Pathogenesis of bleeding colonic diverticulosis: new concepts.

Diverticulosis of the colon is a common disease of the older population which develops in those people who eat highly refined carbohydrate diets free of roughage. High intraluminal pressures are created in the resultant narrowed colon which tends to push the mucosa between circular muscle bundles through oblique connective tissue clefts containing the vasa recta. Massive bleeding of the colon is most commonly due to right-sided diverticulosis. It can be explained by chronic injury to the vasa recta which lie within the submucosa of the diverticulum from the apex to the antimesenteric orifice.

The role of procalcitonin in reducing antibiotics across the surgical pathway.

Procalcitonin (PCT) is widely considered as a highly sensitive biomarker of bacterial infection, offering general and emergency surgeons a key tool in the management of surgical infections. A multidisciplinary task force of experts met in Bologna, Italy, on April 4, 2019, to clarify the key issues in the use of PCT across the surgical pathway. The panelists presented the statements developed for each of the main questions regarding the use of PCT across the surgical pathway. An agreement on the statements was reached by the Delphi method, and this document reports the executive summary of the

Computed tomography of bowel obstruction: tricks of the trade.

Bowel obstruction (BO) is a frequent cause of hospitalization and surgical consultation for acute abdominal pain. It is usually suspected at physical examination, but clinical and laboratory data are often nonspecific. Thus, computed tomography plays a crucial role in a correct diagnosis of BO. Indeed, computed tomography can confirm a diagnosis of BO, and identify the location and cause of the obstruction. In this review, the computed tomography appearances of common and uncommon causes of BO and pseudo-obstruction are reviewed.

[Septic focus in the retroperitoneum--iatrogenic foreign body of cotton (gossypiboma)].

Retained surgical towels (gossypiboma) present a diagnsotic problem if they remain asymptomatic for many years. We report on one 51 year old patient with repeated septic temperatures in the early post-operative phase after hemicolectomy. The contrast enhanced CT of the abdomen revealed a well-defined round soft-tissue mass with a dense, enhanced wall containing an internal high-density wavy and striped area. At laparotomy a secondary infected surgical sponge with abscess formation was found.

Major acute inflammatory complications of diverticular disease of the colon: planning of surgical management.

BACKGROUND/AIMS: The aim of this study was to determine the most appropriate surgical strategy in the management of patients with major inflammatory complications of colonic diverticular disease. MATERIALS AND METHODS: Out of 259 patients affected by complicated diverticular disease of the colon, 43 consecutive patients (16.6%) who underwent urgent or emergency surgical intervention for diverticular perforation during a 20-year period (1975-1994) were retrospectively analyzed. According to the changes in the surgical approach over the time, the series was divided into two groups: 1975-1985 gr

[Surgical treatment of perforated diverticular sigmoiditis. A retrospective study apropos of 45 cases].

The authors report their experience with 45 cases of perforated diverticular sigmoiditis (10 cases of mesocolic abscesses, 13 cases of localised peritonitis and 22 cases of generalised peritonitis). The mean age of the patients was 69 years and a previous history of diverticulosis was found in 26.6% of patients. 18% were taking steroids or anti-inflammatories. The often atypical symptomatology only suggested sigmoid perforation in 50% of cases. Surgical procedures consisted of 33 immediate resections (group 1) and 12 conservative procedures (group 2). Overall mortality was 17.8%, and was 13% a
