Infections and infestations of the central nervous system in India. A review.

Based on extensive personal experiences during the past three decades, a review is given on infections and infestations of the central nervous system, which are found in India and in other tropical and subtropical areas. Besides the main disease such as tuberculosis, pyogenic infections and parasitic infestations, rare lesions are also discussed. The literature has been cited in extenso and more especially the contributions by Indian neurosurgeons and neurologists.

Clinical, laboratory, and MRI features differentiating tuberculous and pyogenic spondylodiscitis: a propensity score-matched analysis.

BACKGROUND: Differentiating tuberculous spondylodiscitis (TS) from pyogenic spondylodiscitis (PS) is challenging due to overlapping clinical and radiological features. Early and accurate diagnosis is crucial for appropriate management. This study aimed to identify clinical, laboratory, and MRI parameters that may assist in the early differentiation of TS from PS. METHODS: This retrospective study included 76 adult patients diagnosed with TS (n = 21) or PS (n = 55) between January 2023 and July 2025. Clinical, laboratory, and MRI findings were compared between groups. To reduce selection bias,

Endovascular management of an aortic injury in thoracic kyphosis surgery: a minimally invasive rescue for a life-threatening injury.

A patient in his early adolescence, who was treated for T5-T6 tubercular spondylodiscitis with an un-instrumented decompression, presented at 36 months post-index surgery, for post-laminectomy instability and kyphosis, after completing his requisite antitubercular treatment. He underwent thoracic posterior instrumented kyphosis correction and anterior reconstruction, with a T5-T6 partial corpectomy and corpectomy spacer placement, through a posterior midline incision. On the second postoperative day, he started complaining of pain on the left side of his chest, abdomen and left shoulder. On co

Single-stage closing-opening wedge osteotomy of spine to correct severe post-tubercular kyphotic deformities of the spine: a 3-year follow-up of 17 patients.

The correction of severe post-tubercular kyphosis (PTK) is complex and has the disadvantage of being multiple staged with a high morbidity. Here, we describe the procedure and results of closing-opening osteotomy for correction of PTK which shortens the posterior column and opens the anterior column appropriately to correct the deformity without altering the length of the spinal cord. Seventeen patients with PTK (10 males; 7 females) with an average age of 18.3 +/- 10.6 years (range 4-40 years) formed the study group. There were ten thoracolumbar, one lumbar and six thoracic deformities. The n

Vertebral osteomyelitis: long-term disability assessment and prognostic factors.

In the present study, we quantified the long-term sequelae of a series of patients diagnosed with vertebral osteomyelitis during the period 1990-2002 in Albacete (Spain), using two validated questionnaires of spinal dysfunction and also one pain and one global health assessment. It was possible to interview 69 (78%) patients diagnosed with vertebral osteomyelitis, and an additional 90 "normal" people were recruited as controls to establish normal values. We also carried out a multivariate analysis to identify independent risk factors. We found only a 33% rate of spinal disability, only 3% seve

Thoracic and lumbar tuberculous spondylitis treated by posterior debridement, graft placement, and instrumentation: a retrospective analysis in 19 cases.

OBJECT: Surgical treatment of thoracic and lumbar tuberculous spondylitis is controversial. An anterior approach is usually recommended. The aim of the present study was to assess the efficacy of posterior debridement and the placement of posterior instrumentation for the treatment of patients with thoracic and lumbar tuberculous spondylitis. METHODS: Nineteen patients with thoracic and lumbar tuberculous spondylitis underwent single-stage posterior decompression and debridement as well as the placement of posterior interbody grafts if necessary, instrumentation and posterior or posterolatera

[Clinical symptomatology, diagnostics, and treatment in patients with tuberculous meningoencephalitis].

BACKGROUND: Tuberculous meningoencephalitis (TBM) is still associated with a high mortality. The relative rareness of TBM in Western European countries and the accompanying heterogeneous and unspecific clinical symptoms often result in a delayed diagnosis. PATIENTS AND METHODS: We present six HIV-negative patients (age 37-72 years) with a laboratory-confirmed or clinically probable diagnosis of TBM. The diagnosis could be confirmed in three patients by culture of the cerebrospinal fluid (CSF), in one patient by positive tracheal aspirate culture. In the cases with probable TBM, the diagnosis

[Permanent mechanical ventilation at home via a tracheotomy in chronic respiratory insufficiency].

This study describes our experience from 1960 onwards of 222 patients suffering from terminal chronic respiratory failure; we report our results of domiciliary mechanical ventilation for 11-17 hours per day using tracheotomy. The method was easy to use at home at a reasonable cost and was far less than in a medical environment. The results were excellent both for length of survival and quality of life for all cases where respiratory failure was due to chest wall problems (neurological, muscular or restrictive syndromes due to chest deformity). The majority of cases were sequelae of polio, myop

Protection of rats from experimental allergic encephalomyelitis with antiserum to guinea-pig spinal cord.

Inbred AS rats were immunized with guinea-pig spinal cord. The severity of experimental allergic encephalomyelitis (EAE) was studied in groups treated with antiserum to guinea-pig spinal cord and control groups treated with antiserum to other guinea-pig tissue. Prolonged treatment throughout the experiment strikingly reduced the severity of the clinical disease. Early treatment for the first 8 days after immunization had a similar protective effect. Late treatment starting on the 10th day after immunization did not produce significant protection. Protection was produced by antiserum from both

Internal thoracic vessels used as pedicle graft for anastomosis with vascularized bone graft to reconstruct C7-T3 spinal defects: a new technique.

