A rehabilitation programme performed in darkness after anterior cruciate ligament reconstruction reduces the risk of secondary injuries.

PURPOSE: To evaluate whether integrating partial visual deprivation during the reathletization phase after anterior cruciate ligament (ACL) reconstruction improves proprioceptive recovery and reduces the risk of graft failure or contralateral injuries. METHODS: We conducted a retrospective, single-centre cohort study including patients aged older than 16 years who underwent ACL reconstruction with autograft and concomitant anterolateral ligament reconstruction between November 2017 and March 2020. Patients voluntarily chose between two reathletization programmes starting from the fourth posto

Ownership of an artificial limb induced by electrical brain stimulation.

Replacing the function of a missing or paralyzed limb with a prosthetic device that acts and feels like one's own limb is a major goal in applied neuroscience. Recent studies in nonhuman primates have shown that motor control and sensory feedback can be achieved by connecting sensors in a robotic arm to electrodes implanted in the brain. However, it remains unknown whether electrical brain stimulation can be used to create a sense of ownership of an artificial limb. In this study on two human subjects, we show that ownership of an artificial hand can be induced via the electrical stimulation o

Body ownership causes illusory self-attribution of speaking and influences subsequent real speaking.

When we carry out an act, we typically attribute the action to ourselves, the sense of agency. Explanations for agency include conscious prior intention to act, followed by observation of the sensory consequences; brain activity that involves the feed-forward prediction of the consequences combined with rapid inverse motor prediction to fine-tune the action in real time; priming where there is, e.g., a prior command to perform the act; a cause (the intention to act) preceding the effect (the results of the action); and common-sense rules of attribution of physical causality satisfied. We descr

Dizziness in the elderly.

Vertigo, unsteadiness, and other balance-related symptoms are common among older adults. These complaints should be taken seriously because they can lead to falls, injuries, loss of independence, and even death. This article provides a review of the underlying causes for the increased prevalence of dizziness with age, and discusses how specific test procedures may need to be modified for older individuals. Issues related to the management of these symptoms in the aging population are also considered.

Audiovestibular testing in patients with Meniere's disease.

In this article, the present state of the art with respect to audiovestibular testing for Meniere's disease (MD) is reviewed. There is no gold standard for MD diagnosis, and the classic dictum is that even the "best" tests yield positive results in only two-thirds of patients with MD. Still, we advocate the use and further investigation of advanced audiovestibular testing in patients with MD in an attempt to answer the questions that confront any clinician who cares for patients with audiovestibular symptoms.

Differences in physical characteristics and response to rehabilitation for patients with hand dystonia: musicians' cramp compared to writers' cramp.

STUDY DESIGN: Pre-Post, Mixed Factorial Trial. INTRODUCTION: Focal hand dystonia is a challenging movement disorder to rehabilitate in musicians and writers. PURPOSE OF THE STUDY: To compare the neuromusculoskeletal characteristics of those with writers' cramp (WC) and musicians' cramp (MC), and evaluate responsiveness to learning-based sensorimotor training. METHODS: Twenty-seven individuals (14 musicians, 13 writers) participated in 8 weeks of supervised therapy supplemented with a home program. Between-group differences on measures of musculoskeletal (physical), sensory, and motor perfor

Backward walking in Parkinson's disease.

We walk backward on a daily basis, such as when backing away from the kitchen sink or stepping back from a curb as a swiftly moving bus passes. This task may be particularly difficult for individuals with Parkinson's disease (PD) who often fall as a result of moving or being perturbed in the backward direction. The aim of this study was to assess backward walking (BW) in individuals with PD. Both forward walking (FW) and BW were assessed in 78 people with idiopathic PD (H&Y range: 0.5-3) in the ON state, and 74 age- and sex-matched controls. In FW, those with PD had significantly shorter strid

The peripheral nervous system and the perception of verticality.

Orientation of the body with respect to gravity is based on integration of visual, vestibular and somatosensory signals. Here, we investigated the subjective postural vertical (SPV) and visual vertical (SVV) in three patients with bilateral somatosensory deafferentation and a group of age-matched normal subjects. Our hypothesis was that the patients with bilateral somatosensory deafferentation may show tilt induced bias in the construction of their SPV, with a normal SVV. Patient 1 had a severe Guillain Barré syndrome and almost complete absence of peripheral sensation, the two other patients

A comparison of psychometric properties of the smart balance master system and the postural assessment scale for stroke in people who have had mild stroke.

OBJECTIVE: To compare the psychometric properties (including the test-retest reliability, responsiveness, and predictive validity) of the Smart Balance Master (SBM) system and the Postural Assessment Scale for Stroke patients (PASS) in patients with mild stroke. DESIGN: One repeated-measures design (at a 2-wk interval) was used to examine the test-retest reliability of the SBM and PASS, and another similar design was applied to investigate their responsiveness. Patients who participated in the responsiveness study were followed up approximately 1 year later, and the predictive validity of the

Cervicocephalic kinesthetic sensibility in young and middle-aged adults with or without a history of mild neck pain.

Previous research has shown that there is no significant relationship between the degree of structural degeneration of the cervical spine and neck pain. We therefore sought to investigate the potential role of sensory dysfunction in chronic neck pain. Cervicocephalic kinesthetic sensibility, expressed by how accurately an individual can reposition the head, was studied in three groups of individuals, a control group of 20 asymptomatic young adults and two groups of middle-aged adults (20 subjects in each group) with or without a history of mild neck pain. An ultrasound-based three-dimensional

Adaptations and deficits in the vestibulo-ocular reflex after peripheral ocular motor palsies.

