12-month longitudinal cognitive functioning in patients recently diagnosed with bipolar disorder.

OBJECTIVES: Although cognitive deficits are observed in the early stages of bipolar disorder, the longitudinal course of neuropsychological functioning during this period is unknown. Such knowledge could provide etiologic clues into the cognitive deficits associated with the illness, and could inform early treatment interventions. The purpose of the present study was to evaluate cognitive change in bipolar disorder in the first year after the initial manic episode. METHODS: From an initial pool of 65 newly diagnosed patients with bipolar disorder (within three months of the end of the first m

Neonatal hypoxia, hippocampal atrophy, and memory impairment: evidence of a causal sequence.

Neonates treated for acute respiratory failure experience episodes of hypoxia. The hippocampus, a structure essential for memory, is particularly vulnerable to such insults. Hence, some neonates undergoing treatment for acute respiratory failure might sustain bilateral hippocampal pathology early in life and memory problems later in childhood. We investigated this possibility in a cohort of 40 children who had been treated neonatally for acute respiratory failure but were free of overt neurological impairment. The cohort had mean hippocampal volumes (HVs) significantly below normal control val

A randomized trial to examine the effect of mifepristone on neuropsychological performance and mood in patients with bipolar depression.

BACKGROUND: Deficits in neuropsychological performance are found in patients with bipolar disorder and represent a potential treatment target for novel therapeutic strategies. We have previously demonstrated a beneficial effect on spatial working memory (SWM) of treatment for 1 week with the progesterone and glucocorticoid receptor antagonist mifepristone, evident 2 weeks after the cessation of treatment. METHODS: We examined the longer-term efficacy of 600 mg/day of mifepristone as an adjunctive treatment, for 1 week, in a placebo-controlled, randomized, double-blind trial in 60 patients wit

Effects of preoccupation on interpersonal recall: a pilot study.

BACKGROUND: The aim of this pilot study was to examine whether priming preoccupation (rumination) in healthy participants adversely affects the processing of interpersonal information. METHODS: Sixty female undergraduates with moderate or marked preoccupation proneness (selected on the basis of their high preoccupation on eating, shape, and weight issues) were randomized to receive either a general preoccupation prime, a standardized preoccupation prime, or a control prime. Following the prime, participants watched an 8-min videotape of a family interaction and then were asked free recall que

Executive functioning in offspring at risk for depression and anxiety.

BACKGROUND: Executive functioning deficits (EFDs) have been found in adults with major depression and some anxiety disorders, yet it is unknown whether these deficits predate onset of disorder, or whether they reflect acute symptoms. Studies of at-risk offspring can shed light on this question by examining whether EFDs characterize children at high risk for depression and anxiety who are not yet symptomatic. METHODS: This study examined neuropsychological functioning in a sample of 147 children, ages 6-17 years (M age=9.16, SD=1.82), of parents with major depression (MDD) and/or panic disorde

Prefrontal cortex dysfunction during cognitive tests evidenced by functional near-infrared spectroscopy.

Prefrontal cortex oxygenation changes in response to a verbal fluency task and a visual spatial working memory task were investigated by functional near-infrared spectroscopy (fNIRS) on nine patients with schizophrenia. Four patients were reactive to both tests; five reactive to only one of the two tests, suggesting the importance of reporting fNIRS results for each individual.

Interpreting intelligence test results for children with disabilities: is global intelligence relevant?

School psychological and neuropsychological evaluations typically include intellectual and other standardized assessment tools in the identification of children with disabilities. The clinical utility of intellectual assessment in the identification and treatment of these children has been repeatedly challenged, with alternatives such as a response to intervention or global intelligence score interpretation offered to replace the long-held tradition of idiographic interpretation of intellectual factors or subtests for the purpose of differential diagnosis and individualized intervention. Repli

Use of context in pragmatic language comprehension by children with Asperger syndrome or high-functioning autism.

Utilizing relevance theory, this study investigated the ability of children with Asperger syndrome (AS) and high-functioning autism (HFA) to use context when answering questions and when giving explanations for their correct answers. Three groups participated in this study: younger AS/HFA group (age 7-9, n=16), older AS/HFA group (age 10-12, n=23) and a normally functioning control group (age 7-9, n=23). The results indicated that the younger AS/HFA group did less well when answering contextually demanding questions compared to the control group, and the performance of the older AS/HFA group f

Brain mechanisms for reading in children with and without dyslexia: a review of studies of normal development and plasticity.

In this article we review our experience with the application of magnetic source imaging (MSI), the newest of the functional imaging methods, to the study of brain mechanisms for reading among children who read normally and among those with dyslexia. After giving a general description of MSI, we present evidence for reliable and valid maps of the brain mechanism for aural language comprehension as well as for reading. Next, we present data from 39 normal readers, 40 children with dyslexia, and 30 younger children at risk for developing a reading disability. These data show different brain acti

Characterizing the neural mechanisms of skill learning and repetition priming: evidence from mirror reading.

The changes in brain activity related to skill learning and repetition priming in a mirror-reading task were examined using functional MRI. Subjects exhibited significant learning across five training sessions and this learning generalized significantly to different spatial transformations (inverted-mirror reversed text and normal letters spelled backwards). Mirror reading, compared with reading normal text, was associated with extensive activation in occipital, temporal, parietal and frontal regions. Learning to read mirror-reversed (MR) text was associated with increased activation in left i

Psychiatric Characteristics of Students Who Make Threats Toward Others at School.

OBJECTIVE: While studies have focused on identifying potential school shooters, little is known about the mental health and other characteristics of students who make threats. This study aimed to describe these students and factors prompting psychiatric interventions and treatment recommendations. METHOD: Child and adolescent psychiatry threat assessment evaluations of 157 consecutive school-referred youths in grades K-12 between 1998 and 2019 were reviewed for demographics, reasons for referral, nature of threat, psychiatric diagnosis, and psychiatric and educational recommendations. Neuropsychological function in Tourette syndrome.

The accumulated body of scientific evidence regarding intellectual function, presence of learning disorders, and specific neuropsychological deficits in TS suggests that difficulties in these areas are present in a significant percentage of patients with TS. Despite the numerous methodological shortcomings of past neuropsychological studies of TS, relatively robust and consistent findings have emerged. The literature to date has suggested that intellectual ability is normally distributed in TS. Whether or not individuals with TS have significant discrepancies between their verbal and nonverbal