STUDY DESIGN: A report of 4 cases of primary bone tumors (3 cases) or infection (1 case) at the cervicothoracic junction treated with resection-reconstruction. OBJECTIVES: To document a new technique using the internal thoracic vessels as recipient vessels for reconstruction of the cervicothoracic spine with free vascularized fibula grafts. SUMMARY OF BACKGROUND DATA: The cervicothoracic junction is a difficult region in reconstructive spinal surgery. Although nonvascularized fibula grafts can be used to reconstruct the osseous defect, compared with free vascularized fibula grafts they are b

Posterior instrumentation using compressive laminar hooks and anterior interbody arthrodesis for the treatment of tuberculosis of the lower lumbar spine.

STUDY DESIGN: A retrospective review with clinical and radiologic assessment was conducted. OBJECTIVE: To demonstrate the feasibility of posterior short segment augmentation with lamina hook and anterior lumbar interbody arthrodesis in tuberculosis of the lumbar spine by evaluating clinical results, fusion status, and sagittal angle. SUMMARY OF BACKGROUND DATA: There are few articles in the literature describing circumferential treatment of lumbar tuberculosis. We have had significant experience with a uniform operative technique for the treatment of this relatively rare condition and report

Single vertebral compression and involvement of the posterior elements in tuberculous spondylitis: observation on MR imaging.

To examine the atypical presentation of tuberculous spondylitis we evaluated seven cases seen in our hospitals during a five-year period (1989-1994) with procedures including MR imaging and CT scan. Three of the seven cases showed collapse of the single vertebral bodies with relatively spared disk spaces. Posterior elements were also involved on MR imaging in three cases, but the involvement of adjacent disks and vertebral bodies militated against metastases.

Tuberculous spondylodiscitis following intravesical Bacille Calmette-Guérin therapy: A case report and systematic literature review.

BACKGROUND: Intravesical Bacille Calmette-Guérin (BCG) therapy is the standard treatment for non-muscle-invasive bladder cancer. Disseminated BCG infection occurs in fewer than 5% of patients, with skeletal involvement (i.e., tuberculous spondylitis) being reported in only 36 cases worldwide. Here, a 77-year-old male with a history of BCG therapy for non-muscle-invasive bladder cancer developed L1/L2 osteomyelitis treated with percutaneous endoscopic transforaminal discectomy, debridement, and anti-tuberculous therapy. Twelve months later, however, the patient required, posterior instrumentati

Isolated cryptococcal osteomyelitis of the sacrum in an immunocompetent patient: a case report and literature review.

BACKGROUND: Cryptococcus neoformans, an opportunistic fungal pathogen, seldom causes infection in immunocompetent people. Cryptococcal osteomyelitis is an uncommon condition in which Cryptococcus invades the bone. It usually occurs as part of a disseminated infection and rarely in isolation. The spine has been reported as the most common site of cryptococcal osteomyelitis; however, isolated case of sacrum involvement in immunocompetent patients has never been reported. CASE PRESENTATION: We report the case of a 37-year-old man without underlying disease who presented with progressive low back

Clinical Features and Diagnosis of Intramedullary Spinal Cord Abscess in Adults: A Systematic Review.

BACKGROUND AND OBJECTIVES: Intramedullary spinal cord abscess (ISCA) was described 200 years ago but remains poorly understood and is often mistaken for immune-mediated or neoplastic processes. We present a systematic review of ISCA in adults, describing the clinical presentation, diagnostic features, treatment strategies, and outcomes. METHODS: Database searches for intramedullary abscess were performed on April 15, 2019, and repeated on February 9, 2022, using PubMed and EMBASE with 2 unpublished cases also included. Publications were independently reviewed for inclusion by 2 authors follow

Isolated central nervous system histoplasmosis presenting with ischemic pontine stroke and meningitis in an immune-competent patient.

IMPORTANCE: Histoplasmosis, a systemic mycosis caused by the fungus Histoplasma capsulatum, primarily affects immune-suppressed patients and commonly involves the lung and rarely the central nervous system (CNS). Herein, we report a case of isolated CNS histoplasmosis presenting with pontine stroke and meningitis. OBSERVATIONS: A 35-year-old, white, immune-competent man was transferred from an outside facility with worsening dysarthria and confusion after having presented 4 weeks prior with dysarthria, gait ataxia, and bilateral upper extremity weakness. Brain magnetic resonance imaging revea

[Central nervous system tuberculosis with and without HIV infection--clinical, neuroimaging, and neuropathological study].

Clinical data, neuroimaging, and neuropathology of 17 patients with central nervous system tuberculosis were reported. Of this population, 12 were men, 5, women; ages ranged from 23 to 75 years (mean, 46.9). There were three HIV positive patients among them. More than a half of patients had disturbance of consciousness as initial symptom. Neurological signs were variable such as visual acuity loss, hemiparesis, paraparesis, cerebellar ataxia, and tremor, though disturbance of consciousness was the most frequent (36%). Neuroimaging (X-ray CT and MRI) revealed meningeal enhancement (53%), tuberc
