Effectiveness of platelet-rich plasma therapy in promoting wound healing and shoulder function recovery after shoulder surgery.

This retrospective propensity score-matched cohort study examined whether Platelet-Rich Plasma (PRP) use was associated with improved wound healing and functional recovery after shoulder surgery. We analyzed records from Changsha Hospital of Traditional Chinese Medicine (January 2018-December 2023) and included 170 patients undergoing rotator cuff repair (n = 100), shoulder replacement (n = 40), or subacromial decompression (n = 30). Within each surgery type, patients receiving PRP (treatment group [TG]) were matched 1:1 to patients not receiving PRP (control group [CG]) using propensity score

Influence of statin use on progression of postoperative fatty infiltration in the arthroscopic rotator cuff repair.

PURPOSE: The purpose of this study was to determine the influence of hyperlipidemia and statin (hydroxyl-methylglutaryl-coenzyme-A reductase inhibitors) use on fatty infiltration (FI) of the rotator cuff muscle after arthroscopic rotator cuff repair (ARCR). The presence or absence of statin use and type of statins used (type 1 naturally derived statins and type 2 synthetic statins) were examined. METHODS: This was a retrospective review of 620 cases (620 shoulders) who underwent arthroscopic rotator cuff repair. Total cholesterol (TC), low-density lipoprotein (LDL), triglycerides (TG) levels,

Global Fatty Degeneration Index on Preoperative Magnetic Resonance Imaging Predicts Early Postoperative Shoulder Stiffness at 3 Months After Primary Rotator Cuff Repair.

PURPOSE: To determine whether the fatty infiltration of rotator cuff muscles, as measured by magnetic resonance imaging (MRI) preoperatively and assessed using the Goutallier fatty degeneration index (GFDI), can predict early postoperative shoulder stiffness (POSS) after rotator cuff repair (RCR). METHODS: This retrospective longitudinal cohort study included patients who underwent primary RCR, had available medical records, and underwent MRI before RCR between November 2012 and July 2022. Patients were excluded on the basis of the following criteria: (1) preoperative shoulder stiffness, (2)

Corticosteroids or platelet-rich plasma injections for rotator cuff tendinopathy: a randomized clinical trial study.

BACKGROUND: Studies evaluating the role of both corticosteroids and platelet-rich plasma (PRP) in the treatment of rotator cuff (RC) tendinopathies have been contradicting. We compared structural and clinical changes in RC muscles after corticosteroids and PRP injections. METHODS: This is a randomized double-blind clinical trial. All individuals with diagnosis of RC tendinitis during 2014-2017 were considered. Individuals were randomly allocated to either receive PRP or corticosteroids. Overall, 3cc of PRP was injected within the subacromial joint and another 3cc was injected at the site of t

Clinical Outcomes, Tendon Integrity, and Shoulder Strength After Revision Rotator Cuff Reconstruction: A Minimum 2 Years' Follow-up.

BACKGROUND: The retear rate after primary rotator cuff (RC) reconstruction is high and commonly leads to poorer clinical outcomes and shoulder function. In the case of primary failure, revision RC reconstruction (RCR) has become increasingly important to re-create RC integrity and improve outcomes. To date, clinical and structural outcomes after RCR have not been sufficiently investigated and described at midterm follow-up. Hypothesis/Purpose: The purpose was to evaluate the clinical and radiological outcomes after revision RCR. It was hypothesized that revision RCR significantly improves clin

Serial structural MRI evaluation of arthroscopy rotator cuff repair: does Sugaya's classification correlate with the postoperative clinical outcomes?

INTRODUCTION: Sugaya's classification is the most commonly used for postoperative evaluation of rotator cuff repairs. However, the correlation between this classification and clinical outcomes after supraspinatus tendon repair were not performed with serial MRI examinations in standardized time intervals. MATERIALS AND METHODS: This prospective case series involved 54 patients undergoing repair of the supraspinatus tendon tear. Magnetic resonance imaging (MRI, 1.5 T) was used to determine the Sugaya's classifications at 3, 6, and 12 months, and these data were correlated with the visual analo

Association between pre-operative magnetic resonance imaging and reparability of large and massive rotator cuff tears.

PURPOSE: It is recognized that a percentage of large and massive rotator cuff tears (RCTs) cannot be anatomically repaired. We hypothesized that factors identified on pre-operative MRI would be associated with rotator cuff reparability. METHODS: A single-surgeon retrospective study was performed on patients who had undergone either an anatomical or partial repair of a large or massive RCT. Pre-operative MRI images were evaluated by a fellowship-trained shoulder surgeon, blinded to the surgical outcome. Stump location, tear dimension in the coronal and sagittal plane, fatty infiltration, muscl

Does a positive neer impingement sign reflect rotator cuff contact with the acromion?

BACKGROUND: One possible cause of shoulder pain is rotator cuff contact with the superior glenoid (cuff-glenoid contact) with the arm in flexion, as occurs during a Neer impingement sign. It has been assumed that the pain with a Neer impingement sign on physical examination of the shoulder was secondary to the rotator cuff making contact with the anterior and lateral acromion. QUESTIONS/PURPOSES: We determined if the arm position where pain occurs with a Neer impingement sign would correlate with the position where the rotator cuff made contact with the superior glenoid, as determined by arth

Pullout strength of suture anchors: effect of mechanical properties of trabecular bone.

This study investigated the relationships between trabecular microstructure and elastic modulus, compressive strength, and suture anchor pullout strength. Twelve fresh-frozen humeri underwent mechanical testing followed by micro-computed tomography (microCT). Either compression testing of cylindrical bone samples or pullout testing using an Arthrex 5mm Corkscrew was performed in synthetic sawbone or at specific locations in the humerus such as the greater tuberosity, lesser tuberosity, and humeral head. Synthetic sawbone underwent identical mechanical testing and microCT analysis. Bone volume

Staged reverse shoulder arthroplasty with structural allograft-prosthesis composite and lower trapezius tendon transfer for massive proximal humeral bone loss following gunshot injury: a case report.

Massive proximal humeral bone loss with concomitant rotator cuff deficiency is a rare and complex reconstructive challenge, particularly after high-energy trauma. Reverse shoulder arthroplasty (RSA) with an allograft-prosthesis composite (APC) restores humeral length and soft-tissue attachment, while tendon transfer addresses residual rotational deficits. We report a 35-year-old male who presented two weeks after a gunshot injury with an 8-cm open wound and extensive proximal humeral bone loss. Following serial debridement and culture-directed antibiotics, staged reconstruction was undertaken.

Identifying the axillary nerve during shoulder surgery: an anatomic study using advanced imaging.

BACKGROUND: The axillary nerve (AXN) is one of the more commonly injured nerves during shoulder surgery. Prior anatomic studies of the AXN in adults were performed using cadaveric specimens with small sample sizes. Our research observes a larger cohort of magnetic resonance imaging (MRI) studies in order to gain a more representative sample of the course of the AXN and aid surgeons intraoperatively. METHODS: High-resolution 3T MRI studies performed at our institution from January 2010 to June 2019 were reviewed. Four blinded reviewers with musculoskeletal radiology or orthopedic surgery train
