were 8% to 11% greater than those r

Patient Knowledge and Habits as Predictors of Foot Complications in Diabetes: A Cross-Sectional Study.

BACKGROUND: This cross-sectional study aimed to identify patients' awareness of predictors of diabetic foot complications (DFCs) based on 1) knowledge of hemoglobin A1c (HbA1c), 2) performance of fasting blood glucose (FBG) at home, and 3) type of footwear used and relationship with DFCs. METHODS: A total of 4,652 diabetic patients were seen between 2006 and 2021 at Dr. Khalid Edrees Specialized Medical Center. A questionnaire was completed by trained nurses at the patients' initial visit, collecting patients' knowledge of the HbA1c test, monitoring of FBG at home, and type of footwear used a

Feasibility of a Low-Intensity, Technology-Based Intervention for Increasing Physical Activity in Adults at Risk for a Diabetic Foot Ulcer: A Mixed-Methods Study.

BACKGROUND: Among adults with diabetes, 19-34% will develop a diabetic foot ulcer (DFU), which increases amputation risk and health care costs, and worsens quality of life. Regular physical activity, when increased gradually, may help prevent DFUs. In this mixed-methods study, we examined the feasibility of a low-intensity, technology-based behavioral intervention to increase activity in adults at risk for DFUs. METHOD: Participants at risk for a DFU (n = 12; 66% female; mean age = 59.9 years) received four in-person exercise and behavioral counseling sessions over 2-3 weeks, supplemented wit

Geographical distribution and prevalence of podoconiosis in Rwanda: a cross-sectional country-wide survey.

BACKGROUND: Podoconiosis is a type of tropical lymphoedema that causes massive swelling of the lower limbs. The disease is associated with both economic insecurity, due to long-term morbidity-related loss of productivity, and intense social stigma. Reliable and detailed data on the prevalence and distribution of podoconiosis are scarce. We aimed to fill this data gap by doing a nationwide community-based study to estimate the number of cases throughout Rwanda. METHODS: We did a population-based cross-sectional survey to determine the national prevalence of podoconiosis. A podoconiosis case wa

Work boot design affects the way workers walk: A systematic review of the literature.

Safety boots are compulsory in many occupations to protect the feet of workers from undesirable external stimuli, particularly in harsh work environments. The unique environmental conditions and varying tasks performed in different occupations necessitate a variety of boot designs to match each worker's occupational safety and functional requirements. Unfortunately, safety boots are often designed more for occupational safety at the expense of functionality and comfort. In fact, there is a paucity of published research investigating the influence that specific variations in work boot design ha

Biomechanical Response of Military Booted and Unbooted Foot-Ankle-Tibia from Vertical Loading.

A new anthropomorphic test device (ATD) is being developed by the US Army to be responsive to vertical loading during a vehicle underbody blast event. To obtain design parameters for the new ATD, a series of non-injurious tests were conducted to derive biofidelity response corridors for the foot-ankle complex under vertical loading. Isolated post mortem human surrogate (PMHS) lower leg specimens were tested with and without military boot and in different initial foot-ankle positions. Instrumentation included a six-axis load cell at the proximal end, three-axis accelerometers at proximal and di

Hallux valgus.

Hallux valgus is a common foot problem whose cause and progression is multifactorial, complex, and poorly known. Hallux valgus shows a predilection toward women. It is a progressive disorder with no treatment known to slow or stop progression. Surgery is indicated in healthy individuals when nonoperative measures fail. Adverse effects of surgery include infection and recurrence. Many procedures have been described, including soft tissue and bony reconstruction of the first ray. The procedure that is indicated depends on the severity of the deformity.

Dynamic impression insole in rheumatoid foot with metatarsal pain.

BACKGROUND: Custom molded insoles with metatarsal supports are used to redistribute excessive loading under the metatarsal heads in patients with metatarsalgia. However, these pressure reductions are usually insufficient for the rheumatoid foot with painful deformed metatarsal heads. We developed an effective insole made by sequential foam padding under successive walking impression. METHODS: Seventeen consecutive rheumatoid arthritic outpatients with metatarsal pain participated in this repeated measures study of 7-mm flat Ethylene Vinyl Acetate, custom molded and dynamic impression insoles.

Off-loading the diabetic foot for ulcer prevention and healing.

BACKGROUND: Retrospective and prospective studies have shown that elevated plantar pressure is a causative factor in the development of many plantar ulcers in diabetic patients and that ulceration is often a precursor of lower extremity amputation. In this article, we review the evidence that relieving areas of elevated plantar pressure (off-loading) can prevent and heal plantar ulceration. RESULTS: There is no consensus in the literature concerning the role of off-loading through footwear in primary or secondary prevention of ulcers. This is likely due to the wide diversity of intervention a

[Early functional treatment of surgically managed Achilles tendon rupture].

40 acute traumatic ruptures of the Achilles tendon were evaluated in a prospective study after open repair using an early functional rehabilitation protocol. The age of the patients averaged 43 (23-64) years. The operation consisted of a Kessler suture and microadaptation of the tendon. A special boot was used for after treatment. The boot was used 9 (1-18) weeks and full weight bearing was reported after 7 (1-16) weeks. The sick leave time averaged 5.5 (0-18) weeks. After 1 year, length and ankle joint motion were restored. Outcome was assessed using a 100-point score described by Thermann. A

Common adolescent dance injuries.

Dancers are an interesting and exciting group of patients. An understanding of the pathomechanics of their injuries coupled with their genuine interest in a prompt recovery teaches much about the diagnosis, treatment, and prognosis of similar lower extremity disorders occurring in the population at large. The majority of dance injuries occur in the lower extremity, commonly in the foot and ankle. Conservative therapy is usually successful; surgical intervention is only rarely indicated. Rehabilitation following all injuries is important as well as attention to proper alignment and technique.