Palsy of a nerve might be expected to lower vestibulo-ocular reflex (VOR) responses in its fields of motion, but effects of peripheral neuromuscular disease were unknown. We recorded the VOR during sinusoidal head rotations in yaw, pitch, and roll at 0.5-2 Hz and static torsional gain in 43 patients with unilateral nerve palsies. Sixth nerve palsy (n = 21) reduced both abduction and adduction VOR gains in darkness. In light, horizontal visually enhanced VOR (VVOR) gains were normal in moderate and mild palsy. In severe palsy, horizontal VVOR gains remained low in the paretic eye when it was fi

Midbrain projecting dorsal column nucleus neurons in a reptile.

The origin of midbrain projecting cells in the dorsal column nucleus (DCN) was investigated in reptiles, Caiman crocodilus, using a retrograde tracer. Labeled neurons were confined to a caudal-central portion of the DCN. Midbrain projecting DCN neurons had round, oval, or triangular soma and were small. While neurons that project to the spinal cord in Caiman are also located in the caudal half of the DCN, midbrain projecting cells are located more dorsally and are smaller than those whose axons terminate in the spinal cord. Taken together, these observations suggest that the DCN in Caiman is s

Eye movements during torso rotations in labyrinthine-defective subjects.

The aim of this study was to examine whether the chronic loss of vestibular function modifies perceptual and oculomotor responses during torso rotations in darkness. Subjects (4 patients with complete vestibular loss and 7 healthy volunteers) were seated on a rotating chair. Stimuli consisted of sinusoidal chair rotations (+/-30 degrees, 0.1 Hz and 0.011 Hz). We used 2 conditions: space stationary head (neck stimulation) and space stationary head and shoulders (torso stimulation). Horizontal eye deviations and slow component of eye movements were analysed. The results showed that eye movements

A critical maturational period of reduced brain vulnerability to developmental injury. I. Behavioral studies in cats.

Groups of cats with resection of the neocortex of the left cerebral hemisphere at postnatal (P) ages (in days) 5-15 (P10), 30 (P30), 60 (P60), 90 (P90), 120 (P120), and in adulthood, were compared using a comprehensive battery of 16 neurobehavioral tests administered when they were at least 6 months post-lesion. For all behaviors, except 3 (including the paw contact placing reaction which never recovered), the performance was significantly better for the cats lesioned between P10 and P30 compared to cats lesioned at older ages. For 10 of the behaviors, the transition from age-at-lesion P30 to

Discrimination of patients with acoustic neuroma and peripheral vestibular lesions with human posture dynamics.

A group of normal subjects (n = 17) was compared with groups of patients with a diagnosis of vestibular neuritis (n = 18), and acoustic neuromas (n = 35). Fisher linear discriminant analysis was applied to distinguish clusters of parameters characteristic for each disease. Hence it was possible to distinguish the vestibular neuritis patients from the normal group with statistical significance (p < 0.01). Also the patients with an acoustic neuroma could be distinguished from the normal subjects with statistical significance (p < 0.05).

Gender differences in the balance of healthy elderly as demonstrated by dynamic posturography.

BACKGROUND: Prior studies indicate that older women fall more often than men although there is no evidence of gender-based balance differences. Using a force platform, we measured the effects of restricted sensory input and support surface movement to detect gender differences in balance. METHODS: Healthy, elderly community dwellers (N = 234, mean age = 76 +/- 5 years, 52% female) were administered the following perturbations on the balance platform: The platform and/or visual surround were fixed or tilted proportionally to the subject's sway with the eyes open or closed, forward or backward

Cerebellar ataxia and muscle spindle sensitivity.

1. The cerebellum has long been known to participate in movement control. One of the enduring theories of cerebellar function is that it "tunes" and coordinates sensorimotor traffic in other parts of the CNS. In particular, it has been implicated in the control of the sensitivity of muscle spindle stretch receptors through the fusimotor system. 2. The stretch sensitivity of spindle primary endings can be varied approximately over a 10-fold range by fusimotor efferent action. For many years it has been believed that cerebellar dysfunction is associated with reduced drive to the fusimotor system

Sensory symptoms in cranial dystonia: a potential role in the etiology?

Cranial dystonia is normally considered as a pure movement disorder. Sensory symptoms have not received much attention, but we found ill-defined pain, discomfort, distortion of sensory modalities, 'phantom' kinetic or postural sensations in the orofacial areas subsequently involved by the dyskinesia in all of 11 consecutive patients, preceding by weeks or months the motor syndrome. Physicians were often mislead, initially making diagnoses such as trigeminal neuralgia, dental problems, sicca syndrome, chronic conjunctivitis, glossitis or stomatitis. The patients reported that the orofacial move

Analogy between training for dancers and problems of adjustment to microgravity: an evaluation of the subjective vertical in dancers.

"Moderne dance" (as opposed to a more academic or classical dance form) uses techniques from kinesiology, anatomy and improvization which are adapted to a cultural, technological and political environment. The function of a choreographic system is to take and give a measure of the world. This includes, with the present tendency of the evolution of culture, a new "naturalism" which seeks the secrets of the body. Dance movements express in terms of space the dimension fo the infinite. It gives somehow the measure of a world within which everything is relative. Except for the speed of light, time

[Critical remarks on so-called cervicogenic vertigo].

A critical review of neurophysiological literature is given about the part played by the neck proprioceptors in the regulation of vestibular functions. In comparison with vestibular and optokinetic influences the neck proprioceptors are of secondary importance. A critical review of the publications by the supporters of a cervical genesis of vertigo leads to the conclusion that "cervical nystagmus" must be proven before the concept of neck-triggered vertigo can be accepted. In relation to all forms of vertigo, vertigo induced by cervical disorder represents a very low percentage only.
