[Surgical complications following endoscopic diagnostic and therapeutic manipulations of the papilla Vateri].

The indications for retrograde cholangiopancreatography and for endoscopic papillotomy from the surgeon's view point are discussed. Endoscopic papillotomy is an operation of insufficient volume in chronic pancreatitis with obstacles along the Wirsungian's duct and in bile duct calculosis in younger age. These cases require surgical intervention or its association with endoscopic manipulations. Summarized data are reported on the risk after diagnostic or therapeutic procedures: complications may develop in 1 to 3.6 per cent after retrograde cholangiopancreatography and in 4 to 10 per cent after

Pulsed Radiofrequency of the Sacral Dorsal Root Ganglia as a Potential Rescue Therapy for Refractory Chronic Pelvic Pain: A Case Series.

BACKGROUND Chronic pelvic pain (CPP) is a prevalent, multifactorial condition with neuropathic, visceral, myofascial, and psychogenic components; a subset of patients remain refractory to multimodal pharmacotherapy and conventional interventional procedures. Pulsed radiofrequency (PRF) of the dorsal root ganglia (DRG) is a minimally invasive neuromodulatory technique that can attenuate pathological nociceptive transmission while preserving neural integrity. We describe 3 patients with severe, treatment-refractory CPP of predominantly neuropathic origin who underwent sacral DRG PRF as rescue th

Traumatic bilothorax following right thoracoabdominal gunshot injury: a case report of stepwise endoscopic and surgical management.

INTRODUCTION: Bilothorax, defined as the presence of bile in the pleural space, is an exceptionally rare cause of exudative effusion, usually related to hepatobiliary surgery or interventions. Post-traumatic bilothorax after gunshot injury is particularly uncommon and may be overlooked, leading to preventable pleuropulmonary morbidity. CASE PRESENTATION: We report a 33-year-old man who sustained a right thoracoabdominal gunshot wound with lung, diaphragmatic, and hepatic injuries requiring emergency thoracotomy, diaphragmatic repair, and perihepatic packing. After initial stabilisation and dra

Cholangitis, Choledocholithiasis and a Cholecystocolonic Fistula: A Case Report on a Rare Triple Surgical Challenge.

Cholecystocolonic fistula (CCF) is an uncommon complication of gallstone disease and is usually diagnosed intraoperatively. Concomitant choledocholithiasis and acute cholangitis create competing priorities: urgent biliary drainage and definitive treatment of the fistula. A 78-year-old woman presented with 3 days of upper abdominal pain, fever, vomiting and jaundice. She was febrile and hypotensive but stabilised after intravenous fluid resuscitation without vasopressors. Laboratory and imaging findings met Tokyo Guidelines 2018 criteria for severe acute cholangitis. Computed tomography demonst

Fatty Pancreas and the Association Between Pancreatic Manipulation Burden and Post-ERCP Pancreatitis.

BACKGROUND: Fatty pancreas has been associated with pancreatitis after ERCP, but its interaction with procedure-related pancreatic injury is unknown. AIMS: We evaluated whether fatty pancreas modified the association between cumulative pancreatic manipulation and postprocedural pancreatitis. METHODS: This multicenter retrospective cohort included 679 patients with native papillae undergoing biliary endoscopic retrograde cholangiopancreatography. Fatty pancreas was defined as a pancreas-to-spleen attenuation ratio < 0.7 on preprocedural noncontrast computed tomography. A pancreatic manipulati

[Acute pancreatitis: Progress through Restraint?].

Acute pancreatitis (AP) is a potentially life-threatening disease, often progressing in 2 phases: initial sterile inflammation, followed by later infected necrosis. Advances in care have shifted management toward a minimally invasive, step-up approach. AP is diagnosed based on typical abdominal pain, elevated lipase, or characteristic imaging - amylase is no longer essential. In hypertriglyceridemic AP, plasmapheresis offers no proven benefit. (Endo)sonography is mandatory. Contrast-enhanced CT should be delayed unless necrosis is suspected or diagnosis remains uncertain; optimal timing is ≥72

Combination use of intravenous ketamine-midazolam as a sedative agent in endoscopic retrograde cholangiopancreatography: a randomized control trial.

Endoscopic retrograde cholangiopancreatography (ERCP) is an invasive endoscopic procedure that requires deep sedation of the patient. Propofol is the most widely used sedative for advanced endoscopic procedures. However, its use is constrained by high cost and the need for administration by an anaesthetist. Our study aims evaluate the efficacy of ketamine-midazolam as a sedative agent during ERCP, assessing the depth of sedation. This prospective single-blinded randomized control trial (RCT) including patients undergoing ERCP. Patients were randomly assigned to either ketamine-midazolam or mid

Effectiveness of prophylactic pancreatic stents in preventing post-endoscopic retrograde cholangiopancreatography pancreatitis in high-risk patients: a 16-year comprehensive study.

BACKGROUND: Cannulation of the common bile duct (CBD) during endoscopic retrograde cholangiopancreatography (ERCP) can be technically challenging, especially when repeated unintended pancreatic duct cannulation occurs. We evaluated the effectiveness of prophylactic pancreatic stent (PS) placement in preventing post-ERCP pancreatitis (PEP) under such conditions. This is the first comprehensive study of its kind conducted in Greece, and one of the few in Europe. METHODS: This retrospective study included patients who underwent their first ERCP between January 1, 2008, and March 1, 2024, and rec

Clinical Course of Isolated Recurrent, Persistent Complex Perianal Fistulas Without Luminal Crohn's Disease: A Multicenter Case Series of 24 Patients.

BACKGROUND: Isolated complex perianal fistulas, without luminal evidence of inflammatory bowel disease in the gastrointestinal tract, pose diagnostic and treatment dilemmas for gastroenterologists and colorectal surgeons. For patients who develop recurrent complex fistulas, a presumptive diagnosis of Crohn's disease may be made. It is unclear whether these cases of isolated perianal disease in the absence of luminal inflammation truly represent isolated severe cryptoglandular fistulas or rather an early presentation of Crohn's disease. We aimed to investigate the clinical course and outcomes o

Comparing Stone Recurrence Following Surgical Common Bile Duct Exploration or Endoscopic Stone Extraction for Patients with Common Bile Duct Stones.

Background: Laparoscopic common bile duct exploration (LCBDE) and endoscopic retrograde cholangiopancreatography (ERCP) are two methods of retrieving common bile duct (CBD) stones. Aspects of CBD stone management, such as sphincterotomy, have been implicated as risks for CBD stone recurrence although evidence is weak. The aim of this study was to compare stone recurrence following LCBDE and/or ERCP. Methods: Data were collected retrospectively for patients undergoing LCBDE and/or ERCP for CBD stones at a single center from 2008 to 2018. Primary outcome was stone recurrence (>6 months after duc
